Traditional plain film evaluation of facial fractures includes a lateral view of the face. This projection is often not exploited to its full potential because the many overlapping shadows are perceived to detract from its usefulness. A unique case of pachydermoperiostosis, multiple epiphyseal dysplasia, and secondary osteoarthritis is described. An additional interesting feature was the presence of acroosteolysis of the distal phalanges of fingers and toes. Scintigraphic, thermographic, plethysmographic, and capillaroscopic studies suggested increased blood flow through clubbed fingers, raising the possibility that this may play a role in the pathogenesis of pachydermoperiostosis.

Chest radiographs of 70 patients with Legionnaires' disease were evaluated and compared to the authors' previous series of 24 patients as well as others in the literature. A larger number of patients in the more recent survey had pleural effusion, and there were 3 with cavitation whereas none was seen previously. Analysis of the radiograph showing the most marked changes in survivors versus patients who died showed no consistent pattern, indicating that the radiograph cannot be used as a prognostic indicator. To examine the reliability of clinical features as possible components of a clinical method of prognostic staging for patients with rectal cancer, the results obtained when two pairs of physicians interviewed and examined a consecutive series of 60 patients with unresected rectal cancer were compared. Immunopathologic evaluation of 27 dogs with pemphigus and pemphigoid demonstrated that direct immunofluorescence testing was vastly superior to indirect immunofluorescence testing for diagnosis. Therapeutic studies in 31 dogs with pemphigus and pemphigoid demonstrated that glucocorticoids given for systemic effect were unsatisfactory as the sole form of treatment because of lack of efficacy of unacceptable side effects, or both, in over 50% of the cases.

Indications and results are presented for nearly all the cardiac pacemaker implantations in children in The Netherlands from 1963 through mid-1979. Five infants and 71 older children received a pacemaker for a total of 184 patient years. Two of the infants died within a week of birth; four children died after the pacemaker failed to control a sick sinus syndrome, which in three of them developed after a Mustard operation for transposition of the great arteries. Renewed interest in the natural history of Ebstein's anomaly has been created by recent surgical innovations in its management. Tricuspid valve replacement and various types of plastic reconstruction are the two methods of treating Ebstein's anomaly. Replacement of the tricuspid valve has been more successful in older children and adults than in infants, but overall, less successful than procedures involving mitral or aortic valve prosthesis. Scleral tissue from two cases of nanophthalmos was examined by amino acid analysis, light microscopy, histochemistry, and transmission electron microscopy. Perifibrillar aggregates, similar to proteoglycans, were prominent in the nanophthalmic sclera. The sclerae were thicker than normal and the bundles of collagen fibrils were less ordered. The clinical features of vortex vein compression seem causally related to the disordered and thickened sclera, which, in turn, may be caused by dysfunctional proteoglycans, or interaction with the scleral collagen, or both.

Advanced abdominal pregnancy was encounter 10 times in 102,000 deliveries over a period of 10 years at 1 hospital. The clinical features, difficulties in diagnosis and management, and the outcome of this uncommon condition are discussed. The most frequent symptoms encountered in this series were abdominal pain (100%), nausea and vomiting (70%), general malaise (40%), and painful fetal movements (40%). A monoclonal gammopathy was observed in three patients with long-term and widespread scleredema (Buschke's disease). There was no evidence of multiple myeloma in any patient. Deposition of monoclonal immunoglobulins in the skin was not detected by direct immunofluorescence microscopy. The importance of AR for a prompt and upkeep therapy is underlined by the Authors on the base of a statistical and clinical study about 67 cases of thoracic trauma treated from January 1975 to November 1979. The different alteration of the chest wall and surrounding structures are taken into consideration. The possible association of thoracic trauma with head trauma, severe shock and bony traumatism are also considered from a statistical and from a clinical point of view. Clinical observations were made on 20 experimentally infected and 70 clinical cases of feline babesiosis. The experimental cats showed a remarkable ability to adapt to the disease. Lethargy, anorexia and anaemia were recorded in both groups. Icterus was only occasionally seen. Elevated body temperature was not a feature of the disease. All untreated animals eventually died. Blood smears revealed increased polychromatophils, Howell-Jolly bodies, nucleated erythrocytes and anisocytosis indicative of a regenerative anaemia. Erythrophagocytosis by monocytic type leucocytes was also observed.
