Population ageing and healthcare demand: The case of Slovenia.

The aim of this paper is to explore the consequences of demographic ageing on healthcare demand in Slovenia for primary care, secondary care, hospital day-care treatments, and hospitalisations. In the paper, we develop a model for making projections of the total number of treatments using the age-group projection method with the scenario approach. The model allows the number of treatments to be observed with respect to medical services, age groups and main disease groups. The results are presented for the cross-section years 2015, 2025 and 2035. The smallest increase in the number of treatment

Is There Truly "No Significant Difference"? Underpowered Randomized Controlled Trials in the Orthopaedic Literature.

BACKGROUND: Randomized controlled trials (RCTs) are considered the gold standard in evidence-based medicine. Underpowered RCTs that describe comparative outcomes without significance are of questionable benefit. The present study hypothesizes that a substantial proportion of RCTs in the orthopaedic literature that do not note significant differences between groups are inadequately powered. METHODS: Using the ISI Web of Science database, we searched all English-language journals in the orthopaedic category for RCTs published from January 2012 to December 2013. Qualifying articles were analyzed

Detection of influenza-like illness aberrations by directly monitoring Pearson residuals of fitted negative binomial regression models.

BACKGROUND: Emerging novel influenza outbreaks have increasingly been a threat to the public and a major concern of public health departments. Real-time data in seamless surveillance systems such as health insurance claims data for influenza-like illnesses (ILI) are ready for analysis, making it highly desirable to develop practical techniques to analyze such readymade data for outbreak detection so that the public can receive timely influenza epidemic warnings. This study proposes a simple and effective approach to analyze area-based health insurance claims data including outpatient and emerg

Do drug seizures predict drug-related emergency department presentations or arrests for drug use and possession?

BACKGROUND: Direct evidence of the effect of drug seizures on drug use and drug-related harm is fairly sparse. The aim of this study was to see whether seizures of heroin, cocaine and ATS predict the number of people arrested for use and possession of these drugs and the number overdosing on them. METHOD: We examined the effect of seizure frequency and seizure weight on arrests for drug use and possession and on the frequency of drug overdose with autoregressive distributed lag (ARDL) models. Granger causality tests were used to test for simultaneity. RESULTS: Over the short term (i.e. up to

Numeric score-based conditional and overall change-in-status indices for ordered categorical data.

Planned interventions and/or natural conditions often effect change on an ordinal categorical outcome (e.g., symptom severity). In such scenarios, it is sometimes desirable to assign a priori scores to observed changes in status, typically giving higher weight to changes of greater magnitude. We define change indices for such data based upon a multinomial model for each row of a c × c table, where the rows represent the baseline status categories. We distinguish an index designed to assess conditional changes within each baseline category from two others designed to capture overall change. One

Clinical and economic impact of various strategies for varicella immunity screening and vaccination of health care personnel.

BACKGROUND: Exposure to patients with varicella or herpes zoster causes considerable disruption to a health care facility's operations and has a significant health and economic impact. However, practices related to screening for immunity and immunization of health care personnel (HCP) for varicella vary widely. METHODS: A decision tree model was built to evaluate the cost-effectiveness of 8 different strategies of screening and vaccinating HCP for varicella. The outcomes are presented as probability of acquiring varicella, economic impact of varicella per employee per year, and cost to preven

Insights into the classification of myasthenia gravis.

BACKGROUND AND PURPOSE: Myasthenia gravis (MG) is often categorized into thymoma-associated MG, early-onset MG with onset age <50 years, and late-onset MG with onset age ≥50 years. However, the boundary age of 50 years old between early- and late-onset MG remains controversial, and each category contains further subtypes. We attempted to classify MG from a statistical perspective. METHODS: We analyzed 640 consecutive MG patients using two-step cluster analysis with clinical variables and discrimination analysis, using onset age as a variable. RESULTS: Two-step cluster analyses categorized MG

Task independent identification of sensor location on upper limb from orientation data.

In this paper we describe a novel method for sensor placement identification, and demonstrate the effectiveness of this method on an upper limb neuroprothesis for tremor suppression under a variety of tasks. Our objective is to facilitate long-term tremor monitoring; tremor is the most prevalent movement disorder. Two assumptions are made: 1) movement and tremor demonstrate an additive effect further down the kinematic chain; 2) most applications have chained or fixed sensor locations. These assumptions justify obtaining absolute location through identifying relative location and thus to allow

Risk for exposure to alcohol, tobacco, and other drugs on the route to and from school: the role of alcohol outlets.

Despite the national push encouraging children to walk to school, little work has been done to examine what hazards children encounter on the route to school. This study examined the association between the presence of alcohol outlets on children's route to school and perceived safety on the route to school as well as exposure to alcohol, tobacco, and other drugs (ATOD). Data come from a community-based epidemiological study of 394 urban elementary school students. Participants' residential address, school location, and alcohol outlet data were geocoded and the route to school was mapped. The

A time-domain hybrid analysis method for detecting and quantifying T-wave alternans.

T-wave alternans (TWA) in surface electrocardiograph (ECG) signals has been recognized as a marker of cardiac electrical instability and is hypothesized to be associated with increased risk for ventricular arrhythmias among patients. A novel time-domain TWA hybrid analysis method (HAM) utilizing the correlation method and least squares regression technique is described in this paper. Simulated ECGs containing artificial TWA (cases of absence of TWA and presence of stationary or time-varying or phase-reversal TWA) under different baseline wanderings are used to test the method, and the results

Significant increase of Echinococcus multilocularis prevalence in foxes, but no increased predicted risk for humans.

The emergence of the zoonotic tapeworm Echinococcus multilocularis, causative agent of alveolar echinococcosis (AE), poses a public health risk. A previously designed risk map model predicted a spread of E. multilocularis and increasing numbers of alveolar echinococcosis patients in the province of Limburg, The Netherlands. This study was designed to determine trends in the prevalence and worm burden of E. multilocularis in foxes in a popular recreational area in the southern part of Limburg to assess the risk of infection for humans and to study the prevalence of E. multilocularis in dogs in

Kinetics of struvite precipitation in synthetic biologically treated swine wastewaters.

An experimental design was set up to understand the influence of five process parameters on the kinetics of struvite precipitation in synthetic swine wastewaters. The responses studied were the kinetics of phosphorus (P) removal, the struvite precipitation rate and the dissolution rate of amorphous calcium phosphates (ACP). The kinetic study showed that the P-removal was complete in less than 1 h and was influenced positively by the added MgO. The precipitation of struvite with MgO was confirmed to follow a first-order kinetic. This study showed that ACP co-precipitated with struvite during th

Sparse kernel machine regression for ordinal outcomes.

Ordinal outcomes arise frequently in clinical studies when each subject is assigned to a category and the categories have a natural order. Classification rules for ordinal outcomes may be developed with commonly used regression models such as the full continuation ratio (CR) model (fCR), which allows the covariate effects to differ across all continuation ratios, and the CR model with a proportional odds structure (pCR), which assumes the covariate effects to be constant across all continuation ratios. For settings where the covariate effects differ between some continuation ratios but not all

Relative contributions of neighbourhood and animal movements to Coxiella burnetii infection in dairy cattle herds.

Q fever in dairy cattle herds occurs mainly after inhalation of contaminated aerosols generated from excreta by shedder animals. Propagation of Coxiella burnetii, the cause of the disease between ruminant herds could result from transmission between neighbouring herds and/or the introduction of infected shedder animals in healthy herds. The objective of this study were (i) to describe the spatial distribution C. burnetii-infected dairy cattle herds in two different regions: the Finistère District in France (2,829 herds) and the island of Gotland in Sweden (119 herds) and (ii) to quantify and c

Lyme disease risk influences human settlement in the wildland-urban interface: evidence from a longitudinal analysis of counties in the northeastern United States.

The expansion of human settlement into wildland areas, including forests in the eastern United States, has resulted in fragmented forest habitat that has been shown to drive higher entomological risk for Lyme disease. We investigated an alternative pathway between fragmentation and Lyme disease, namely whether increased risk of Lyme disease results in a reduced propensity to settle in high-risk areas at the interface of developed and undeveloped lands. We used longitudinal data analyses at the county level to determine whether Lyme disease incidence (LDI) influences the proportion of the popul

Can reduced predation offset negative effects of sea louse parasites on chum salmon?

The impact of parasites on hosts is invariably negative when considered in isolation, but may be complex and unexpected in nature. For example, if parasites make hosts less desirable to predators then gains from reduced predation may offset direct costs of being parasitized. We explore these ideas in the context of sea louse infestations on salmon. In Pacific Canada, sea lice can spread from farmed salmon to migrating juvenile wild salmon. Low numbers of sea lice can cause mortality of juvenile pink and chum salmon. For pink salmon, this has resulted in reduced productivity of river population

Modeling complexity of EMA data: time-varying lagged effects of negative affect on smoking urges for subgroups of nicotine addiction.

INTRODUCTION: Ecological momentary assessments (EMA) are increasingly used in studies of smoking behavior. Through EMA, examination of lagged relationships is particularly useful for establishing a temporal order of events and for identifying types and timing of risk factors. The time-varying effect model (TVEM) handles EMA data challenges and addresses unique questions about the time-varying effects. METHODS: Generalized TVEM was applied to EMA data from a smoking cessation study to investigate a "time-varying lagged" effect of negative affect on high smoking urges. Participants included 224

Small doses from artificial UV sources elucidate the photo-production of vitamin D.

To clarify the relation between UV exposure and vitamin D status, 201 volunteers wore personal electronic UV dosimeters during daylight hours, to record their UV exposure over a 10 week period when ambient UV levels were significantly less than the summer maxima. Blood samples to determine serum 25-hydroxyvitamin D3 [25(OH)D3] levels were taken at the end of week 4 and week 8. Participants were then given a single full-body exposure of approximately 2 SED from one of four artificial UV sources with different spectral outputs and a further blood sample taken at study completion, nominally week

Trends in late-life activity limitations in the United States: an update from five national surveys.

This article updates trends from five national U.S. surveys to determine whether the prevalence of activity limitations among the older population continued to decline in the first decade of the twenty-first century. Findings across studies suggest that personal care and domestic activity limitations may have continued to decline for those ages 85 and older from 2000 to 2008, but generally were flat since 2000 for those ages 65-84. Modest increases were observed for the 55- to 64-year-old group approaching late life, although prevalence remained low for this age group. Inclusion of the institu
