Risk factors for severe pulmonary regurgitation after repair of tetralogy of Fallot with transannular patch.

BACKGROUND: Severe pulmonary regurgitation may result in right ventricular volume overload and decreased right ventricular function. Severe pulmonary regurgitation can be predicted prior to repair of tetralogy of Fallot. The aim of this study was to determine the risk factors for severe pulmonary regurgitation in repaired tetralogy of Fallot with transannular patch. METHODS: This was a cross-sectional study in 43 patients with repaired tetralogy of Fallot using transannular patch. This study was carried out in Dr. Cipto Mangunkusumo hospital during 2015 to 2018. Participants were followed up

Percutaneous management of failed bioprosthetic pulmonary valves in patients with congenital heart defects.

AIMS: We reviewed our center experience in the field of transcatheter pulmonary valve-in-valve implantation (TPViV), that is emerging as a treatment option for patients with pulmonary bioprosthetic valve (BPV) dysfunction. METHODS: Between April 2008 and September 2015, a total of six patients with congenital heart disease (four men) underwent TPViV due to stenosis of preexisting BPV. Four patients received a Melody Medtronic Transcatheter Pulmonary Valve and two an Edward Sapien Valve. RESULTS: No procedural-related complications occurred. After valve implantation, right ventricular systoli

Right ventricular dysfunction in patients with idiopathic dilated cardiomyopathy: Prognostic value and predictive factors.

BACKGROUND: Right ventricular (RV) dysfunction is an important predictor of impaired prognosis in idiopathic dilated cardiomyopathy. AIMS: To determine the prognostic role of RV dysfunction, independent of left ventricular (LV) dysfunction. METHODS: A total of 136 consecutive patients (73% men; mean age 59.0±13.2 years) with idiopathic dilated cardiomyopathy (LV ejection fraction ≤ 45%) were enrolled retrospectively. Thirty-four patients (25%, group 1) presented with RV dysfunction, defined as tricuspid annular plane systolic excursion (TAPSE) ≤ 15 mm; 102 patients (group 2) had preserved RV

Homologous monocuspid valve patch in right ventricular outflow tract reconstruction.

BACKGROUND: In the surgical repair of tetralogy of Fallot or pulmonary atresia, pulmonary regurgitation may be detrimental in the postoperative period. We have used homograft monocuspid valve patch to prevent pulmonary insufficiency. METHODS: From September 1996 to December 1998, twenty-five patients, 4 months to 8 years of age (median 10.1 months) had homograft monocuspid valve in the procedure of right ventricular outflow tract reconstruction. The function of the monocuspid valve was assessed by echocardiogram and graded as trivial to mild, mild to moderate, moderate, and severe. We evaluat

Repair of tetralogy of Fallot in neonates and young infants.

BACKGROUND: The timing of repair of tetralogy of Fallot (TOF) remains controversial. Advantages to early complete repair include removal of right ventricular outflow tract obstruction, alleviation of systemic hypoxia, and avoidance of palliation with an arteriopulmonary shunt. METHODS AND RESULTS: This is a retrospective review of 99 children with TOF pulmonary stenosis (TOF/PS) or TOF pulmonary atresia (TOF/PA) who were <90 days of age undergoing early complete repair. Fifty-nine were prostaglandin E dependent, and 91% of neonates were symptomatic at the time of repair. Univariate and multiv

Evaluation of pulmonary perfusion following an acute heroin overdose in dogs.

This research was designed to analyze the effects of heroin overdose on pulmonary perfusion in dogs. In view of this pulmonary perfusion by radionuclide method, arterial blood pressure by the femoral artery, mean right ventricular pressure and pulmonary wedge pressure by Swang-Ganz catheter were detected in 7 medium sized healthy dogs. In addition PaO2, SaO2, PaCO2 and blood pH were also determined by current methods. All the dogs received an acute bolus of purified heroin at dosage of 12.5 mg/Kg. All the measurements were made at baseline and at 30 seconds, 10 minutes, 20 minutes and 30 minut

Primary cardiac amyloidosis: A case presentation.

A young man with primary cardiac amyloidosis presented clinically as a case of 'restrictive' cardiomyopathy with an initial diagnosis of constrictive pericarditis, probably of tuberculous origin. Cardiac catheterization showed features compatible with restrictive cardiomyopathy or constrictive pericarditis but pericardial biopsy was negative. Percutaneous right ventricular endomyocardial biopsies established amyloid infiltration of the myocardium, but rectal, tongue and liver biopsies were all negative for amyloidosis. Bence-Jones protein was absent from the urine. Serum electrophoresis, immun

Malignant Anomalous Aortic Origin of the Left Main Coronary Artery Managed by Ostial Reimplantation: A Case Report.

Anomalous origin of the left main coronary artery (LMCA) from the right coronary sinus with an interarterial course is a rare congenital variant associated with a high risk of myocardial ischemia and sudden cardiac death. We describe a 39-year-old man presenting with exertional chest pressure and dyspnea whose coronary CT angiography revealed an anomalous LMCA arising from the right sinus of Valsalva and coursing between the aorta and pulmonary artery without evidence of atherosclerosis. Transthoracic echocardiography was normal, and stress testing showed no inducible ischemia. The patient und

Warfarin-Aspirin Therapy in Congenitally Corrected Transposition of the Great Arteries After Lateral Tunnel Fontan.

BACKGROUND: Fontan circulation carries high risk of thromboembolic complications, and optimal anticoagulation strategies in pediatric patients remain controversial. CASE SUMMARY: A 13-year-old adolescent boy with dextrocardia, congenitally corrected transposition of the great arteries, ventricular septal defect, and pulmonary stenosis underwent staged surgical palliation including a bidirectional Glenn shunt followed by the lateral tunnel Fontan procedure. Postoperative echocardiography demonstrated a functioning Fontan circulation with reduced right ventricular systolic dysfunction and mild-

Refractory High Intracranial Pressure following Intraventricular Hemorrhage due to Moyamoya Disease in a Pregnant Caucasian Woman.

Intraventricular hemorrhage during pregnancy is usually followed by a poor recovery. When caused by moyamoya disease, ischemic or hemorrhagic episodes may complicate the management of high intracranial pressure. A 26-year-old Caucasian woman presented with generalized seizures and a Glasgow Coma Score (GCS) of 3 during the 36th week of pregnancy. The fetus was delivered by caesarean section. The brain CT in the mother revealed bilateral intraventricular hemorrhage, a callosal hematoma, hydrocephalus and right frontal ischemia. Refractory high intracranial pressure developed and required bilate

Repeated extracorporeal membrane oxygenation for support of an Adult with Congenital Heart Disease and reperfusion pulmonary oedema.

We present the case of 28-year-old woman with a history of complex congenital cardiac surgery who developed cardiovascular collapse with reperfusion pulmonary oedema and right ventricular failure after surgical replacement of a severely stenosed right ventricle to pulmonary artery conduit. She required two separate episodes of support with extracorporeal membrane oxygenation and is alive and well 6 months after her initial cardiorespiratory crisis. We believe that consideration of a second period of support with extracorporeal membrane oxygenation is appropriate for select adults with CHD, pro

Radiofrequency catheter ablation of sinus node reentrant tachycardia.

Sinus node reentrant tachycardia is a relatively uncommon (5%-15%) form of recurrent paroxysmal supraventricular tachycardia (SVT). We describe a case of symptomatic sinus node reentrant tachycardia in a 67-year-old male with ischemic heart disease, congestive heart failure, and depressed ventricular function. Adenosine administered during an electrophysiology study caused prolongation of the tachycardia cycle length due to atrial cycle length prolongation (without atrio-His prolongation) prior to tachycardia termination. Right atrial mapping revealed the earliest site of atrial activation in

The effect of left ventricular size on right ventricular hemodynamics in pediatric survivors with hypoplastic left heart syndrome.

BACKGROUND: Outcome status in patients with hypoplastic left heart syndrome (HLHS) is partially dependent on right ventricular (RV) systolic function. In other disease states, ventricular function is impacted by anatomy and physiology of the contralateral ventricle. In HLHS, it is suggested that a relatively larger left ventricular (LV) size may negatively impact RV function because it becomes a "passenger" without providing any systolic or diastolic physiologic benefit. The purpose of this study was to determine whether LV size adversely affects RV systolic function in surviving patients with

Pulmonary Congestion on Lung Ultrasound in Ambulatory Patients With Heart Failure With Preserved Ejection Fraction.

BACKGROUND: Early detection of pulmonary congestion among ambulatory patients with heart failure with preserved ejection fraction (HFpEF) is critical to optimize decongestive therapy before overt decompensation, yet traditional tools are insensitive. We sought to examine the prevalence of B-lines, an ultrasound measure of pulmonary congestion, and their clinical and imaging correlates in patients with HFpEF. METHODS AND RESULTS: In a prospective, multisite observational study, using a pocket ultrasound device, 8-zone lung ultrasound examination was performed in outpatients with HFpEF, left ve

Mixed lineage kinase 3 requires a functional CRIB domain for regulation of blood pressure, cardiac hypertrophy, and left ventricular function.

Mixed lineage kinase 3 (MLK3) modulates blood pressure and left ventricular function, but the mechanisms governing these effects remain unclear. In the current study, we therefore investigated the role of the MLK3 Cdc42/Rac interactive binding (CRIB) domain in cardiovascular physiology. We examined baseline and left ventricular pressure overload responses in a MLK3 CRIB mutant (MLK3C/C) mouse, which harbors point mutations in the CRIB domain to disrupt MLK3 activation by Cdc42. Male and female MLK3C/C mice displayed increased invasively measured blood pressure compared with wild-type (MLK3+/+)

Targeting the Hidden Substrate Unmasked by Right Ventricular Extrastimulation Improves Ventricular Tachycardia Ablation Outcome After Myocardial Infarction.

OBJECTIVES: This study sought to determine whether ablation of hidden substrate unmasked by right ventricular extrastimulation (RVE) improves ablation outcome of post-myocardial infarction (MI) ventricular tachycardia (VT). BACKGROUND: In patients with small or nontransmural scars after MI, part of the VT substrate may be functional and, in addition, masked by high-voltage far-field signals arising from adjacent normal myocardium. METHODS: In 60 consecutive patients, systematic analysis of electrograms recorded from the presumed infarct area was performed during sinus rhythm, RV pacing at 50

Progression of heart failure after biventricular pacing: Is there a subgroup of "favorable nonresponders"?

BACKGROUND: Left bundle branch block (LBBB) and dominant R wave in lead V1 (RV1) post-biventricular pacing (BiVp) are associated with better clinical outcomes. However, some patients continue to deteriorate despite these favorable electrocardiographic changes. OBJECTIVE: We tested the hypothesis that baseline LBBB and post-BiVp RV1 are associated with better cardiac function and clinical outcomes in patients with progressive deterioration in heart failure after BiVp ("clinical nonresponders") than in patients without these electrocardiographic criteria. METHODS: Consecutive patients with adv

Heart transplants for patients with Chagas' heart disease.

The role of heart transplants for treating Chagas' heart disease is not quite clear. Immunosuppression could lead to resurgence of T. cruzi infection with acute or chronic damage to the allograft. There are few publications regarding this issue. Thus we reported the follow-up of 18-patients with Chagas' heart disease submitted to orthotopic heart transplants from 1985 to 1993 at The Heart Institute. The patients were in functional class IV or II, with sustained ventricular tachycardia episodes. The mean left ventricular ejection fraction was 25 +/- 9% and the mean right ventricular ejection wa

Amplitude of low-frequency fluctuation (ALFF) alterations in heart transplant patients with cognitive impairment: a resting-state fMRI study.

BACKGROUND: The aim of our study was to investigate the changes in brain functional activity in heart transplant patients and to explore the relationship between abnormal spontaneous brain activity and cognitive function through amplitude of low-frequency fluctuations (ALFF). METHODS: Sixty-eight heart transplant patients and 56 healthy controls were assessed by the Montreal Cognitive Assessment (MoCA) scale and the Mini-Mental Status Examination (MMSE) scale, and resting-state functional magnetic resonance scans were performed. Cortical analysis was applied to calculate the ALFF, and two-sam

Systolic improvement and mechanical resynchronization does not require electrical synchrony in the dilated failing heart with left bundle-branch block.

BACKGROUND: Biventricular (BiV) and left ventricular (LV) pacing similarly augment systolic function in left bundle-branch block (LBBB)-failing hearts despite different electrical activation. We tested whether electrical synchrony is required to achieve mechanical synchronization and functional benefit from pacing. METHODS AND RESULTS: Epicardial mapping, tagged MRI, and hemodynamics were obtained in dogs with LBBB-failing hearts during right atrial, LV, and BiV stimulation. BiV and LV both significantly improved chamber hemodynamics (eg, 25% increase in dP/dt(max) and aortic pulse pressure)
