BACKGROUND: This study assessed outcomes after treatment of patients with capitellum fracture diagnosed >4 weeks after the trauma (delayed) who presented with stiff elbow.
METHODS: We reviewed 7 patients with stiff elbows after delayed diagnosis of capitellum fractures between February 2007 and February 2012. They were treated with arthrolysis by twin incisions, late open reduction and internal fixation, and a hinged external fixator. According to the Bryan-Morrey-McKee classification, 3 patients had type I capitellum fractures and 4 patients had type IV.
RESULTS: Mean follow-up was 28 month

BACKGROUND: With the improvements in wound healing through the use of intravenous prophylactic antibiotics and technical refinements, postoperative elbow infections have become less common but still occur in certain elective elbow surgeries. The objective of this study was to evaluate the safety and efficacy of prophylactic application of vancomycin into the operative site to reduce the incidence of infection after the open release of post-traumatic stiff elbows.
METHODS: A retrospective review of 272 such patients during a 4-year period was performed. In the control group (93 patients), simp

INTRODUCTION: This case report presents an exceptionally rare occurrence of bilateral Galeazzi fractures with dorsal displacement of both distal radius fragments and the distal ulna in a 39-year-old male following a high-impact motorcycle accident. This specific injury configuration, which has not been categorized within the Walsh system, was effectively managed through open reduction and internal fixation utilizing limited contact dynamic compression plates.
CASE REPORT: Fracture reduction was achieved using a bilateral Henry's approach, which resulted in spontaneous realignment of the dista

Subcutaneous anterior transposition of the ulnar nerve is a common surgical treatment for cubital tunnel syndrome. However, there are surgical failures associated with the new compressive sites at the edge of flexor carpi ulnaris (FCU) and resubluxation posterior to the medial epicondyle of the transposed nerve. To reduce the muscle volume at the edge of FCU, we approach the ulnar nerve by dividing the muscle belly of the FCU humeral heads. This procedure can reduce repeated traction forces on the transposed nerve at the edge of the FCU. To keep the transposed ulnar nerve anteriorly, we use a

The diagnosis of ischemic stroke in toddlers in ED is a challenge due to non-specific neurological symptoms and difficulties in conducting a detailed neurological examination in toddlers. Magnetic Resonance Imaging (MRI) requires patient sedation and the cooperation of several medical personnel. A 33-month-old male presented with the immobility of the left upper extremity after a fall from a child chair. A head computerized tomography scan revealed no obvious bleeding. An orthopedic surgeon, a neurosurgeon, and a pediatrician were consulted but could not provide a definitive diagnosis. The fol

BACKGROUND: For manual wheelchair users, overuse of the upper limbs can cause upper limb musculoskeletal disorders, which can lead to a loss of autonomy. The main objective of this study was to quantify the risk level of musculoskeletal disorders of different slope propulsions in manual wheelchair users using fuzzy logic.
METHODS: In total, 17 spinal cord injury participants were recruited. Each participant completed six passages on a motorized treadmill, the inclination of which varied between (0° to 4.8°). A motion capture system associated with instrumented wheels of a wheelchair was used.

The purpose of this research was to compare the frequency and magnitude of head impact events between Pee Wee and Bantam ice hockey players. Videos of Pee Wee and Bantam boys' ice hockey were analysed to determine the frequency and type of head impact events. The head impact events were then reconstructed in the laboratory using physical and finite element models to determine the magnitude of strain in the brain tissues. The results showed that Pee Wee boys experienced more head impacts from elbows and boards, while Bantam players had more head impacts to the glass. Pee Wee and Bantam players

OBJECTIVE: To analyse the anatomical variations of the median nerve motor branches in the elbow region.
METHODS: Twenty upper limbs of 10 adult male cadavers were prepared by intra-arterial injection of a solution of 10% glycerol and formaldehyde. All cadavers belonged to the institution anatomy laboratory.
RESULTS: We found a great variability within the distribution of median nerve branches leading to forearm muscles. Only three limbs (14%) presented the normal standard of innervation described in anatomy treatises. The pronator teres muscle (PTM), flexor carpi radialis (FCR), palmaris lon

The patient was a 53-year-old male. He showed steppage gait at the age of 11 and equinus foot at 13. He walked unaided with shoe-insoles to support his heels. Atrial fibrillation and cardiac hypertrophy were found in his 30s, and ventricular tachycardia (VT) was observed at the age of 48. Electrophysiological studies were performed, but VT was not sustained, symptomatic, or showed signs of infra-Hisian block, and a pacemaker was not indicated. At 53, he was introduced to a neurologist because of tetraplegia after the first episode of syncope. A spinal MR showed ossification of posterior longit

Osteochondritis dissecans (OCD) of the humeral capitellum most commonly affects young athletes engaged in sports that repetitively stress the elbow. It is characterized by localized injury of subchondral bone of the humeral capitellum. To determine the best treatment option for OCD in young athletes, it is important to differentiate between stable and unstable OCD lesions. Stable lesions can be treated with rest, whereas unstable lesions, as well stable lesions that do not respond to conservative therapy, may require a surgical approach. Magnetic resonance imaging is the diagnostic study of ch

Complex regional pain syndrome is a condition that usually affects the upper or lower extremities. The cause is not clearly understood. We report a case of a severe form of a rapidly progressive complex regional pain syndrome type I developing after a right shoulder injury managed with spinal cord stimulation (SCS). After failed conservative treatments, a rechargeable SCS system was implanted in the cervical spine. Allodynia and dystonia improved but the patient subsequently developed similar symptoms in lower right extremity followed by her lower left extremity. The patient became wheelchair

Reconstructive hand surgery improves the ability to meet the needs of daily life and the independence of patients who have lost their upper extremity function due to cervical spinal cord injury. Tendon transfer procedures provide the potential to restore key functions, such as elbow and wrist extension or hand grip control, ameliorate joint balance, reduce pain in spasticity and prevent joint contractures. The choice of the optimal donor muscle should be based on a thorough understanding of the biomechanical principles of the muscle-tendon unit. Intraoperative sarcomere length measurements all

Gangrene of the hand associated with acute upper extremity venous insufficiency has been seen in four limbs in three patients treated at Vanderbilt University Medical Center. All three patients had life-threatening illnesses associated with diminished tissue perfusion, hypercoagulability, and venous injury. One patient progressed to above-elbow amputation, but venous thrombectomy in one limb and thrombolytic therapy in two others were successful in preventing major tissue loss. All three patients eventually died from their underlying illness. Thirteen previously reported patients with "venous

Active metacarpophalangeal flexion with simultaneous active extension of interphalangeal joints places the fingers in the position in preparation for prehension. This volar flexion of the fingers is due to the action of their intrinsic muscles, and this movement, which is irreplaceable, has been used by the author for the last twenty years as a specific exploratory test of intrinsic finger muscles. It can be investigated as an emergency procedure in cases of ulnar nerve wounds at the elbow or injury to the upper forearm before suture, and clearly demonstrates the presence of an ulnar claw hand

BACKGROUND: Supracondylar humerus fractures (SCHF) in adolescents transitioning to skeletal maturity are less common, and literature on the management of complex cases with open reduction internal fixation (ORIF) is limited. The objective of this study was to retrospectively evaluate SCHFs treated with ORIF at a single tertiary pediatric center.
METHODS: Patients of ages 7-18 who received ORIF between 2015 and 2024 for SCHF with intercondylar extension (T-type; OTA/AO 13-E/4.2) or extraarticular SCHF (eSCHF; OTA/AO 13-M/3.1, 13/E/1.1) were included. Patients treated with percutaneous pinning

Chemical burns are often deep with difficult initial clinical evaluation, especially those due to alkalic agents, which have a strong penetrating power. They therefore require specialized care in a Burn Unit. Self-inflicted burns are infrequent but their management represents a real challenge. We report the case of a 47-year-old referred to our Burn Center for the evaluation of a self-inflicted corrosion with an alkalic agent (soda), injected at the crease of the left elbow. The patient, right handed, was a nurse and had notably a psychiatric history of depressive syndrome. We observed a deep,

This study reports the use of silicone prosthesis as a temporary spacer for comminuted fracture of the radial head. Twelve men and eight women with a mean age of 42 years (17-54) were operated on in an average of five days following a comminuted fracture of the radial head. Silicone radial head prosthesis was implanted with a mean duration of eight months (3-18) between implantation and removal. At a mean period of five years after initial surgery, the mean DASH score was 40 (24-82), the mean MEPS score was 86 (60-100) giving ten excellent results, five good and five means.
