Vascular lesions in intravascular drug abusers in Guilan, north of Iran.

Intravenous drug abuse is an increasing social and health problem. Repeated injuries to the veins, injection of some types of insoluble subs … le patients, pseudo-aneurysm was present in 23 (52%), venous thrombosis in eight (18%), necrotizing fasciitis in three (7%), vascular abscess in two, and arterio-venous fistula in one (5%). Most of the vascular lesions involved the groin or cubital fossa and presented as a pulsatile mass. They may benefit from early referral to a vascular surgery unit. Infe …

The evolving characteristics and care of necrotizing soft-tissue infections.

BACKGROUND: Necrotizing soft-tissue infections such as necrotizing fasciitis and Fournier's gangrene are a source of high morbidity and mortality. These difficult cases are increasingly being referred to burn centers for specialized wound and critical care issues. In this study, we examine our institution's recent experience with a large series of necrotizing soft-tissue infections. STUDY DESIGN: A retrospective chart review was performed of 65 consecutive patients over a 5-year period with necrotizing soft-tissue infections that required radical surgical debridement. RESULTS: Overall surviv…

Clinical application of porcine small intestinal submucosa in the management of infected or potentially contaminated abdominal defects.

The repair of abdominal wall defects in potentially contaminated or grossly infected fields presents a difficult clinical problem. Polypropy … ing of bacterial contamination. The early postoperative complication rate was 50%. One patient with fasciitis had degradation of the small intestinal submucosa and loss of the bioprosthesis within 7 days. Other early complications included seroma (n=2), ileus (n=1), and wound infection (n=8). No patient experienced chronic infection. Mean follow up was 15.7 …

A fluoroquinolone induces a novel mitogen-encoding bacteriophage in Streptococcus canis.

This study investigated whether the recently recognized emergence of canine streptococcal toxic shock syndrome (STSS) and necrotizing fasciitis (NF) might be partly attributed to the use of fluoroquinolones to treat Streptococcus canis infections in dogs. Both mitomycin and the fluoroquinolone enrofloxacin caused bacteriophage-induced lysis of S. canis strain 34, an isolate from a case of canine STSS and NF. Fluoroquinolone-evoked, bacteriophage-induced lysis occurred over a range of concentrations similar to those that would occur after treatment of dogs with these agents. To search for a pos…

Changing management trends in penetrating colon trauma.

PURPOSE: Recent prospective studies have recommended primary repair for all penetrating colon injuries. We evaluated our management trends given these recommendations and assessed our results of primary repair. METHODS: A retrospective review was conducted of 145 patients with penetrating colon injuries received between January 1, 1991, and December 31, 1997. The patients were characterized according to demographics and severity of injury. Morbidity was defined as failure of a primary repair, abscess, fistula, wound dehiscence, fasciitis, sepsis, organ failure, or coagulopathy. The periods 19…

Host variation in cytokine responses to superantigens determine the severity of invasive group A streptococcal infection.

Cytokines elicited by superantigens have been suggested to play a central role in severe systemic clinical manifestations of gram-positive sepsis. Here we provide evidence for a potent inflammatory cytokine response in acute invasive group A streptococcal infections, and show a direct correlation between the magnitude of this response and the severity of systemic manifestations of the disease. Severe invasive cases suffering from toxic shock and/or necrotizing fasciitis had significantly higher frequencies of IL-2-, IL-6-, and TNF-alpha-producing cells in their circulation as compared to non-s…

Is colostomy always necessary in the treatment of open pelvic fractures?

BACKGROUND: Wound management in open pelvic fractures has used fecal diversion, debridement, and closure by secondary intention to prevent p … lved the pubis anteriorly, iliac crest, or anterior thigh. Pelvic sepsis was defined as cellulitis, fasciitis, or infection of a pelvic hematoma. Diversion consisted of loop or end colostomy. RESULTS: Eighteen patients with open fractures were identified. Four died from closed head injury and blood loss. The remaining 14 were treated as follows. Five patie …

Streptococcal toxic shock-like syndrome.

BACKGROUND: After decades marked by a decrease in the incidence and severity of scarlet fever and streptococcal soft-tissue infections, inva … ent in streptococcal toxic shock-like syndrome and the high frequency of progression to necrotizing fasciitis, myositis, or even death are unprecedented. The type and extent of tissue injury, systemic toxicity, and multiorgan system failure associated with this syndrome appear to be mediated by streptococcal pyrogenic exotoxins via the induction of biologic …

Loop Taping for Heel Fat Pad Syndrome: A Crossover Pilot Randomized Controlled Trial.

OBJECTIVES: Heel fat pad syndrome (HFPS) is a common but under-investigated cause of plantar heel pain. No randomized controlled trials (RCTs) have evaluated targeted treatments. This study evaluated the immediate and short-term effects of novel loop taping on pain, function, and fat pad thickness in adults with HFPS. DESIGN: Two-period crossover pilot RCT. INTERVENTIONS: Participants completed two taping conditions (loop and control) in randomized order, separated by a 4-to-7-day washout, with each participant serving as their own control. OUTCOME MEASURES: The primary outcome was immediat

Ultrasound-Guided Percutaneous Fasciotomy and Debridement for Chronic Plantar Fasciopathy: Six-Year Patient-Reported Outcomes from a Retrospective Case Series.

BACKGROUND: Ultrasound-guided percutaneous fasciotomy and debridement (USPFD) is a minimally invasive treatment for patients with plantar fasciopathy refractory to conservative management. Previous studies have shown clinical improvement with USPFD, but pain relief and patient satisfaction have not been demonstrated beyond 2 years. METHODS: This retrospective case series included patients who underwent USPFD at a quaternary academic medical center between April 2013 and January 2018, with refractory plantar fasciopathy and at least 4 years' follow-up. Patient outcomes were assessed through el

Randomised control trial to compare the efficacy of traditional Thai massage and ultrasound therapy for treating plantar heel pain.

BACKGROUND: Massage is suggested to be an effective treatment for chronic plantar heel pain (PHP). There is, however, no scientific evidence to support this claim. In the present study Traditional Thai Massage (TTM) has been compared with Ultrasound therapy (US) for treating PHP. METHODS: Sixty PHP patients with a Myofascial Trigger Point (MTrP) present in the calf were randomly assigned to receive a 40-minute single treatment of either US or TTM. Pain Intensity (VAS), Pressure Pain Threshold (PPT), Ankle Dorsiflexion Range of Motion (DROM), and Foot Skin Temperature (FST), were measured befo

A Death From Necrotizing Wound Infection After Cesarean Section: Resurgence of Group A Streptococcal Puerperal Sepsis.

Necrotizing wound infections are potentially lethal complications of surgeries, including cesarean deliveries. A 32-year-old female with obesity and hidradenitis suppurativa (HS) underwent uncomplicated cesarean section. Four days later, she developed abdominal pain and imaging showed ascites; she was treated with antibiotics. She progressed to septic shock and expired the following day from cardiopulmonary arrest. Hospital blood cultures grew no microorganisms. Autopsy revealed soft tissue purulence around the incision and copious purulent ascites without evidence of tubo-ovarian abscess, ret

Patient's characteristics and outcomes in necrotising soft-tissue infections: results from a Scandinavian, multicentre, prospective cohort study.

PURPOSE: Necrotising soft-tissue infections (NSTI) are characterised by necrosis, fast progression, and high rates of morbidity and mortality, but our knowledge is primarily derived from small prospective studies and retrospective studies. METHODS: We performed an international, multicentre, prospective cohort study of adults with NSTI describing patient's characteristics and associations between baseline variables and microbiological findings, amputation, and 90-day mortality. RESULTS: We included 409 patients with NSTI; 402 were admitted to the ICU. Cardiovascular disease [169 patients (41

Transvaginal therapy of genuine stress incontinence.

Two minimally invasive techniques for treatment of genuine stress incontinence, a transvaginal retropubic urethropexy and a transvaginal sling, using Cooper's ligament as the anchoring structure, are reported along with the early results. These surgeries can be done easily in conjunction with vaginal reconstructive procedures. Twenty-seven women were operated on between October 1998 and September 1999. Seventeen women underwent the transvaginal retropubic urethropexy for genuine stress incontinence, whereas 10 women underwent the transvaginal sling for genuine stress incontinence with maximal

Necrotizing myofasciitis: an atypical cause of "acute abdomen" in an immunocompromised child.

We report the case of an immunocompromised 15-year-old boy who presented with symptoms mimicking an "acute abdomen" related to necrotizing myofasciitis of the anterior abdominal wall. CT demonstrated the abdominal wall process as the cause of the patient's symptoms and sonographically guided aspiration confirmed the diagnosis. Despite prompt diagnosis and aggressive surgical debridement, the infection continued to progress and the patient died within 24 h of presentation.

Closure of wounds in the upper extremity using a skin stretching device.

A skin stretching device (SSD) harnessing the viscoelastic properties of skin using incremental traction has been used in 20 patients with skin deficits in the upper extremity. Complications were few considering the extensive damage to the skin and included slight partial dehiscence, necrosis of skin edges, local infection and hypertrophic scars. All wounds healed without the need for further surgical procedures. Application of the SSD is simple and it can even be used at the bedside under local anaesthesia. It reduces the need for more complicated surgical procedures like grafts or flaps.
