BACKGROUND: Proximal traumatic lower-extremity amputation has become the signature injury of the war in Afghanistan. Casualties present in extremis and often require immediate operative control of arterial inflow to prevent exsanguination. This study evaluated the use of this strategy and its complications.
METHODS: This is a retrospective analysis of case notes of UK service personnel, identified from the UK Joint Theatre Trauma Registry, who sustained traumatic lower-extremity amputation requiring suprainguinal vascular control, following improvised explosive device injury in Afghanistan, b

Many difficult decisions are faced in the early management of severely burned patient. The decision to amputate an extremity or extremities may be very difficult but reduce morbidity and enhance survival of the patient. In a total of 1144 patients from January 2000 and June 2011, there were 44 patients (3.8%) undergoing amputations of the digits or upper extremity proximal to the wrist or lower extremity above the ankle. Amputations were significantly higher in males and the mean hospitalization time was also significantly higher in these patients. Majority of the patients had non-viable tissu

BACKGROUND: Multidetector computed tomographic angiography (MDCTA) is increasingly being used for the assessment of extremity vascular injury. However, to date, there are only retrospective series and a single small prospective study evaluating its efficacy. Therefore, the objective of this study was to prospectively evaluate the ability of MDCTA to detect arterial injury in the injured upper and lower extremities.
METHODS: After institutional review board approval, all trauma patients aged 16 years or older admitted to a Level I trauma center who sustained extremity trauma and underwent init

The Center for the Intrepid (CFI) is a unique facility among the three amputee care centers that comprise the Armed Forces Amputee Care Program. The mission of the CFI is threefold: (1) to provide the best possible patient care to the severely war-wounded, (2) to educate providers in the most advanced methods of rehabilitation for the severely wounded, and (3) to perform research to improve the care of these war-wounded patients. The center's program is based on three critical factors: (1) concentration of similarly injured patients as a cohort, (2) a multidisciplinary approach to patient care

INTRODUCTION: The experience on management of crush injury after a devastating earthquake is lacking, and there are even less reports on the front-line critical care of these patients. A front-line intensive care unit (ICU) was set up in a tent after the disastrous Wenchuan earthquake (May, 12, 2008, China), where 32 patients suffering from crush injury were treated from May 12 to May 26. This study summarized our experience on management of 32 crush injury patients in a front-line tent ICU.
METHODS: We retrospectively analyzed the clinical data of 32 crush injury patients treated in our fron

A retrospective review of 20 free-tissue transfers for reconstruction of traumatic lower extremity wounds was undertaken. The wounds consisted of Gustilo type IIIb injuries resulting from motor vehicle accidents, falls, motorcycle and four-wheel, all-terrain vehicle accidents, and a laceration from a lawnmower blade. Fifty percent of the procedures were for open tibia fractures. The most common flap used was the rectus abdominis, followed by the latissimus dorsi, serratus anterior, and radial forearm. There were two major complications resulting in flap loss. Adequate and multiple debridements

The concept of emergency free tissue transfer for severe extremity injuries represents a cutting-edge technology. We discuss our very positive results with this technique. The conceptual reasons for these favorable results, compared with conventional approaches, are also discussed. An initial, radical debridement is the most important part of the operation. This is then followed by osteosynthesis. The correct type of free tissue transfer is chosen according to the requirements of the soft tissue defect. Qualitatively better results with earlier definitive rehabilitation were achieved with free

Proper throwing mechanics may enable an athlete to achieve maximum performance with minimum chance of injury. While quantifiable differences do exist in proper mechanics for various sports, certain similarities are found in all overhand throws. One essential property is the utilisation of a kinetic chain to generate and transfer energy from the larger body parts to the smaller, more injury-prone upper extremity. This kinetic chain in throwing includes the following sequence of motions: stride, pelvis rotation, upper torso rotation, elbow extension, shoulder internal rotation and wrist flexion.

A retrospective analysis of 4,411 consecutive pediatric fracture patients managed between 1979 and 1983 demonstrated that only 3.6% (170 patients) required internal fixation. Two patient groups with sufficient follow-up study were compared: Group 1-90 skeletally immature children and young adolescents, and Group 2-66 skeletally mature adolescents. Upper-extremity fractures, especially of the distal humerus, and displaced epiphyseal fractures were the major indication for internal fixation in Group 1, while lower-extremity diaphyseal and intra-articular fractures predominated in Group 2. BACKGROUND: The reverse sural artery flap (RSAF) was reported to be a less technically demanding method for the coverage of defects in the distal lower leg, which can be elevated with short operative times. However, several studies pointed out the high frequency of partial necrosis in patients with comorbidities, which was primarily attributed to inadequate venous drainage. To overcome this challenge, we hypothesized that venous supercharging could effectively alleviate congestion of RSAF, potentially minimizing partial necrosis and related complications not only in healthy patients but also i

BACKGROUND: The purpose of this study was to compare 18-month clinical and patient-reported outcomes between patients with severe lower-limb injuries treated with a transtibial amputation or a hind- or midfoot amputation. Despite the theoretical benefits of hind- and midfoot-level amputation, we hypothesized that patients with transtibial amputations would report better function and have fewer complications.
METHODS: The study included patients 18 to 60 years of age who were treated with a transtibial amputation (n = 77) or a distal amputation (n = 17) and who were enrolled in the prospective

Improved knowledge of vascular anatomy has enabled surgeons to preoperatively identify perforators and design free-style flaps based on that perforator. Options for choosing the optimal donor site tissues are increased with the free-style technique. This reduces donor site morbidity while providing the same reconstructive success as traditional free skin flaps. The free-style technique allows the surgeon to successfully complete reconstruction when aberrant anatomy is encountered. With the necessary skills in perforator flap dissection and supermicrosurgery, the armamentarium of the reconstruc

The purpose of this study was to define the trends in fracture complexity and overall injury severity of orthopedic trauma patients at a level I trauma center. A retrospective review of a prospectively collected trauma database was performed to determine the Injury Severity Score (ISS) and AO/OTA classification of the most common fractures among all patients presenting from 1995 to 1999 and from 2008 to 2012. Inclusion criteria were lower extremity fractures of the femur and tibia and pelvic fractures within the years of interest. Exclusion criteria were age younger than 18 years, pathologic f

Both, hamstring and ACL injuries are among the most typical injuries, particularly in change of direction and high speed running sports. They're also difficult to treat. Therefore, in the past few years, sports medicine practitioners and exercise scientists have mainly been focusing on the development and implementation of preventive programs in order to reduce the number of lower limb injuries, mainly by improving knee alignment. A number of studies have been able to prove the success of these training interventions, which are mainly addressing sensorimotor abilities and plyometric activities

The distally based sural flap is an efficient flap for reconstruction of soft tissues defects of lower limb. The unstable vascular pedicle, however, is prone to compression by the subcutaneous tunnel, especially when a long pedicle covers the distal area of the foot. The aim of the present study was to introduce a modified surgical technique that leaves the skin extension over the pedicle and to report the clinical results of this modification. A total of 25 consecutive patients with a mean age of 51.7 ± 14.7 years underwent surgery. We modified the conventional sural flap technique by leaving

BACKGROUND: Multidetector computed tomographic angiography (MDCTA) has become the gold standard for the early assessment of lower extremity vascular injury. The objective of this study was to evaluate the predictive value of MDCTA documented vessel run-off to the foot on limb salvage rates after lower extremity vascular injury.
METHODS: All trauma patients undergoing lower extremity MDCTA for suspected vascular injury assessed at 2 high-volume Level I trauma centers between January 2009 and December 2012. Demographics, clinical data and outcomes (compartment syndrome requiring fasciotomy and

Posttraumatic infected massive bone defects in lower extremities are difficult to repair because they frequently exhibit massive bone and/or soft tissue defects, serious bone infection, and excessive scar proliferation. This study aimed to determine whether these defects could be classified and repaired at a single stage. A total of 51 cases of posttraumatic infected massive bone defect in lower extremity were included in this study. They were classified into four types on the basis of the conditions of the bone defects, soft tissue defects, and injured limb length, including Type A (without s
