Surgical outcome of mycobacterium other than mycobacterium tuberculosis pulmonary disease.

Between January 1995 and May 1999, MOTT were cultured from sputum, bronchoalveolar lavage or resected lung specimens in 110 cases. 17 patients with MOTT pulmonary disease underwent pulmonary resection. Preoperatively, 5 of 17 patients had been diagnosed with MOTT pulmonary disease. The diagnosis of others was based on positive cultures from surgically resected material, and organism identification was successfully performed by the microplate DNA-DNA hybridization procedure. Surgical resections performed included wedge resection in 7, lobectomy in 6, and segmentectomy in 4. Antibiotics were gen

[Solitary adrenal metastasis of pulmonary cancer].

We describe the case of a 46-year-old man with lung cancer and simultaneous solitary adrenal metastases. Adrenalectomy was performed 12 weeks after lung resection through a right subcostal laparotomy. Treatment was complemented with chemotherapy. Twelve months after adrenalectomy the patient was found free of signs of disease and was in satisfactory condition. The advantages of and indications for surgical resection of suprarenal metastasis are discussed in the light of published literature. In some cases, survival may improve with exeresis and chemotherapy.

A multidisciplinary approach to primary nonseminomatous germ cell tumors of the mediastinum.

The introduction of cis-platinum-based chemotherapy has dramatically improved the prognosis for patients with primary nonseminomatous germ cell tumors of the mediastinum. Since 1978, 12 male patients (mean age, 29 years) have been seen with a large mediastinal mass, normal testes, and abnormal testicular tumor markers. Eleven patients had raised alpha-fetoprotein levels (median, 1,300 micrograms/L; normal, less than 10 micrograms/L), and 3 had elevated levels of the beta fraction of human chorionic gonadotropin (median, 8,000 IU/L; normal, less than 5 IU/L). Two patients were treated by primar

A follow-up examination of patients with pulmonary tuberculosis resected on suspicion of tumour.

Thoracotomy was performed in 91 patients who proved to have pulmonary tuberculosis. A short course of antituberculous chemotherapy was given preoperatively in 66 cases. The surviving 89 patients received antituberculous chemotherapy postoperatively, and were-re-examined from 1 to 14 years after the operation. No signs of dissemination or other tuberculous manifestations were found 1 year after termination of chemotherapy, even if preoperative treatment had not been given. One recurrence was seen 7 years after operation despite 9 months postoperative antituberculous chemotherapy. This was the o

Postnatal growth and development of the lung.

The lung is qualitatively different at birth, compared to adult life, in many species. In the neonatal period, the peripheral part of the acinus is formed by primary saccules, and it is from the walls of these that alveoli develop postnatally in rats, mice, and rabbits. Alveolar development starts in utero in man and probably in several other species, including sheep, cat, and, probably, mammals with relatively advanced physical development at birth. A sequence of events is well established in rats and mice in the postnatal period. A phase of expansion first occurs, during which lung growth la

The Necessity of Lymphadenectomy for Non-Small Cell Lung Cancer Smaller Than 6 mm in Solid Size.

BACKGROUND: This study aimed to evaluate the necessity of lymph node sampling for specific non-small cell lung cancer (NSCLC). METHODS: Patients with small-size (≤2 cm) NSCLC who underwent surgical resection between 2009 and 2022 were retrospectively screened. The characteristics of patients with nodal metastasis were demonstrated. For selected patients, the perioperative and long-term outcomes with and without lymphadenectomy were compared. RESULTS: Of the 2713 patients enrolled in this study, 75 (2.76 %) had involvement of nodules with a minimum solid size of 6 mm. No patient with pure gro

[A Case of Long-Term Prognosis of Advanced Rectosigmoid Cancer with Multiple Metastases Treated with Multidisciplinary Therapy].

The case was 50-year-old woman, who was diagnosed with advanced rectosigmoid cancer with multiple liver metastases and solitary lung metastasis. She was started with chemotherapy. Two courses of mFOLFOX6+panitumumab and 8 courses of mFOLFOX6+bevacizumab were administered as a first-line. As a second-line, 1 course of FOLFIRI+ramucirumab was given. Six months later, CT showed that lung metastasis had disappeared and all liver metastases had become smaller, therefor surgery was selected. Laparoscopic partial hepatectomy was performed, and 2 months later, laparoscopic high anterior resection was

Comparison of Outcomes Between Ablation and Lobectomy in Stage IA Non-Small Cell Lung Cancer: A Retrospective Multicenter Study.

BACKGROUND AND OBJECTIVE: Image-guided thermal ablation (IGTA) has been increasingly used in patients with stage IA non-small cell lung cancer (NSCLC) without surgical contraindications, but its long-term outcomes compared to lobectomy remain unknown. This study aims to evaluate the long-term outcomes of IGTA versus lobectomy and explore which patients may benefit most from ablation. METHODS: After propensity score matching, a total of 290 patients with stage IA NSCLC between 2015 and 2023 were included. Progression-free survival (PFS) and overall survival (OS) were estimated using the Kaplan

Uniportal Video-Assisted Anatomical Lung Volume Reduction Surgery in Severe Emphysema.

Lung volume reduction surgery (LVRS) is an important treatment option for patients with advanced emphysema and is typically performed in a non-anatomical fashion. This study reports the outcome of anatomical LVRS by means of uniportal video-assisted thoracoscopic surgery (VATS).We retrospectively evaluated patients who underwent anatomical LVRS between June 2017 and September 2023 at our institution. Patient characteristics, including demographic data, lung function, as well as morbidity and mortality, were extracted from hospital records.A total of 44 patients (17 males, 38.6%) underwent anat

Effect of preoperative cognitive and physical optimization in the prevention of postoperative cognitive deficit in elderly patients with lung resection - cognition trial.

BACKGROUND: Postoperative cognitive dysfunction (POCD) can develop from 7 days to 1 year after surgery. It is a long-lasting disorder that affects various cognitive domains, including memory, executive function, attention, verbal fluency, and visuospatial performance. Mild symptoms are often overlooked or misinterpreted as signs of normal ageing. The impact of POCD on recovery and quality of life after surgery is not fully understood. This study aims to assess the effects of preoperative cognitive and physical training on postoperative cognitive outcomes and recovery in elderly patients underg

Controlling nutritional status predicts postoperative survival and acute exacerbation in resected non-small cell lung cancer with interstitial lung disease.

BACKGROUND: Several immune-nutritional markers have been reported as convenient prognostic factors in malignancies, including lung cancer. The controlling nutritional status (CONUT) score has been reported as a prognostic factor in patients with resected lung cancer. However, its significance in resected cases of lung cancer complicated by interstitial lung disease (ILD) has not been previously reported. METHODS: This study included 120 patients with non-small cell lung cancer complicated by ILD who underwent lung resection at our department between 2010 and 2019. Based on the CONUT score, pa

Evaluating the necessity of lymph node sampling in lung adenocarcinoma with ground glass opacities.

BACKGROUND: Ground glass opacity is observed frequently in the early stages of lung adenocarcinoma and is associated with a favorable prognosis and a low incidence of lymph node metastasis. However, the necessity of lymph node sampling in these patients is questionable, although current guidelines still recommend it. METHODS: Radiologic and clinical data were retrospectively collected and analyzed for 2,298 patients with lung cancer who underwent surgical resection for lesions ≤15 mm during 2022. Based on the consolidation tumor ratios, patients were categorized into 4 groups (pure ground gla

Retention Rate of Free Pericardial Fat Grafts after Bronchial Stump Coverage.

The postoperative course of the graft tissue after bronchial stump coverage remains unclear. We retrospectively analyzed 44 patients who underwent anatomical lung resection followed by bronchial stump coverage using free pericardial fat grafts. All patients underwent minimally invasive video-assisted thoracoscopic surgery. Computed tomography scans showed a graft retention rate of 100% on 60 days after surgery, 61% on 180 days, and plateauing at around 20% after 1 year. Free pericardial fat grafts, harvested minimally invasively, demonstrated a promising retention rate after surgery, making th
