A new proximal femoral nail antirotation design: Is it effective in preventing varus collapse and cut-out?

OBJECTIVES: This study aims to compare the mechanical features of the existing proximal femoral nail antirotation (PFNA) system and the new PFNA system that we designed using three-dimensional (3D) finite element analysis. MATERIALS AND METHODS: This experimental study was conducted between 2019 and 2020. We constructed two femur models with Arbeitsgemeinschaft für Osteosynthesefragen (AO) type A1 fractures using 3D computed tomography scans. The new and standard PFNA designs were inserted into the femur models and subsequently transferred to the program. We investigated the distribution of s

Modified Oblique Triangular Neurovascular Island Flap Technique to Prevent the Development of Hooked Nail Deformity.

Hooked fingernail deformity can develop after any type of fingertip amputation. A more proximal amputation is associated with a higher probability of developing hooked fingernails. Proximal fingertip amputations with very short remaining nail beds are recommended for revision amputation with nail bed ablation. This procedure eliminates the possibility that the patient may have a functional nail. When the nail matrix is still retained, an oblique triangular neurovascular island flap may preserve the nail and digit length. At our institution, the modified oblique triangular neurovascular island …

Nail-patella-like renal disease masquerading as Fabry disease on kidney biopsy: a case report.

BACKGROUND: Genetic changes in the LIM homeobox transcription factor 1 beta (LMX1B) have been associated with focal segmental glomerulosclerosis (FSGS) without the extra-renal or ultrastructural manifestations of Nail-patella syndrome (NPS) known as Nail-patella-like renal disease (NPLRD). Fabry disease (FD) is an X-linked lysosomal disease caused by the deficiency of alpha-galactosidase A. The classic form of the disease is characterized by acroparesthesia, angiokeratomas, cornea verticillata, hypertrophic cardiomyopathy, strokes, and chronic kidney disease. Podocyte myelin bodies on ultrastr

Active Er-laser drug delivery using drug-impregnated gel for treatment of nail diseases.

Active Er-laser drug delivery under the nail plate using a drug-impregnated gel containing liquid methylene blue clusters is demonstrated for the first time. The effect of the agar-agar concentration in the gel and the gel plate thickness on the number of Er:YLF-laser pulses required for formation of a through microhole in the gel and in the nail plate with subsequent active drug delivery is discussed. The influence of the laser pulse energy, the gel plate thickness, and the external pressure applied to the gel on the rate of delivery of methylene blue under the nail plate through a single mic

Dermal exposure to plasticizers in nail polishes: An alternative major exposure pathway of phosphorus-based compounds.

Phosphorus-based compounds are used as plasticizers in the manufacture of many products found in the indoor environment. Here we quantitatively investigated dermal exposure to phosphorus-based compounds contained in 45 nail polishes purchased in Japan. The alternative plasticizer triphenyl phosphate (TPhP) was detected in some samples of the nail polishes made in the USA (concentration, 1.1-1.8 wt%). The potential dermal exposure rates for TPhP, estimated using ConsExpo (version 5.0; Dutch National Institute for Public Health and the Environment), were in the range 200 (5%ile)-1700 (50%ile)-50

Extra-articular distal tibial fractures, is interlocking nailing an option? A prospective study of 147 cases.

PURPOSE: Distal tibia fractures comprise about 7%-10% of lower extremity trauma. Because of the peculiarity of the soft tissue and subcutaneous location of the bone there are many controversies in the ideal treatment of distal tibia fractures especially extra articular pilon fractures. Plating is fraught with complications of wound dehiscence and infection. There are limited studies which document outcomes in such cases using intramedullary interlocking nail. We intend to study the outcome and complications of extra articular distal tibial fractures treated with interlocking nailing. METHODS:

Interlocking nailing of femoral shaft fractures with an extremely narrow medullary canal is associated with iatrogenic fractures.

INTRODUCTION: Controversy exists regarding the use of reamed interlocking nailing in femoral shafts with extremely narrow medullary canals (diameter ≤ 9 mm). The aims of this study were to (1) investigate the association of age and sex on femoral canal diameter in patients with a simple femoral shaft fracture and (2) compare the outcomes and complications of interlocking nailing between wide and extremely narrow intramedullary canals. PATIENTS AND METHODS: For the purposes of this retrospective cohort study, consecutive patients with simple femoral shaft fractures were recruited between Janua

Unstable Ankle Fracture Fixation Using Locked Fibular Intramedullary Nail in High-Risk Patients.

Open reduction internal fixation is the gold standard for unstable ankle fracture fixation; however, complications in patients with multiple medical comorbidities are common. Intramedullary nail fixation of the fibula can help to mitigate these difficulties. A retrospective chart review was performed on all patients who underwent fixation for unstable ankle fracture between January 2015 and March 2016 at our level I trauma center. Comorbidities in the patient sample included were one or several of diabetes, renal disease, hypertension, advanced age with osteoporosis, hemorrhagic blisters, and

Brachyonychia Associated with Acroosteolysis in Chronic Kidney Disease: How Phalange Shape Influences Nail Morphology.

Brachyonychia is a rare manifestation in patients with chronic kidney disease. Longtime disease, secondary hyperparathyroidism, and hemodialysis are common conditions among those who present it. We evaluated 8 cases who presented brachyonychia in the nephrology department and compared the clinical versus the radiographic findings, and evaluated how the tissue adjusts to the underlying bone structure, giving different forms to the nails. We conclude that brachyonychia and acroosteolysis in chronic kidney disease suggest long-term disease, secondary hyperparathyroidism, and hemodialysis, besides…
