Unexplained intraoperative cardiac tamponade during thoracoscopic lobectomy: a case report.

BACKGROUND: Cardiac tamponade is a life-threatening emergency characterized by reduced cardiac output and circulatory failure. It most commonly occurs during cardiac surgery, interventional cardiology procedures, pericardiocentesis, or central venous catheterization, as well as in the setting of acute myocardial infarction or aortic dissection. To the best of our knowledge, cardiac tamponade during video-assisted thoracoscopic surgery (VATS) lobectomy has not been reported and may has an insidious presentation, making diagnosis challenging in the absence of obvious pericardial injury. CASE PR

Obstructive sleep apnea is associated with a higher 28-day incidence of venous thromboembolism and stroke after total hip arthroplasty.

BACKGROUND: Obstructive sleep apnea (OSA) is a prevalent condition that is strongly associated with significant cardiovascular comorbidities and a predictor of poor outcomes, particularly in the postsurgical population. There are limited data for venous thromboembolism (VTE) in post-total hip arthroplasty (THA) patients and examining this association in an observational model can be challenging without a significant number of patients. METHODS: As part of this retrospective multicentric cohort study, we queried the TriNetX U.S. Collaborative Network, a deidentified, aggregated electronic heal

Beyond HbA1c: Multivariate Clustering and Propensity-Weighted Analysis Identify High-Risk Patients with Diabetes Undergoing Bariatric Surgery.

INTRODUCTION: Preoperative glycemic control is a key concern in bariatric surgery. However, the extent to which suboptimal HbA1c levels impact short-term postoperative morbidity remains uncertain. This study evaluated the association between preoperative HbA1c categories and 30-day postoperative morbidity in a large Middle Eastern bariatric surgery cohort, using propensity score weighting and unsupervised clustering to refine risk stratification. METHODS: This retrospective cohort analysis included 1778 bariatric surgery patients operated between 2019 and 2024 at a tertiary referral center in

Inhaled nitric oxide for reducing major adverse events requiring intensive life support in adults undergoing cardiac surgery with cardiopulmonary bypass: protocol for a phase III, double-blind, multicenter randomized controlled trial (NORISC Trial).

INTRODUCTION: Cardiac surgery remains a high-risk intervention, with postoperative complications, including ischemic events (myocardial infarction and stroke), acute kidney injury, respiratory failure, and neurocognitive dysfunction, affecting up to 30% of patients and resulting in increased mortality. While inhaled nitric oxide (NO) is traditionally used for pulmonary hypertension and right ventricular dysfunction, recent experimental and clinical studies suggest its organ-protective potential may mitigate ischemia-reperfusion injury and hemolysis induced toxicity. However, whether these phys

Tranexamic acid in high-risk shoulder arthroplasty patients: safety across thromboembolic, cardiac, renal, and neurologic risk profiles.

INTRODUCTION: Tranexamic acid (TXA) effectively reduces blood loss and transfusion requirements in shoulder arthroplasty. However, concerns regarding thromboembolic, neurologic, cardiac, and renal complications have limited its use in medically high-risk patients. This study evaluated temporal trends and the safety of TXA in high-risk patients undergoing total shoulder arthroplasty (TSA). METHODS: A retrospective cohort study was conducted using TriNetX, identifying patients who underwent shoulder arthroplasty (2012-2025). Patients were stratified by preexisting high-risk conditions, includin

Cardiac Assessment and Takotsubo-stunning among COPD-exacerbations in-Hospital (CATCH study): when the lungs break your heart-protocol for a prospective observational cohort study.

INTRODUCTION: Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) may cause stress-induced transient acute cardiac dysfunction through myocardial stunning, in the form of exacerbation-triggered Takotsubo syndrome (referred to as Takotsubo stunning). Although prior studies suggest an association between AECOPD and transient cardiac dysfunction, existing evidence is limited to retrospective cohorts, case reports and expert consensus. Therefore, the incidence and clinical impact of Takotsubo stunning with acute heart failure (AHF) during AECOPD remain unknown and may be overlooke
