Prognostic utility of preoperative platelet-to-albumin ratio in surgically treated oral cavity cancer patients.

BACKGROUND: This study aimed to explore the prognostic utility of the preoperative platelet-to-albumin ratio (PAR) among patients with oral cavity squamous cell carcinoma (OSCC). METHODS: We retrospectively reviewed of 355 patients with surgically-treated OSCC between 2008 and 2017. The optimal PAR cutoff for patient stratification was determined through X-tile analysis. Prognostic variables for disease-free survival (DFS) and overall survival (OS) were identified using Cox proportional hazards models. We developed a PAR-based nomogram to predict personalized OS. RESULTS: We determined the o

Predicting age at Alzheimer's dementia onset with the cognitive clock.

INTRODUCTION: Intervention of Alzheimer's dementia hinges on early diagnosis and advanced planning. This work utilizes the cognitive clock, a novel indicator of brain health, to develop a dementia prediction model that can be easily applied in broad settings. METHODS: Data came from over 3000 community-dwelling older adults. Cognitive age was estimated by aligning Mini-Mental State Examination (MMSE) scores to a clock that represents the typical cognitive aging profile. We identified a mean cognitive age at Alzheimer's dementia onset and predicted the corresponding chronological age at person

Masibambane-Ladies Chat: Developing an Online Gender-Enhanced PrEP Information-Motivation Workshop for Young South African Women.

This article describes the processes of transforming an in-person group-based intervention to promote uptake of PrEP among young woman in South Africa to an online interactive "workshop" during the COVID-19 pandemic. Beginning in person and continuing virtually, we used a step-by-step participatory approach with multiple stakeholder groups to develop nine activities to increase knowledge about, as well as motivation and intention to take PrEP, and to address gender-based barriers to PrEP. Activities were informed by our theoretical framework and formative work with young women ages 18-25. We d

Quality assessment, inclusive community development, and collective learning: An institutional perspective from Germany.

This article reviews the German discourse on quality of life, quality assurance, and outcome measurement in services for persons with intellectual disabilities. Following institutional assumptions of path dependencies in organizational development, it is argued that concepts such as quality assurance must be understood in the context of the national support system development. For the Federal Republic of Germany, it can be noted that previous approaches to quality assurance of services based on measurement and evaluation tools have not been the drivers of innovation for inclusion. The driving

Setting priorities for people with intellectual disability/intellectual developmental disorders across the lifespan: a call to action by the World Psychiatric Association.

People with DSM-5 intellectual disability/intellectual developmental disorder (ID/IDD) or ICD-11 disorders of intellectual development (DID) have multiple healthcare needs, but in many countries these needs are neither recognised nor managed effectively. This paper discusses the negative impact that stigma, discrimination and social exclusion have on the identification and care of persons with ID/IDD in low- and middle-income countries (LMICs). It also reviews different models of care for children, adolescents and adults. In discussing some initiatives in LMICs the emphasis is on early diagnos

Tracking the Adaptation and Compensation Processes of Patients' Brain Arterial Network to an Evolving Glioblastoma.

The brain's vascular network dynamically affects its development and core functions. It rapidly responds to abnormal conditions by adjusting properties of the network, aiding stabilization and regulation of brain activities. Tracking prominent arterial changes has clear clinical and surgical advantages. However, the arterial network functions as a system; thus, local changes may imply global compensatory effects that could impact the dynamic progression of a disease. We developed automated personalized system-level analysis methods of the compensatory arterial changes and mean blood flow behav

Between personal and relational privacy: understanding the work of informed consent in cancer genetics in Brazil.

Drawing from perspectives of both bioethics and anthropology, this article explores how the boundaries between personal and relational privacy are negotiated by patients and practitioners in the context of an emerging domain of cancer genetics in Brazil. It reflects on the place of informed consent in the history of bioethics in North America in contrast to the development of bioethics in Brazil and the particular social cultural context in which consent is sought in Brazilian public health care. Making use of empirical research with families and individuals receiving genetic counselling relat

Regulating the use of human bodily material.

The articles in this special issue consider recent developments in the law regulating the use of human bodily material and the wider implications of those developments. For some time, the law has accepted that a person who has undertaken "work and skill" on excised bodily material may obtain at least a possessory right; but the person from whom the material came did not have such a right. Now, however, the law has recognised that people may have some legal rights regarding their own bodily material. What is the nature and source of those rights? Should they be expanded? If so, what legal princ

[Diagnosis and treatment of leprous neuropathy in practice].

Leprosy still affects 240,000 persons every year in the world. It is a particularly common cause of neuropathy and severe disabilities in developing countries. With increasing migration, new cases of leprosy are regularly diagnosed in developed countries, where it still remains rare and so underestimated. Cutaneo-nevritic leprosy is the most frequent form of leprosy. It may be diagnosed by the clinical features and the cutaneous histology and bacteriology. Neuritic leprosy without obvious skin lesions is reported in 5 to 15% of leprosy patients. It must be suspected in persons from areas of en

The Internet as a source of data to support the development of a quality-of-life measure for eating disorders.

Despite the attractiveness of the Internet as a data source on individuals' experiences with health conditions, few have studied its use in quality-of-life instrument development. In this article, the authors describe the use of Internet-based unsolicited first-person narratives to supplement qualitative material derived from other sources (published articles and interviews) in the early stages of development of a quality-of-life instrument for eating disorders. In a systematic Internet search, they identified 31 posted first-person narratives. Sixteen (52%) authors had anorexia nervosa, 11 (3

Hypermetropia is not associated with hypertension: the Blue Mountains Eye Study.

PURPOSE: An association between hypermetropia and hypertension was recently reported. We sought to verify this finding in the Blue Mountains Eye Study cohort (n = 3654; ages 49 to 97 years). DESIGN: Cohort study. METHODS: We defined hypermetropia as mean spheric equivalent refraction (SER) > 1.00 diopter, myopia as mean SER < -1.00 diopters, and emmetropia as mean SER < or = 1.00 diopters and > or = -1.00 diopters, inclusive. We used the 2003 World Health Organization/International Society of Hypertension guidelines to define severe hypertension as grade 2 or higher. RESULTS: Of 1290 people

Changing patterns and trends in systemic fungal infections.

Invasive mycoses are a significant and growing public health problem. Although bloodstream infections with Candida albicans may be decreasing in frequency, the number of persons at risk for them continues to grow. Moreover, infections with other Candida species, such as Candida glabrata, are increasing in incidence. Invasive mould infections in general, and Aspergillus infections in particular, are becoming more frequent. Fungal opportunistic infections in persons with AIDS are no longer a major problem in developed countries, but are resulting in significant morbidity and mortality in develop

Progressive vaccinia.

The resumption of smallpox vaccination for health care workers and other first responders has raised concern about the occurrence of complications in people with immunodeficiency disorders, including those infected with human immunodeficiency virus. During the era of universal vaccination, roughly 1 person per million vaccinees in the general population developed progressive vaccinia, which is characterized by the relentless outward spread of infection from the vaccination site and eventual dissemination to other areas on the body. Review of 56 cases reported in the English-language medical li

[Anthrax as biological warfare weapon].

Anthrax-disease of herbivorous animals, occasionally affecting humans, is regarded as an ideal biowarfare weapon. This was witnessed during a tragic accident in Swierdłowsk some time ago and nowadays it has been observed among post office workers in the USA. Depending on the way the pathogen enters the body, several forms of disease develops: skin anthrax specific for the man (with mortality up to 20%), intestinal anthrax resulting from the consumption of infected food or flash of infected animals (with mortality up to 60-80%) and inhalation pulmonary anthrax (with mortality up to 90%). Prevalence of antibodies to human parvovirus B19 nonstructural protein in persons with various clinical outcomes following B19 infection.

Persistent infections with human parvovirus B19 (B19) associated with debilitating chronic disease have been described, although evidence linking B19 to these more unusual clinical outcomes has been inconclusive. Recent reports have suggested that the development of antibodies to the B19 nonstructural protein (NS1) following B19 infection might be linked to development of severe arthropathy and chronic infection. To confirm these findings, the C-terminal region of the NS1 protein was expressed for use in Western blot assays for detection of anti-NS1 IgG antibodies in human serum. Among 91 pers

Perilymphatic fistula in teens and young adults: emphasis on preexisting sensorineural hearing loss.

Sudden hearing loss and new-onset vestibular disorders can be caused by perilymphatic fistulas (PLFs). In children, the existence of a severe or profound sensorineural hearing loss (SNHL) has been associated with a perilymphatic fistula when a sudden hearing loss or vestibular disturbance occurs. Likewise, at the other end of the age spectrum, several large series of PLFs in adulthood have been presented. This report focuses on a neglected age group, those in their teens and young adulthood. Twenty-three patients between the ages of 17 and 26 were studied who had a preexisting SNHL and had dev

Hallux varus--a study of thirty cases.

Although both congenital and acquired hallux varus have been described, the deformity has been considered uncommon. Contrary to this belief, we noted that it is fairly common in the unshod population. Analysis of 30 cases shows that congenital hallux varus is the most common type encountered and it is of three types, namely: primary, secondary, and teratogenic. The congenital primary and secondary varieties are apparently easy to treat in infancy, but if left untreated, they progress with age in unshod persons. The more severe grades of hallux varus are associated with medial deviation of the

Pneumococcal septicemia in children with sickle cell anemia. Changing trend of survival.

Streptococcus pneumoniae infection has been the predominant cause of death among children with sickle cell anemia (SS). We report our observed change in the pattern of progression of septicemia to meningitis and death in nonimmunized SS children who were not receiving prophylactic penicillin in the face of a persistently high incidence of pneumococcal disease. Of 233 SS children less than ages 6 years observed for 781 person-years, the overall incidence rate of pneumococcal septicemia was 5.9 episodes per 100 person-years. Prior to July 1972, of 23 children who had pneumococcal septicemia, eig

Prevalence of Toxoplasma infection in a cohort of homosexual men at risk of AIDS and toxoplasmic encephalitis.

The purpose of this study was to characterize the epidemiologic, clinical, and laboratory parameters of a cohort of men at risk of AIDS-associated toxoplasmic encephalitis. One hundred seventeen (11%) of the 1,073 participants at the time of enrollment into the Chicago Multicenter AIDS Cohort Study (MACS) were seropositive for Toxoplasma antibodies. Significant differences in prevalence of antibodies between African-American, Hispanic, or white men were not observed (p = 0.49). One hundred one (86%) of the 117 antibody-positive participants had at least one follow-up serology performed and 6 (
