Multimodal Approach for Recalcitrant Melasma Using Picosecond Laser and Topical JAK Inhibition: A Case Report.

BACKGROUND: Melasma is a chronic hyperpigmentation disorder with high relapse rates, particularly in Fitzpatrick skin types III-V. Increasing evidence implicates cytokine-driven inflammation and visible light-induced melanogenesis as key contributors to disease persistence. These mechanisms support the rationale for exploring a multimodal therapeutic approach. CASE PRESENTATION: A woman in her early 30s with phototype IV presented with a 5-year history of recalcitrant centrofacial melasma. Previous treatments-including hydroquinone, oral tranexamic acid, and superficial chemical peels-produce

Atypical Clinical Course of Griscelli Syndrome Type 2 With Primarily Neurologic Presentation and Adult-Onset in a 46-Year-Old Male.

Griscelli Syndrome Type 2 (GS2) is a rare autosomal recessive disorder caused by pathogenic mutations in the RAB27A gene. Typically, it is characterized by cutaneous hypopigmentation, immunodeficiency, with or without neurological abnormalities secondary to hemophagocytic lymphohistiocytosis (HLH). Without treatment, GS2 often results in fatal outcomes within the first decade of life. Adult-onset GS2 is extremely rare, with few documented cases. We describe a 48-year-old patient with a neurological presentation: cerebellar dysarthria, ataxia, nystagmus, and muscle hypotonia in the lower limbs.

RPE Abnormality Is a Potential Primary Cause for Retinal Degeneration in Mucopolysaccharidosis Type VI Patients and a Rat Model.

PURPOSE: Mucopolysaccharidosis type VI (MPS VI) is a rare autosomal recessive lysosomal storage disorder caused by pathogenic variants in ARSB, leading to deficiency of N-acetylgalactosamine 4-sulfatase and accumulation of glycosaminoglycans. Although enzyme replacement therapy (ERT) alleviates systemic symptoms, its efficacy for ocular complications is limited. Because ocular manifestations may require distinct therapeutic approaches, a precise understanding of the underlying retinal pathology is essential. This study aimed to characterize ocular and retinal involvement in MPS VI through clin

Temporal modulation of microglial repopulation attenuates retinal degeneration in retinitis pigmentosa.

Retinitis pigmentosa (RP) is an inherited retinal degenerative disorder characterized by the progressive loss of retinal pigment epithelial cells and rod and cone photoreceptors and irreversible vision loss, with no effective therapies currently available. Microglia, the resident immune cells in the retina, are known to aggravate neurodegeneration when chronically activated. Here, we investigated the impact of temporally controlled microglial depletion and repopulation on retinal degeneration in rd10 mice. Using the CSF1R inhibitor PLX5622, we achieved efficient microglial depletion during the

A Microfluidic Gradient Platform for High-Throughput Evaluation of Blue-Light-Induced Oxidative Stress and Antioxidant Protection in Retinal Pigment Epithelial Cells.

The retinal pigment epithelium (RPE) is a monolayer of cells located between retinal photoreceptors and the choroid, playing a critical role in maintaining visual function by protecting the retina and supporting photoreceptor metabolism. Damage to RPE cells can lead to visual disorders, including macular degeneration. Chronic exposure to high-energy blue light has been shown to elevate intracellular reactive oxygen species (ROS) in RPE cells, causing oxidative stress and cellular damage. In this study, a microfluidic platform incorporating a gradient-generating structure was developed to estab

MiRGD peptideticle targeted delivery of hinge-truncated soluble VEGF receptor 1 fusion protein to the retinal pigment epithelium cell line and newborn mice retina.

Eye-related Angiogenesis and vascular permeability changes lead to retinal vascular disorders. There is an important need to design a novel targeted anti-VEGF drug delivery system to inhibit neovascularization in the retina. The peptide-based carriers are promising for gene therapy due to their flexibility in design, ease of production, structural diversity, low toxicity, and immunogenicity. The hinge-truncated soluble VEGF receptor 1 (htsFLT01) protein, has the ability to bind to both VEGF and PlGF molecules. In the present study, htsFLT01 gene delivery by targeted MiRGD peptide carrier was i

RPGRORF15 Mutations Disrupt Lysosomal Lipid Metabolism in Retinal Pigment Epithelium Cells and Cause Retinitis Pigmentosa.

PURPOSE: X-linked retinitis pigmentosa (XLRP) is a severely blinding retinal disease, most of which are due to mutations in retinitis pigmentosa GTPase regulator (RPGR). The patients with RPGR mutations exhibit severe retinal pigment epithelium (RPE) atrophy and photoreceptor degeneration. Previous research mainly focused on the role of RPGR in the connecting cilia of photoreceptors. However, the mechanism underlying RPE deficiency in patients remains unclear. Moreover, the function of RPGR in RPE cells has not been investigated. METHODS: To investigate the mechanisms underlying RPE atrophy a

Atypical Retinitis Pigmentosa With Macular Sparing in a Patient With Compound Heterozygous ABCA4 Variants: A Case Report and Diagnostic Challenge.

Inherited retinal dystrophies are a complex group of disorders causing progressive vision loss. The ABCA4 gene is associated with a wide spectrum of retinopathies, most commonly Stargardt disease, which is characterized by central macular degeneration. Retinitis Pigmentosa (RP) is a less common but recognized ABCA4-associated phenotype, typically involving severe, pan-retinal degeneration. A 25-year-old male presented with a 5-year history of nyctalopia and progressive peripheral visual field loss. Best-corrected visual acuity was 1/20 in the right eye and 20/20 in the left eye. Acute Stress and Autoimmune Markers: Evaluating the Psychoneuroimmunology Axis in Firefighter Recruits.

Chronic psychological stress is known to influence immune function and contribute to development of autoimmune disorders through dysregulated inflammatory responses. This study investigates relationships between acute stress, life trauma, and autoimmune salivary biomarkers in firefighter recruits during psychophysical stress training. Salivary samples were collected from firefighter recruits during two stress tests to evaluate responses to acute stress. Samples were obtained at three time points-pre-stress, post-stress, and recovery-across both tests. Cortisol was measured to characterize acut

Ultraviolet-tyrosinase cascade caged antisense oligonucleotide for precise treatment of hyperpigmentation.

Hyperpigmentation is a skin disorder characterized by the abnormal accumulation of melanin, in which the melanocortin-1 receptor (MC1R) was recognized as a key target for melanin production. Antisense oligonucleotide (ASO), by targeting mRNA to inhibit protein synthesis, holds a promising gene therapy for hyperpigmentation. However, challenges such as nonspecific activation, off-target toxicity, and limited cellular permeability hinder their broader clinical application. Inspired by the Chinese proverb "Turn an enemy into a friend", we introduce a dual-gated, ultraviolet (UV)-tyrosinase (TYR)

H105A peptide eye drops promote photoreceptor survival in murine and human models of retinal degeneration.

BACKGROUND: Photoreceptor death leads to inherited blinding retinal diseases, such as retinitis pigmentosa (RP). As disease progression often outpaces therapeutic advances, developing effective treatments is urgent. This study evaluates the efficacy of small peptides derived from pigment epithelium-derived factor (PEDF), which are known to restrict common cell death pathways associated with retinal diseases. METHODS: We tested chemically synthesized peptides (17-mer and H105A) with affinity for the PEDF receptor, PEDF-R, delivered as eye drops to two RP mouse models: rd10 (phosphodiesterase 6

Preserving blood-retinal barrier integrity: a path to retinal ganglion cell protection in glaucoma and traumatic optic neuropathy.

Retinal ganglion cells (RGCs) are the visual gateway of the brain, with their axons converging to form the optic nerve, making them the most vulnerable target in diseases such as glaucoma and traumatic optic neuropathy (TON). In both diseases, the disruption of the blood-retinal barrier(BRB) is considered an important mechanism that accelerates RGC degeneration and hinders axon regeneration. The BRB consists of the inner blood-retinal barrier (iBRB) and the outer blood-retinal barrier (oBRB), which are maintained by endothelial cells(ECs), pericytes(PCs), and retinal pigment epithelial (RPE),

The Programme for Manufacture of hPSC‑Based Products for AMD and PD.

Pluripotent stem cells (hPSCs) possess the unique ability to self-renew and differentiate into various functional cell types, positioning them as key "seed cells" in regenerative medicine. The development of PSC-based therapies offers new hope for treating complex diseases, and degenerative diseases, particularly those caused by the loss of specific cellular functions, such as macular degeneration, Parkinson's disease, spinal cord injury, diabetes, and cartilage damage. Cell replacement therapies using hPSC-committed cells can help halt disease progression or potentially cure these conditions.

Multicentre, prospective, observational clinical registry of paediatric and adult patients with vitiligo on phototherapy, or novel topical or systemic immunomodulatory therapies.

Vitiligo is the most common depigmentation disorder, affecting around 1% of the population worldwide. Currently, no national or international registry for patients with vitiligo exists. Individual dermatologists maintain descriptive databases of such patients, which involve patients in one centre. However, such single-centre registries do not aim to establish the short- and long-term safety (pharmacovigilance) and effectiveness of vitiligo therapies. As an initial step for the development of a national UK registry for patients with vitiligo, development of a pilot multicentre registry in the U

Parkin-Mediated Ubiquitination of DCUN1D1: Implications for CXCL10 Regulation in Vitiligo.

Vitiligo is a skin disorder marked by the loss of pigmentation due to melanocyte destruction. Our previous study demonstrated that DCUN1D1 is a novel regulator of CXCL10 and is associated with mitochondrial dysfunction. RNA-seq data revealed that the expression of Parkin, a mitophagy-related molecule, is decreased in vitiligo patients. Parkin functions as an E3 ubiquitin ligase. Based on these findings, we hypothesize that Parkin ubiquitinates DCUN1D1, thereby influencing the level of CXCL10 during the progression of vitiligo. Parkin and DCUN1D1 expression levels were assessed in both vitiligo

Modeling aging and retinal degeneration with mitochondrial DNA mutation burden.

Somatic mitochondrial DNA (mtDNA) mutation accumulation has been observed in individuals with retinal degenerative disorders. To study the effects of aging and mtDNA mutation accumulation in the retina, a polymerase gamma (POLG) exonuclease-deficient model, the PolgD257A mutator mice (D257A), was used. POLG is an enzyme responsible for regulating mtDNA replication and repair. Retinas of young and older mice with this mutation were analyzed in vivo and ex vivo to provide new insights into the contribution of age-related mitochondrial (mt) dysfunction due to mtDNA damage.
