Comparative efficacy of traditional Chinese exercises in low back pain: a systematic review and network meta-analysis.

BACKGROUND AND PURPOSE: Low back pain (LBP) is becoming increasingly common and has a significant negative impact on the physical and mental health of patients. A certain number of studies have been conducted on traditional Chinese exercises (TCEs) to treat LBP. However, the optimal treatment remains uncertain. The aim of this study was to systematically review the literature and perform a network meta-analysis (NMA) to compare the efficacy of different TCE and combination therapies for clinical practice. METHODS: We systematically searched 8 databases from inception to February 2025 for rand

Comparison of treatment effects between ACR response criteria and target-based outcomes for contemporary rheumatoid arthritis drug approval trials: A meta-epidemiological study.

OBJECTIVE: Regulatory priorities diverge between agencies - the FDA maintained ACR20 response as primary endpoint in rheumatoid arthritis (RA) trials, whereas the EMA emphasized target-based outcomes. We aim to systematically compare treatment effect estimates between American College of Rheumatology (ACR) response criteria (ACR20/50/70) and target-based outcomes in placebo-controlled trials of approved RA therapies. METHODS: A meta-epidemiological study was performed on randomized placebo-controlled trials investigating approved biological and targeted DMARDs (bioDMARDs and tsDMARDs) in pati

Registration and reporting characteristics of trials investigating exercise therapy following total knee arthroplasty: a systematic review.

BACKGROUND AND PURPOSE: Prospectively registering the primary trial outcome is important to reduce selective outcome reporting and increase the trustworthiness of findings, which guide clinical practice. The objectives of our systematic review were to explore and compare the reporting characteristics of prospectively and non-prospectively registered trials investigating exercise therapy following total knee arthroplasty. METHODS: Randomized trials comparing effects of exercise therapy after total knee arthroplasty for osteoarthritis were identified in 4 databases from 2000 to August 12, 2024.

Effects of exercise interventions on adolescents with mild to moderate scoliosis: a meta-analysis.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a prevalent spinal deformity that impairs physical function and quality of life. While supervised exercise is recommended for moderate cases, its effectiveness remains debated. This meta-analysis evaluates the impact of exercise therapy on Cobb angle, trunk rotation (ATR), and quality of life (QoL) in AIS patients. METHODS: We systematically searched PubMed, Web of Science, EBSCO and China National Knowledge Infrastructure (CNKI) databases from inception to October 2025 for randomized controlled trials (RCTs) investigating exercise in AIS.

Intervention versus observation for asymptomatic kidney stones: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: The detection rate of asymptomatic stones has increased with advances in imaging technology. Although many such stones remain stable, some patients later experience pain, obstruction, infection, stone growth, or need for intervention. Current guideline-based management generally favors surveillance for many asymptomatic stones while emphasizing individualized shared decision-making. We therefore conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to compare active stone-directed intervention versus observation for asymptomatic kidney stones. Cross-exclusion of patients with chronic obstructive pulmonary disease and heart failure in randomized controlled drug trials.

Chronic obstructive pulmonary disease (COPD) and heart failure (HF) frequently coexist, yet patients with both conditions are often excluded from randomized controlled trials(RCTs). We conducted a secondary analysis of 72 RCTs (28 COPD/COPD exacerbations (ECOPD); 44 HF) including 181,325 participants. In COPD/ECOPD trials, only 10.8% of participants with HF, while 6.9% explicitly excluded them. In HF trials, 31.4% included participants with COPD, whereas 47.2% excluded them. There is frequent cross-exclusion of patients with coexisting COPD and HF in RCTs, limiting the applicability of current

Level 1 versus experiential evidence for mitigating pancreatic fistula following pancreatoduodenectomy: Establishing a foundation for future progress.

BACKGROUND: Clinically relevant postoperative pancreatic fistula remains the dominant problem following pancreatoduodenectomy. Level 1 evidence, accrued over 35 years, now includes dozens of randomized controlled trials and meta-analyses, which assess myriad mitigation techniques. Their conclusions are not generally reflected in actual adoption into clinical practice; meanwhile, experiential data might enable more comprehensive evaluation of mitigation. A conundrum arises: how can surgeons reconcile these contrasting sources to effect best practice? METHODS: Level 1 evidence studies were coll
