Effectiveness of Cranberry Supplementation for Prevention and Treatment of Infectious Urinary Tract Disease in Dogs and Cats: A Systematic Review.

Cranberry and cranberry extracts are available for treatment or prevention of urinary tract disease in dogs and cats, but guidance regarding their use is lacking. The objectives were to identify and assess literature pertaining to the efficacy of cranberry and cranberry extract supplementation for prevention and treatment of bacterial cystitis and subclinical bacteriuria in dogs and cats. A systematic review was performed, and three studies involving a total of 122 animals (106 dogs and 16 cats) were identified. No studies reported statistically significant or numerical impacts of treatment. T…

Treatment of a urethral stricture by image-guided placement of a custom-made absorbable stent in a standing, sedated horse.

A 10-year-old Irish Sport Horse gelding developed complications from a general anesthesia resulting in sling support and recurrent urinary catheterization. The horse subsequently presented signs of dysuria and pollakiuria, was diagnosed with sabulous cystitis, and developed a urethral stricture from the repeated catheterizations, which was confirmed on urethroscopy. Clinical signs persisted despite conservative management with topical corticosteroids and urethral bougienage with balloon dilators. An image-guided approach was used to treat the stricture with a custom-made polydioxanone stent pl…

Why d-Mannose May Be as Efficient as Antibiotics in the Treatment of Acute Uncomplicated Lower Urinary Tract Infections-Preliminary Considerations and Conclusions from a Non-Interventional Study.

Urinary tract infections (UTIs) are very frequent in women and can be caused by a range of pathogens. High recurrence rates and increasing antibiotic resistance of uropathogens make UTIs a severe public health problem. d-mannose is a monosaccharide that can inhibit bacterial adhesion to the urothelium after oral intake. Several clinical studies have shown the efficacy of d-mannose in the prevention of recurrent UTIs; these also provided limited evidence for the efficacy of d-mannose in acute therapy. A recent prospective, non-interventional study in female patients with acute cystitis reported…

Unexpected presentation of acute porphyria.

Acute porphyrias are rarely reported in Southeast Asia. They may be underdiagnosed due to their clinical mimicry and lack of awareness among physicians. There is a common cognitive bias to gravitate towards common conditions. In this case report, a 28-year-old woman, who presented with seizures, rhabdomyolysis hyponatraemia and altered mental state, was initially diagnosed as amphetamine overdose. She had presented 3 days prior with abdominal pain, treated for acute cystitis and discharged. On readmission for seizures a day later, she was extensively worked up for altered mental state. Despite…

Candida urinary tract infections in adults.

Candiduria is commonly seen in hospitalized patients and most of the patients are asymptomatic, but it may be due to cystitis, pyelonephritis, prostatitis, epididymo-orchitis or disseminated candidiasis. Major risk factors are diabetes mellitus, indwelling urinary catheters, use of broad-spectrum antibiotics, urinary obstruction, and admission to intensive care units. Candida urinary tract infections can be caused by hematogenous spread following candidemia, or retrograde route via the urethra. The presence of Candida species in urine in asymptomatic patients does not warrant antifungal therap…

The Structure of an Infectious Human Polyomavirus and Its Interactions with Cellular Receptors.

BK polyomavirus (BKV) causes polyomavirus-associated nephropathy and hemorrhagic cystitis in immunosuppressed patients. These are diseases for which we currently have limited treatment options, but potential therapies could include pre-transplant vaccination with a multivalent BKV vaccine or therapeutics which inhibit capsid assembly or block attachment and entry into target cells. A useful tool in such efforts would be a high-resolution structure of the infectious BKV virion and how this interacts with its full repertoire of cellular receptors. We present the 3.4-Å cryoelectron microscopy str…

Infections in patients on BCR-ABL tyrosine kinase inhibitor therapy: cases and review of the literature.

The introduction of BCR-ABL-tyrosine kinase inhibitors (TKI) for treatment of hematologic malignancies has made a significant impact on patient outcome. Contingent upon their targeted and off-target activity, therapy-associated infectious complications may occur. We present a case of cytomegalovirus pneumonitis and a case of adenovirus hemorrhagic cystitis in two patients with Philadelphia chromosome-positive acute lymphoblastic leukemia on BCR-ABL TKI treatment and review the literature to summarize the infectious complications based on clinical data. As life-threatening infections may occur,…

Evaluation of toxicity of a chronic alternate day metronomic cyclophosphamide chemotherapy protocol in dogs with naturally occurring cancer.

Sterile hemorrhagic cystitis (SHC) is an important complication of cyclophosphamide chemotherapy in dogs as it is reported in up to 23% of cases with various protocols. The current study reports toxicities of a protocol of metronomic cyclophosphamide, and identifies risk factors for development of adverse effects. A retrospective cohort study of dogs treated with metronomic cyclophosphamide at an intended dose of 25 mg/m2 every other day was conducted. Fifty dogs were included with a median length of treatment of 90 days (range: 1 to 1305 days). Treatment was discontinued in 22 dogs (44%) due …

Urethral obstruction in male cats in some Northern United States shows regional seasonality.

Feline lower urinary tract disease (FLUTD) is a term encompassing several different conditions affecting the feline lower urinary tract. Certain FLUTD aetiologies, such as idiopathic cystitis, urethral plugs or urolithiasis, commonly produce urethral obstruction (UO) in male cats. It is widely accepted that environmental, behavioural and dietary factors can play a role in the aetiopathogenesis of these conditions. We investigated the seasonal prevalence of UO by analysing admission dates of 2443 male cats with UO from eight practices in the Northern USA over a 4-year period. A significantly gr…
