Predicting mortality risk following major lower extremity amputation using machine learning.

OBJECTIVE: Major lower extremity amputation for advanced vascular disease involves significant perioperative risks. Although outcome prediction tools could aid in clinical decision-making, they remain limited. To address this, we developed machine learning (ML) algorithms capable of predicting 1-year mortality following major lower extremity amputation. METHODS: The Vascular Quality Initiative (VQI) database was queried to identify patients who underwent major lower extremity amputation for non-traumatic and non-malignant causes between 2012 and 2024. A total of 75 features were collected fro

Personalized AI-Driven Real-Time Models to Predict Stress-Induced Blood Pressure Spikes Using Wearable Devices: Proposal for a Prospective Cohort Study.

BACKGROUND: Referred to as the "silent killer," elevated blood pressure (BP) often goes unnoticed due to the absence of apparent symptoms, resulting in cumulative harm over time. Chronic stress has been consistently linked to increased BP. Prior studies have found that elevated BP often arises due to a stressful lifestyle, although the effect of exact stressors varies drastically between individuals. The heterogeneous nature of both the stress and BP response to a multitude of lifestyle decisions can make it difficult if not impossible to pinpoint the most deleterious behaviors using the tradi

Enhancing a deep learning model for pulmonary nodule malignancy risk estimation in chest CT with uncertainty estimation.

OBJECTIVE: To investigate the effect of uncertainty estimation on the performance of a Deep Learning (DL) algorithm for estimating malignancy risk of pulmonary nodules. METHODS AND MATERIALS: In this retrospective study, we integrated an uncertainty estimation method into a previously developed DL algorithm for nodule malignancy risk estimation. Uncertainty thresholds were developed using CT data from the Danish Lung Cancer Screening Trial (DLCST), containing 883 nodules (65 malignant) collected between 2004 and 2010. We used thresholds on the 90th and 95th percentiles of the uncertainty scor

Dynamics of botnet propagation model in complex networks considering hybrid method for botnet detection.

In this paper, a dynamic epidemic model of botnet attack propagation in scale-free networks is introduced based on the epidemic model. The proposed attack propagation model is based on the Susceptible-Exposure-Infected-Improved-Vaccinated-Recovery (SEIRVS) epidemic model. Here, an Intrusion Detection System (IDS) for botnet attack detection is also presented. This method is based on a combination of machine learning and metaheuristic algorithms, the Golden Ratio Optimization (GRO) algorithm, Bat Algorithm (BA), and K-Nearest Neighbor (KNN) algorithms named (GRO-BA-K-NN), which includes three s

The ethics of blood stewardship and quantitative futility assessments to predict certain death in neurotrauma patients.

Universally accepted guidelines to predict futile resuscitation in severely bleeding trauma patients with traumatic brain injury do not exist. These patients may consume vast volumes of blood products in futile cases, which is especially problematic during times of local and national blood scarcity. However, determining which patients have no chance of survival is complicated and often reliant on the traumatologist's individual judgment, which may be inconsistent. Traumatologists often face the ethical dilemma of balancing their obligations to provide appropriate care for patients and to conse

Development of a machine learning-based model to predict urethral recurrence following radical cystectomy: a multicentre retrospective study and updated meta-analysis.

Urethral recurrence (UR) following radical cystectomy for bladder cancer represents an aggressive disease failure with typically poor survival outcomes. Our study aimed to assess the predictive risk factors for UR, to develop and validate an easy-to-use predictive tool of UR in our region based on machine learning algorithms and to update the meta-analysis of risk factors for UR in combination with our results to improve its accuracy and reliability. Clinicopathological data from the patients who underwent RC between 2010 and 2020 at multiple centres were collected. To confirm the independent

Performance and utility of a rapid diagnostic test for cholera: notes from Haiti.

The present study details work done at the National Public Health Laboratory in Haiti (LNSP), comparing the results of a cholera rapid diagnostic test (RDT) with culture-based methods. As of October 21, 2011, 644 specimens were tested by both RDT and culture-based method at the LNSP. The sensitivity and specificity of RDT were 95% and 80%, respectively, with a positive predictive value of 89% and negative predictive value of 91%. In resource-limited settings, the RDT has good utility and should be considered as part of the laboratory testing algorithm.

Streptococcal pharyngitis: diagnosis by gram stain.

Group A beta-hemolytic streptococci were isolated from 49 (10.4%) of 472 patients with pharyngitis. Throat culture results, interpreted by five observers of varying experience, showed the mean sensitivity, specificity, and predictive value of a positive Gram-stained smear of pharyngeal secretions as 73%, 96%, and 71%. Assignment to a high-risk group by clinical algorithm gave sensitivity, specificity, and predictive value of only 45%, 83%, and 23%. The Gram-stained smear is the most accurate method of early diagnosis of streptococcal pharyngitis.

Optimization of print parameters for batch and continuous manufacturing of three-dimensional (3D) printed dosage forms using artificial intelligence and machine learning.

Three-dimensional printing (3DP) holds significant potential for developing personalized pharmaceutical oral dosage forms (printlets). 3D printing has the advantage of fabricating complex geometric structures for versatile drug release profiles, enhancing patient preference, palatability and swallowability, reducing the pill burden, and increasing dose accuracy. Optimizing printing parameters is crucial in determining the quality of the printlets during dosage development. The integration of machine learning (ML) can reduce production costs and time through parameter optimization based on trai

Mapping Kansas City cardiomyopathy, Seattle Angina, and minnesota living with heart failure to the MacNew-7D in patients with heart disease.

INTRODUCTION: The Kansas City Cardiomyopathy Questionnaire (KCCQ), Seattle Angina Questionnaire (SAQ), and Minnesota Living with Heart Failure Questionnaire (MLHFQ) are widely used non-preference-based instruments that measure health-related quality of life (QOL) in people with heart disease. However, currently it is not possible to estimate quality-adjusted life-years (QALYs) for economic evaluation using these instruments as the summary scores produced are not preference-based. The MacNew-7D is a heart disease-specific preference-based instrument. This study provides different mapping algori
