What's the worst that could happen? The risk of illness, injury, and death related to international travel-the Australian experience.

INTRODUCTION: Medical advisors to travellers whose journeys begin and end in Australia will benefit from a review of the associated risks, to guide pre- and post-travel advice and care. METHODS: A literature search was conducted for original research on medical events in Australian residents related to international travel. Important findings were summarised, incidence and risk factor data extracted, and grouped thematically for narrative review. RESULTS: 31 studies published between 1995 and 2024 were identified. Reports of medical events during travel ranged between 28 % and 82 % of travel

Pilot study of intraperitoneal administration of triamcinolone acetonide for cancerous ascites in patients with end-stage gynecological cancer.

OBJECTIVE: Patients with end-stage cancer have poorly controlled ascites retention resulting due to cancerous peritonitis. We intraperitoneally administered triamcinolone acetonide (TA) to patients with end-stage gynecological cancer as a pilot study, and our treatment results are reported herein. PATIENTS AND METHODS: We enrolled 26 patients with end-stage gynecological cancer requiring frequent abdominal paracentesis for ascites drainage between April 2010 and September 2012. The volume of ascites drainage was 2000 to 3000 mL per drainage session, and TA at 10 mg/kg was intraperitoneally ad

Revealing brain network dynamics during the emotional state of suspense using TDA.

Suspense is an affective state that is ubiquitous in human life, from art to quotidian events. However, little is known about the behavior of large-scale brain networks during suspenseful experiences. To address this question, we examined the continuous brain responses of participants watching a suspenseful movie, along with reported levels of suspense from an independent set of viewers. We employ sliding window analysis and Pearson correlation to measure functional connectivity states over time. Then, we use Mapper, a topological data analysis tool, to obtain a graphical representation that c

Current challenges in understanding, diagnosing and managing sepsis-induced cardiac dysfunction.

Sepsis is a severe, life-threatening condition, which may be responsible for multiple complications, including cardiovascular events. Despite being a common complication, sepsis-induced cardiac dysfunction (SICD) is still not entirely understood. This review underlines the diagnosis challenges, discusses the prognosis and proposes a diagnosis path for patients with suspected SICD. Existing severity scoring systems are not able or designed to predict the risk of cardiac dysfunction in septic patients. Current biomarkers are not sufficient for the diagnosis of SICD, only for the exclusion of acu

Periodontal Diseases: Major Exacerbators of Pulmonary Diseases?

Periodontal diseases are a range of polymicrobial infectious disorders, such as gingivitis and periodontitis, which affect tooth-supporting tissues and are linked to playing a role in the exacerbation of several pulmonary diseases. Pulmonary diseases, such as pneumonia, chronic obstructive pulmonary disease (COPD), asthma, tuberculosis, COVID-19, and bronchiectasis, significantly contribute to poor quality of life and mortality. The association between periodontal disease and pulmonary outcomes is an important topic and requires further attention. Numerous resident microorganisms coexist in th

Tai Chi for heart attack survivors: qualitative insights.

OBJECTIVES: Cardiac rehabilitation (CR) programmes are standard of care for patients following a coronary event. While such exercise-based secondary prevention programme do offer benefits, they are used by less than 30% of eligible patients and attrition within these programmes is high. This project is a nested qualitative assessment of a pilot programme considering Tai Chi (TC) as an alternative to CR. We hypothesised that TC may overcome several key barriers to CR. METHODS: A semistructured focus group agenda was used to assess three key domains of feasibility: (1) patients' experiences, (2

Homocysteine and age-associated disorders.

There are numerous theories of aging, a process which still seems inevitable. Aging leads to cancer and multi-systemic disorders as well as chronic diseases. Decline in age- associated cellular functions leads to neurodegeneration and cognitive decline that affect the quality of life. Accumulation of damage, mutations, metabolic changes, failure in cellular energy production and clearance of altered proteins over the lifetime, and hyperhomocysteinemia, ultimately result in tissue degeneration. The decline in renal functions, nutritional deficiencies, deregulation of methionine cycle and defici

Chronic bronchial infection in COPD. Is there an infective phenotype?

Microorganisms, particularly bacteria, are frequently found in the lower airways of COPD patients, both in stable state and during exacerbations. The host-pathogen relationship in COPD is a complex, dynamic process characterised by frequent changes in pathogens, their strains and loads, and subsequent host immune responses. Exacerbations are detrimental events in the course of COPD and evidence suggests that 70% may be caused by microorganisms. When considering bacterial exacerbations, recent findings based on molecular typing have demonstrated that the acquisition of new strains of bacteria o

Obstetrical factors governing the etiopathogenesis of lambdoid synostosis.

Lambdoid synostosis results in skull deformities of varying degrees characterized by occipital flattening over the involved suture and other compensatory changes in skull shape. Such changes include contralateral occipital bossing, contralateral frontal flattening, ipsilateral frontal bossing, and ipsilateral anteroinferior displacement of the pinna (ear shearing). These deformities tend to worsen during the first year of life. The etiology has been attributed to genetic factors and primary disorders of bone growth, in addition to secondary effects of other diseases and modulators of the in ut

Endocrine and immune adaptation in stress.

Several regulatory processes within the immune system have been clearly established. This complex autoregulation represents a self-monitoring system which protects the organism and maintains body homeostasis. There is, however, much evidence to suggest that the immune system is also under external regulation, especially by the endocrine and neural systems. These internal and neuroendocrine signals are thought to interact in both synergistic and antagonistic fashion, and are probably integrated in feedback circuits. Neuroendocrine and immune systems seem to act in parallel in many conditions, f

Efficacy and safety of dutasteride in patients with spinal and bulbar muscular atrophy: a randomised placebo-controlled trial.

BACKGROUND: Spinal and bulbar muscular atrophy (SBMA) is caused by polyglutamine expansion in the androgen receptor, which results in ligand-dependent toxicity. Animal models have a neuromuscular deficit that is mitigated by androgen-reducing treatment. We aimed to assess the efficacy and safety of the 5α-reductase inhibitor dutasteride in patients with SBMA, and to identify outcome measures for use in future studies of the disease. METHODS: We undertook a randomised, double-blind, placebo-controlled, single-site clinical trial in ambulatory, symptomatic men with genetically confirmed SBMA. P

Population genetic structure and historical demography of Saccostrea echinata in the Northern South China sea and Beibu Gulf.

Marine invertebrates, such as oysters, were once thought to form large, panmictic populations with little genetic differentiation due to their high reproductive capacity and dual life stages. However, recent studies have shown significant genetic structuring and moderate gene flow across populations, influenced by factors like ocean currents, historical climate events, and environmental changes. The black-lip oyster (Saccostrea echinata), with its extensive dispersal potential, is ideal for population genetics studies. In current study, mitochondrial DNA (COI gene) was utilized to investigate

ALAT-SEPAR Consensus on the Definition and Classification of Asthma Exacerbations by Severity: A Move Toward International Standardization.

Asthma exacerbation is a significant clinical event that occurs with varying severity, yet a set of universally accepted standardized definitions is still needed. The aim of this consensus developed by ALAT and SEPAR was to fill this gap with a validated proposal for classifying asthma exacerbations into levels of severity: non-severe, severe, and very severe. The consensus was based on an in-depth review of the literature conducted by the scientific committee to identify key parameters for each level of severity, including worsening symptoms, respiratory function changes, and the need for spe

Feasibility of a home-based exercise intervention for patients with systemic sclerosis.

INTRODUCTION: Systemic sclerosis (SSc) is a rare connective tissue disease causing pain, reduced mobility and decreased health-related quality of life (HRQoL). Studies suggest that exercise is a necessary adjunct to the medical treatment. PURPOSE: To examine the feasibility of a 12-week home-based exercise program for SSc patients, and evaluate changes in physical function, HRQoL and SSc-related symptoms. METHODS: Twenty patients were enrolled in the study. Feasibility was measured by adherence, adverse events, and exercise task self-efficacy. At baseline and follow-up patients completed the
