Beyond a motor disorder: a prospective evaluation of sleep quality in cervical dystonia.

BACKGROUND: Little is known about sleep disturbances in cervical dystonia (CD), particularly the relationship to motoric symptoms. It is critical to clarify these points given the impact on quality of life. METHODS: Primary CD patients receiving botulinum toxin (BoNT) injections and age- and gender- matched healthy controls were included. In both groups, sleep quality and daytime sleepiness were assessed. In CD, these assessments were repeated following BoNT injections. CD severity, mood symptoms, and health impact of CD were also assessed. RESULTS: 54 CD patients and 55 controls were includ

The role of sleep in Juvenile idiopathic arthritis patients and their caregivers.

Juvenile idiopathic arthritis is a chronic disease that may lead to various consequences for patients and caregivers, especially in relation to sleep quality. Sleep is an essential process for homeostasis of the organism. In general, caregivers of children with JIA are more susceptible to these sleep disorders and a lower quality of life. This impairment in sleep can potentially affect the health of caregiver. For this reason, it is very important to continue to evaluate the quality of sleep in caregivers and how to support these JIA children's caregivers more effectively.

[Circadian rhythm and inflammatory bowel disease].

Inflammatory bowel disease (IBD), including ulcerative colitis (UC) and Crohn's disease (CD), is a chronic and recurrent inflammatory disorder of the intestinal tract. It has been demonstrated that sleep disturbances are involved in the pathogenesis of the patients with IBD. In addition, it has been shown that melatonin, which is a hormone produced by the pineal gland and maintains circadian rhythm, plays an important role as regulators of inflammation as well as a player in proper immune system and antioxidant system in the intestinal disorders. In this review, we present what is currently kn

Dementia caregivers' lived experience of sleep.

PURPOSE: Poor sleep quality leads to sleep deficiency, an unmet public health problem that is especially acute in caregivers. The purpose of this study was to investigate the dementia caregiver's lived experience of sleep. The specific objectives were to (1) explore causes for poor sleep as identified by the caregiver, (2) gain knowledge about how the caregiver manages the sleep experience, and (3) gain an understanding of how caregivers perceive health promotion suggestions to improve sleep quality. METHODS: This was a qualitative study using a phenomenological framework. In 3 focus groups,

Analysis of the latest Austroads guidelines for fitness to drive as promulgated in March 2012.

After 9 years, Austroads has published new guidelines for fitness to drive. The guidelines have a preamble, which includes a legal disclaimer denying any culpability for those who designed the guidelines. They also warn of the legal responsibility for health professionals to satisfy their obligations, the need to be current with both relevant medical and legal expectations and if in doubt to seek guidance from Medical Defence Organisations. The guidelines are divided into Parts A and B, with A providing broad overview and background information while B deals with specific entities, such as bla

Hypersomnolence and reduced activity in pan-leptin receptor knockout mice.

Excessive obesity correlates with hypersomnolence and impaired cognitive function, presumably induced by metabolic factors and cytokines. Production of the adipokine leptin correlates with the amount of adiposity, and leptin has been shown to promote sleep. To determine whether leptin plays a major role in the hypersomnolence of obesity, we measured sleep architecture in pan-leptin receptor knockout (POKO) mice that do not respond to leptin because of the production of a mutant, non-signaling receptor. The obese POKO mice had more non-rapid eye movement (NREM) sleep and less waking time than t

Successful treatment of widespread eczema and sleep disruption in a 7-month-old infant with methylprednisolone aceponate 0.1%.

A 7-month-old infant, with no known food allergy or intolerance, presenting with eczema lesions affecting many parts of his body, was diagnosed with infantile eczema. A skin barrier defect, due to filaggrin deficiency, was suspected. Prior treatment with emollients was not effective and produced discomfort. After twice-daily application of methylprednisolone aceponate (MPA) 0.1% (Advantan(®)), in addition to temporary withdrawal of emollients, for 10 days. At the follow-up examination, improvements in all eczema lesions, scratching behavior and sleep quality were observed, and eczema was consi

Impact of childhood parental abuse and neglect on sleep problems in old age.

OBJECTIVES: This study examined whether subjective sleep complaints in late adulthood were associated with childhood parental abuse and neglect and whether the association was mediated by current relationship strain and emotional distress. METHODS: Multiple regression analyses were performed on data from 877 older adults aged 60 years and above from the National Survey of Midlife Development in the United States. Adverse childhood experiences were retrospectively recalled at Time 1 (1995-1996). Sleep complaints, relationship strain, and emotional distress were assessed at Time 2 (2004-2006).

Quality of life in infants with atopic dermatitis and healthy infants: a follow-up from birth to 24 months.

AIM: To evaluate quality of life (QOL) in infants with atopic dermatitis (AD) by comparison with healthy infants to find treatment modalities contributing most to QOL in these patients. METHODS: The population (N=134) was recruited in the context of an on-going prospective mother-infant study. QOL was measured using the Infant Dermatitis Quality of Life Index for the infants with AD and a modification of this for the healthy infants. RESULTS: The index scores of infants with AD were 4.15 (SD 2.96), 3.89 (SD 3.62) and 3.23 (SD 2.71) at 6, 12 and 24 months, respectively, the QOL being signific

Isolated nocturnal auditory hallucinations: a case report.

Sleep-related hallucinations (hypnagogic and hypnopompic) are transient and occur in normal individuals. Rarely, they are prominent and distressing and are associated with diverse conditions including narcolepsy. We report a case of prominent hypnopompic auditory hallucinations--which gradually became continuous and distressing with characteristics of schizophrenia-like hallucinations--that responded to a low dose of olanzapine.

Characterizing sleep disorders of adults with tuberous sclerosis complex: a questionnaire-based study and review.

An adult cohort with tuberous sclerosis complex was investigated for the prevalence of sleep disturbances and the relationship with seizure variables, medication, and psychological functioning. Information on 35 adults was gathered using four questionnaires: Epworth Sleepiness Scale (ESS), Sleep and Epilepsy Questionnaire (SEQ), Sleep Diagnosis List (SDL), and Adult Self-Report Scale (ASR). In addition, clinical, genetic and electrophysiological data were collected. Of 35 respondents, 25 had a history of epilepsy. A subjective sleep disorder was found in 31% of the cohort. Insomnia scores show

Sleep patterns in Parkinson's disease patients with visual hallucinations.

Visual hallucinations (VHs) in Parkinson's disease (PD) can be a frequent and disturbing complication of the disease with 33% of PD patients undergoing long-term treatment experiencing VHs during the course of their illness. One line of evidence that is emerging as a possible risk factor in the occurrence of VHs is the sleep-wake cycle and sleep behavior in patients with PD. This study compared sleep patterns in a group of visually hallucinating Parkinson's patients with a group of nonhallucinating PD patients and an age-matched control group. Nocturnal sleep was assessed by actigraphy and dia

Polysomnography and self-reported sleep, pain, fatigue, and anxiety in children with active and inactive juvenile rheumatoid arthritis.

OBJECTIVE: To compare polysomnography (PSG) and self-reported sleep, symptoms (pain and fatigue), and anxiety between children with active and inactive juvenile rheumatoid arthritis (JRA) and examine relations among sleep, symptoms, and anxiety. METHODS: Two consecutive nights of PSG, self-reported sleep, and symptoms were obtained in 70 children 6-11 years of age with active (n = 35) or inactive (n = 35) JRA. RESULTS: On the second (study) night, PSG and self-reported sleep variables were not different, but pain and fatigue were significantly higher (both p <.02) in children with active com

Risk factors for excessive sleepiness in older adults.

OBJECTIVE: To determine risk factors for excessive daytime sleepiness in older adults. METHODS: This is a cross-sectional study assessing multiple risk factors for excessive daytime sleepiness in older subjects (mean age, 78 years; range 65-98 years) with (n=149) and without (n=144) complaints of frequent excessive daytime sleepiness. Assessment of risk factors included full in-laboratory sleep studies. RESULTS: Excessive sleepiness among the elderly is multifactorial. Multivariable modeling identified the following as simultaneously significant risk factors for excessive sleepiness: severe
