Histomorphic analysis of the development of the pars interarticularis and its association with isthmic spondylolysis.

STUDY DESIGN: Descriptive study analyzing the histologic development of human fetal lumbar spine pars interarticularis. OBJECTIVES: To delineate the histologic development of the fetal pars interarticularis and correlate the development of the pars with isthmic spondylolysis. SUMMARY OF BACKGROUND DATA: Isthmic spondylolysis is present in 5% to 6% of the population. The incidence at birth is zero but rises sharply to 5% at age 4 to 5 years. The origin of the lesion is unknown; however, the current theory states that isthmic spondylolysis results from a fatigue fracture through a congenitally

Clinical spectrum of female genital malformations in prenatal diagnosis.

INTRODUCTION: Fetal genital malformations represent a rare and heterogeneous group of congenital malformations of the disorders of sexual development (DSD) spectrum. METHODS: A thorough literature review on the main topics in the prenatal approach towards DSD was conducted. RESULTS: First, a thorough overview on prenatal characteristics of the most common fetal genital malformations of ovaries, uterus and external genitalia, and second, a standardized approach for differential diagnosis in the presence of direct and indirect prenatal signs of DSDs. CONCLUSIONS: This review is mainly directe

An Overview of Obesity, Cholesterol, and Systemic Inflammation in Preeclampsia.

Preeclampsia (PE), an inflammatory state during pregnancy, is a significant cause of maternal and fetal morbidity and mortality. Adverse outcomes associated with PE include hypertension, proteinuria, uterine/placental abnormalities, fetal growth restriction, and pre-term birth. Women with obesity have an increased risk of developing PE likely due to impaired placental development from altered metabolic homeostasis. Inflammatory cytokines from maternal adipose tissue and circulating cholesterol have been linked to systemic inflammation, hypertension, and other adverse outcomes associated with P

Prenatal hypocalvaria after prolonged intrauterine exposure to angiotensin II receptor antagonists.

We report a case of prenatal exposure to angiotensin II receptor antagonists (ARA II) from the beginning of pregnancy in a patient with a hypokinetic dilated cardiomyopathy. This case report emphasizes the fetal renal impact of prolonged intrauterine exposure to renin-angiotensin system (RAS) blockers, and highlights that this exposure can cause severe prenatal hypocalvaria. This delayed ossification can be reversible after birth, but the presence of anhydramnios indicates an early and irreversible block of RAS blockers in the fetus that is responsible for fetal kidney development abnormalitie

Red Blood Cell Alloimmunization in the Pregnant Patient.

Alloimmunization to red blood cell (RBC) antigens represents a challenge for physicians caring for women of child bearing potential. Exposure to non-self RBC antigens may occur during transfusion or pregnancy leading to the development of antibodies. If a subsequent fetus bears that antigen, maternal antibodies may attack the fetal red blood cells causing red cell destruction and clinically significant hemolytic disease of the fetus and newborn (HDFN). In the most severe cases, HDFN may result in intrauterine fetal demise due to high output cardiac failure, effusions and ascites, known as "hyd

A novel MYH7 gene mutation in a fetus with left ventricular noncompaction.

Left ventricular noncompaction (LVNC) is a recently defined cardiomyopathy characterized by a pattern of prominent trabecular meshwork and deep intertrabecular recesses. LVNC is rarely described in fetal life, and a small number of cases have been reported. We report the first fetal case, to our knowledge, of LVNC associated with a novel mutation in the MYH7 gene (c.1625A>C; p.Lys542Thr). This patient showed cardiomegaly on prenatal ultrasonographic examinations, with features indicating noncompaction of the myocardium apparent in the second trimester. This case highlights the importance of pr

Stem and progenitor cell dysfunction in human trisomies.

Trisomy 21, the commonest constitutional aneuploidy in humans, causes profound perturbation of stem and progenitor cell growth, which is both cell context dependent and developmental stage specific and mediated by complex genetic mechanisms beyond increased Hsa21 gene dosage. While proliferation of fetal hematopoietic and testicular stem/progenitors is increased and may underlie increased susceptibility to childhood leukemia and testicular cancer, fetal stem/progenitor proliferation in other tissues is markedly impaired leading to the characteristic craniofacial, neurocognitive and cardiac fea

A safe strategy to decrease fetal lead exposure in a woman with chronic intoxication.

During pregnancy skeletal lead is mobilized by maternal bone turnover and can threaten fetal development. The exact strategy suggested to women of childbearing age, who were chronically exposed to lead, and, thus, have high bone lead burden, is not well established. We describe 4 years of follow-up of a 29-year-old woman with chronic lead intoxication. We (a) advised her to delay conception until 'toxicological clearance', (b) treated her with multiple courses of lead chelator, DMSA, and (c) prescribed oral calcium. Patient had low blood lead and protoporphyrin level during pregnancy until del…

Placental abnormalities in ovine somatic cell clones at term: a light and electron microscopic investigation.

To investigate the reasons for fetal losses after somatic cell nuclear transfer, an immunohistochemical and ultrastructural analysis of cloned placentae was performed. The main features observed were a marked reduction of villous vascularization, hypoplasia of trophoblastic epithelium, lack of binucleate cells, immaturity of placental vessels and reduced vasculogenesis. By means of transmission electron microscopy (TEM), a diffuse thickening and lamination of subtrophoblastic basement membrane (SBM) were noted in cloned placentae. These results led us to hypothesize, through an autoamplificati

Prune-belly syndrome diagnosed at 14 weeks' gestation with severe urethral obstruction but normal kidneys.

A fetus was diagnosed with prune-belly syndrome (PBS) found by ultrasound demonstration of cystomegaly at 12 weeks' gestation. Fetal urinary electrolyte and beta2-microglobulin concentrations at 14 weeks' gestation indicated a poor renal prognosis. The pregnancy was therefore terminated. An autopsy showed defects of the abdominal wall muscles and megalocystis caused by severe urethral obstruction. However, the kidneys were histologically normal. Together with previous reports, the present case suggests that renal dysplasia in PBS with severe uretheral obstruction may develop after 14 weeks' ge

Fetal alcohol syndrome: craniofacial and central nervous system manifestations.

Magnetic resonance imaging (MRI) is undertaken on fetal alcohol syndrome (FAS) subjects to document central nervous system (CNS) anomalies. The abnormalities found include agenesis and hypoplasia of the corpus callosum, cavum septi pellucidi, cavum vergae, ventriculomegaly, hypoplasia of inferior olivary eminences, small brain stem, and micrencephaly. Craniofacial anomalies range from the well-recognized FAS physiognomy to the more severe frontonasal "dysplasia" (median cleft face). CNS and craniofacial abnormalities are predominantly symmetric and central or midline. The association of these

Evidence for decidua-trophoblast interactions in early human pregnancy.

Medium conditioned by decidual cells decreased the growth of cultured BeWo choriocarcinoma cells. The degree of inhibition was dependent on the concentration of the conditioned medium used, and suggested that maternal decidua might regulate the growth of the fetal placenta. Medium from BeWo cells and primary trophoblast had the opposing effect and increased the growth of cultured decidual cells for up to 120 h of culture. These results suggest that a regulatory loop to control placental and decidual growth exists at the materno-fetal interface, and this may be an important factor in the develo

Genetic counselling for trisomy 20 mosaicism in amniotic fluid cell cultures.

Trisomy 20 mosaicism in amniotic fluid cell cultures has an obscure origin and significance. In only a few cases has the mosaicism been confirmed in fetal tissues, but never in neonatal tissues. There is some indication that the trisomic cells might originate from epithelial cells deriving from the urinary tract. A review of 20 demonstrated cases reveals that no consistent malformation pattern is associated with this finding. Since it is questionable whether the presence of this mosaicism in amniocentesis can be regarded as a manifestation of abnormal fetal development, we suggest that it does…

[2 cases of malformations of a limb in infants of mothers treated with an antiemetic in a very early phase of pregnancy].

Two cases of fetal malformation of a limb are described. The mothers of our patients took an antiemetic, Debendox (dicyclomine hydrochloride 10 mg, doxylamine succinate 10 mg, pyridoxine hydrochloride 10 mg), in early pregnancy, five-six weeks after the last menstrual period. The drug was administered during limbs-buds development period which is thought to occur at 43-46 days after conception. The fetal malformation described, are confronted with congenital abnormalities reported by other authors who suspect that, in early pregnancy, Debendox may have teratogenic properties. Nevertheless, pro

Sex-specific signatures of fetal programming for gestational diabetes mellitus in umbilical cord: A cohort study from Tanzania.

BACKGROUND: Gestational diabetes mellitus (GDM) is defined as glucose intolerance first recognized during pregnancy. Offspring of GDM mothers have an increased risk of developing type 2 diabetes (T2D), obesity, and other metabolic disorders later in life. METHODS: We analyzed metabolic, genetic, epigenetic, and transcriptional profiles in umbilical cord blood (UCB) from male and female offspring of GDM mothers versus normoglycemic controls in Tanzania. We constructed polygenic and methylation risk scores for T2D and GDM to assess whether diabetes-related susceptibility is detectable at birth.

Cell-free DNA in maternal blood and artificial intelligence: accurate prenatal detection of fetal congenital heart defects.

BACKGROUND: DNA cytosine nucleotide methylation (epigenomics and epigenetics) is an important mechanism for controlling gene expression in cardiac development. Combined artificial intelligence and whole-genome epigenomic analysis of circulating cell-free DNA in maternal blood has the potential for the detection of fetal congenital heart defects. OBJECTIVE: This study aimed to use genome-wide DNA cytosine methylation and artificial intelligence analyses of circulating cell-free DNA for the minimally invasive detection of fetal congenital heart defects. STUDY DESIGN: In this prospective study,

Socio-Emotional and Cognitive Development in Intrauterine Growth Restricted (IUGR) and Typical Development Infants: Early Interactive Patterns and Underlying Neural Correlates. Rationale and Methods of the Study.

Intrauterine growth restriction (IUGR) is defined as a fetal growth retardation, resulting in an estimated fetal weight less than the 10th centile for gestational age. IUGR developing brain is affected by the atypical fetal growth, presenting altered structure and connectivity and increased risk for neurodevelopmental impairments. Behaviorally, IUGR infants show reduced responsiveness and engagement with human faces during mother-child exchanges. The neural mechanisms of these patterns of interactions remain unexplored, as well as their potential role in shaping socio-cognitive trajectories of

The impact of maternal smoking on embryonic morphological development: the Rotterdam Periconception Cohort.

STUDY QUESTION: Is periconceptional maternal smoking associated with embryonic morphological development in ongoing pregnancies? SUMMARY ANSWER: Smoking during the periconceptional period is associated with a delayed embryonic morphological development which is not fully recuperated beyond the first trimester of pregnancy. WHAT IS KNOWN ALREADY: Smoking during pregnancy decreases prenatal growth, increasing the risk of preterm birth, small for gestational age (GA) and childhood obesity. STUDY DESIGN, SIZE, DURATION: Between 2010 and 2018, 689 women with ongoing singleton pregnancies were pe

TLR7 alters the maternal immune landscape during influenza A infection to increase maternal and fetal morbidity.

Pregnant women infected with influenza A virus (IAV) are at increased risk of severe disease, leading to maternal and fetal complications. Toll-like receptor 7 (TLR7) recognizes single-stranded RNA viruses, including IAV, yet its role in maternal immune responses and pregnancy outcomes during IAV infection is poorly understood. Here, we demonstrate that TLR7-knockout (TLR7-/-) pregnant mice showed reduced disease severity, despite similar pulmonary viral titers to wild-type (WT) mice. TLR7-/- dams exhibited distinct pulmonary responses, including reduced lymphocyte infiltration, enhanced neutr
