A Dimensional Bus model for integrating clinical and research data.

OBJECTIVES: Many clinical research data integration platforms rely on the Entity-Attribute-Value model because of its flexibility, even though it presents problems in query formulation and execution time. The authors sought more balance in these traits. MATERIALS AND METHODS: Borrowing concepts from Entity-Attribute-Value and from enterprise data warehousing, the authors designed an alternative called the Dimensional Bus model and used it to integrate electronic medical record, sponsored study, and biorepository data. Each type of observational collection has its own table, and the structure

Neurostimulation for primary headache disorders: Part 2, review of central neurostimulators for primary headache, overall therapeutic efficacy, safety, cost, patient selection, and future research in headache neuromodulation.

This article is the second of 2 articles reviewing neurostimulation for primary headaches. In Part 1, we described methods, pathophysiology and anatomy, and history of neuromodulation in the treatment of headache, as well as reviewing the literature on peripheral neuromodulation for primary headaches. Peripheral targets for stimulation include percutaneous nerves, transcranial holocephalic, occipital nerves, auriculotemporal nerves, supraorbital nerves, cervical epidural, and sphenopalatine ganglia. In Part 2, we describe available literature on central neuromodulation in primary headaches. Ce

Amyotrophic lateral sclerosis multiprotein biomarkers in peripheral blood mononuclear cells.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a fatal progressive motor neuron disease, for which there are still no diagnostic/prognostic test and therapy. Specific molecular biomarkers are urgently needed to facilitate clinical studies and speed up the development of effective treatments. METHODOLOGY/PRINCIPAL FINDINGS: We used a two-dimensional difference in gel electrophoresis approach to identify in easily accessible clinical samples, peripheral blood mononuclear cells (PBMC), a panel of protein biomarkers that are closely associated with ALS. Validations and a longitudinal study we

Antiprotozoal compounds: state of the art and new developments.

Protozoa can cause severe diseases, including malaria, leishmaniasis, Chagas disease, sleeping sickness and amoebiasis, all being responsible for morbidity and mortality particularly in tropical countries. To date there are no protective vaccines against any of these diseases, and many of the available drugs are old or elicit serious adverse reactions. Moreover, parasite resistance to existing drugs has become a serious problem. Owing to lack of financial returns, research in this field is of limited interest to pharmaceutical companies and largely depends on funding by public authorities. Thi

Looking to the future: a new decade of pulmonary arterial hypertension therapy.

Pulmonary arterial hypertension (PAH) is a severe and debilitating disease characterised by vascular proliferation and remodelling of the small pulmonary arteries, leading to a progressive increase in pulmonary vascular resistance, increased afterload on the right ventricle and, ultimately, right heart failure. Although there is no "cure" for PAH, the availability of targeted therapies over the past decade has led to major advances in the management of PAH, reflected in improvements in survival in the modern treatment era. However, despite this, disease progression is inevitable in the majorit

Is sickle cell disease sufficiently prioritized in policy and socio-economic research on diseases in Tanzania? Lessons for the past 50 years.

Catastrophic health consequences associated with chronic and genetic disorders, including those related to sickle cell disease (SCD) remain lowly measured and understood. Illnesses associated with SCD, especially the, sickle cell anaemia (SCA) pose significant tolls to individual patients and their families and contribute to poverty due to loss in production and retardation of economic development. This paper synthesises evidence from systematic literature reviews on policy priorities both in theory and practice and studies carried out on SCD. The review was systematically done by drawing evid

Our expanding view of platelet functions and its clinical implications.

Platelets are the primary cell mediator of thrombosis. A deficiency of platelets can result in severe bleeding defects. "Overactive" platelets contribute to life-threatening outcomes in diseases such as heart attack, stroke, and cancer. The use of platelet inhibitors for thrombosis prevention must therefore seek a delicate balance between inhibiting platelet activation and an associated increased bleeding risk. There are currently few platelet inhibitors clinically available, making the search for novel anti-platelet drug targets a major research priority. Several newly identified pathways of

Perspectives of intellectual disability in India: epidemiology, policy, services for children and adults.

PURPOSE OF REVIEW: To provide a panoramic view of the recent developments and current situation in epidemiology, legislation, policies, services, and research in intellectual disability in India. RECENT FINDINGS: A wide variation in point prevalence of intellectual disability has been reported, from around 1/1000 to 32/1000, depending on the case definition, methodology, and population selected. There are a number of acts, policies, and programs addressing intellectual disability, but their implementation needs to improve. Services in terms of prevention, early intervention, health services,

Estimation of the effective doses of nasal-inactivated influenza vaccine in humans from mouse-model experiments.

Mouse models of influenza play an important role in developing effective human influenza vaccines. We have demonstrated that intranasal immunization with inactivated subvirion (SV) vaccines, in conjunction with a cholera toxin B subunit adjuvant (CTB*), provides more effective cross-protection than parenteral immunization in BALB/c mice. In addition, the minimal effective dose of nasal vaccine for providing complete protection against a lethal influenza virus infection is 0.1 microg SV vaccine (containing about 30 ng hemagglutinin [HA]) (with 0.1 microg CTB*) in BALB/c mice (20 g body weight)

Translational research of novel hormones: lessons from animal models and rare human diseases for common human diseases.

Since the 1980s, a number of bioactive molecules, now known as cardiovascular hormones, have been isolated from the heart and blood vessels, particularly from the subset of vascular endothelial cells. The natriuretic peptide family is the prototype of the cardiovascular hormones. Over the following decade, a variety of hormones and cytokines, now known as adipokines or adipocytokines, have also been isolated from adipose tissue. Leptin is the only adipokine demonstrated to cause an obese phenotype in both animals and humans upon deletion. Thus, the past two decades have seen the identification

Qualitative or quantitative differences between Asperger's disorder and autism? Historical considerations.

The histories of autism and Asperger's Disorder (AD), based on original contributions by Kanner and Asperger, are reviewed in relation to DSM-IV diagnostic criteria. Their original articles appear to have influenced the distinction between AD and autism made in the DSM-IV. Based on up-to-date empirical research, however, it appears that AD and autism are not qualitatively distinct disorders, but are different quantitative manifestations of the same disorder. The differences between AD and autism may be a function of individual variability in these areas, not the manifestation of qualitatively

A walk in the park: a case study in research ethics.

Can researchers, interested in novel ways to assess HIV seroprevalence among populations which are otherwise hidden, collect condoms that have been discarded on the ground in a public sex environment and test them for HIV? Researchers, who use other types of abandoned samples, such as discarded syringes, hair or saliva samples, or excess biological samples, confront similar issues. This review evaluates whether such abandoned tissues can be studied based on U.S. Code of Federal Regulations and literature on related issues including: research involving banked tissues, blinded seroprevalence stu

The emergence of tropical medicine in Portugal: the School of Tropical Medicine and the Colonial Hospital of Lisbon (1902-1935).

The School of Tropical Medicine was founded in 1902 along with the Colonial Hospital of Lisbon. The Portuguese government recognized the importance of colonising the tropics and therefore supported the creation of a specific locus of medical training that would prove to be crucial to the clinical and experimental study of tropical diseases. This paper examines the importance of such institutions for the emergence of a new scientific area of research while also functioning as a consolidation factor for the Third Portuguese Colonial Empire. The creation of a new concept of medical practice with

Trends in research on premature ejaculation over the past 25 years.

INTRODUCTION: The authors posit that, among other things, the increasing popularity of pharmacological remedies has reinvigorated research interest in premature ejaculation (PE). However, the authors also posit that the emphasis has shifted from psychological-behavioral studies to biological-pharmacological studies, thereby distracting researchers from potent psychological/relationship factors known to play important roles in the recovery of overall sexual satisfaction. AIM: To explore actual patterns of research on PE, the authors determined trends in the overall quantity and focus of PE res

From 'public service' to artificial insemination: animal breeding science and reproductive research in early twentieth-century Britain.

Artificial insemination (AI) was the first conceptive technology to be widely used in agriculture. Whereas at the beginning of the twentieth century all cows in England and Wales were mated to bulls, by the end of the 1950s 60% conceived through artificial insemination. By then a national network of 'cattle breeding centres' brought AI within the reach of every farmer. In this paper I explore how artificial insemination, which had few supporters in the 1920s and 1930s, was transformed into an 'indispensable' method for reproducing cattle. I discuss the factors that made organised AI possible (

Experiences with an interoperable data acquisition platform for multi-centric research networks based on HL7 CDA.

OBJECTIVES: Today's increasing specialization of medicine necessitates the integration of telematic platforms for cross-institutional cooperation. In order to leverage the strengths of each cooperating institution a centralized unified storage using shared electronic patient records (EPRs) as well as secured remote data entry capabilities for supporting collaborative clinical research and care is essential. The objective of this project was to develop and introduce into routine use such a shared remote data entry (RDE) platform for the German multicentric Epidermolysis Bullosa (EB) network. M

[The situation of rare skin diseases in Germany].

In Germany a disease is considered to be "rare" when its prevalence is less than 5:10,000. However, as far as genetic skin diseases are concerned, they are often much more uncommon with a prevalence in the range of 1:100,000. Currently considerable deficits prevail in both diagnosis and treatment of rare skin diseases. Driven by scientific research on rare skin diseases and benefiting from start-up grants of the Federal Ministry for Education and Research (BMBF), the following networks for rare skin diseases were formed: a) epidermolysis bullosa, b) ichthyosis and related keratinisation disord
