Dupuytren's disease may extend beyond the wrist crease in continuity.

Debate continues about the origin of Dupuytren's disease, which is usually in the palm but is seen elsewhere as ectopic lesions. We describe a young patient with Dupuytren's disease extending proximal to the wrist crease in continuity with the palmar lesion. Our findings support the view that the condition starts within the palmar connective tissue, but there is no palmar aponeurosis in the forearm and the proximal extension probably started in the deep layer of the superficial fascia.

Herpes associated erythema multiforme annularis concentricum.

A young man had large, concentric plaques over the back and extensors of forearms for eight weeks. Past history suggested recurrent herpes labialis. Serum anti HSV-1 IgG titre was raised to 32 micrograms/l and histopathology of an active lesion suggested erythema multiforme. Symptomatic treatment and oral zinc therapy subsided EM lesions but concentric depigmentation developed after healing.

Treatment of post-replantation retardation of bone growth by callus distraction. A report of two cases.

Hand and forearm replantation in children may lead to premature closure of the distal radial or ulnar growth-plates. Following successful replantation, bone growth may be retarded, resulting in a length discrepancy between the radius and ulna with secondary deviation of the replanted hand. We present two cases in which callus distraction was employed to correct a length discrepancy. The relative merits of the various treatment options are discussed.

Vascularised fasciocutaneous flap for reconstruction of the hypopharynx.

The hypopharyngeal mucosa is a thin, pliable lining, which often needs replacement after tumor excision, stenosis and fistulae. The pectoralis major flap (Ariyan and Krizek, 1977) has brought an element of safety and reliability to one-stage pharyngeal reconstruction. The major disadvantage of these flaps is their bulk (2). The vascularised fasciocutaneous flap (especially the radial forearm flap) is a good alternative to be used in pharyngeal repair.

[Free transplantation of skin, underlying fatty tissue and muscle after traumatic loss (author's transl)].

Large extended defects of the soft parts can be covered by free transplantation of the skin and the underlying fatty tissue with the aid of microsurgical techniques. Likewise lacking functionally important muscle areas as the forearm flexor are restored by the free transplanted latissimus dorsi muscle with good function as described in this paper. Indications, operative possibilities, surgical techniques and clinical results are presented and discussed in respect of own case reports.

Transient subcutaneous granulomatosis of the upper extremities in Crohn's disease.

Skin lesions occur fairly commonly in patients with Crohn's disease of the colon (14%). Erythema nodosum and pyoderma gangrenosum are the most frequently recognized. Granulomatosis of the skin, continuous with bowel involvement, has been reported. Involvement of the subcutaneous tissue, discontinuous with intestinal disease, is unusual and we wish to report a case of Crohn's disease of the colon complicated by infraepidermal perivascular granulomas of the forearms. The lesions disappeared spontaneously.

The pathogenesis of congenital radial head dislocation/subluxation.

The pathogenesis of congenital radial head dislocation/subluxation is unknown and has not been previously investigated. In this review, we explore the pathogenesis and define five different primary insults: collagen abnormalities, abnormal endochondral ossification of the developing growth plate, abnormalities of forearm ossification outside the growth plate, disproportionate growth of the radius and ulna, and altered HOX D expression/activity. Finally, the clinical relevance of our review is discussed.

Gigantobilharzia, possible cause of cercarial dermatitis: Case report.

Cercarial dermatitis (swimmer's itch) is a worldwide, often neglected parasitic skin disease characterized by strong maculopapular skin eruption accompanied by intensive itching. A fisherman suffered from forearm dermatitis. Clinical history associated with the recovery of the avian schistosome; Gigantobilharzia from little green bee-eater (Merops orientalis najdanus) and collected Lymnaea snails supported the authors' opinion that patient clinical signs are most likely due to the invasion of avian schistosome cercariae.

Catastrophic complication following heparin therapy.

Heparin-induced thrombocytopenia is a rare but serious and life-threatening complication of heparin use. We describe the case of a 7-year-old girl with tetralogy of Fallot with pulmonary atresia, who developed multiple thrombotic complications following intracardiac repair, requiring below elbow amputation of the left forearm and right forefoot. Our case illustrates the need for greater awareness of heparin-induced thrombocytopenia, to prevent the significant morbidity associated with it.

Attritional rupture of extensor pollicis longus: a rare complication following elastic stable intramedullary nailing of a paediatric radial fracture.

Elastic stable intramedullary nail fixation has become established as an acceptable method of treatment for diaphyseal fractures of both forearm bones in the paediatric population. It is considered safe, minimally invasive and does not compromise physeal growth. We report a case of delayed rupture of extensor pollicis longus due to attrition over the sharp edges of a protruding nail end after elastic stable intramedullary nailing of a paediatric radial diaphyseal fracture.

Invasive squamous cell carcinoma with sporotrichoid metastasis in a patient with cutaneous T cell lymphoma treated with chronic extracorporeal photopheresis.

An 83-year-old Caucasian man with cutaneous T-cell lymphoma developed an aggressive squamous cell carcinoma of the left forearm, which recurred and metastasized after Mohs micrographic surgery and systemic chemotherapy with cis-platin and 5-fluorouracil. He was treated with extracorporeal photopheresis, radiation therapy, PUVA photochemotherapy, and interferon therapy for cutaneous T-cell lymphoma. Aggressive squamous cell carcinoma can occur in the setting of extracorporeal photopheresis.

Unilateral distal extremity swelling with pitting oedema in giant cell arteritis.

We describe a case of an 82-year-old woman with giant cell arteritis, who developed unilateral pitting oedema of the dorsum of the right hand, foot and distal part of the right forearm as well as tenosynovitis of the right wrist extensors, mild synovitis of the right wrist, metacarpophalangeal and metatarsophalangeal joints, during relapse of giant cell arteritis. Increase of corticosteroids resolved the giant cell arteritis, pitting oedema and tenosynovitis.

A lesion of the musculocutaneous nerve. A rare complication of anterior shoulder dislocation.

A 47-year-old male patient with an anterior shoulder dislocation underwent reduction 3 days later. A residual neurologic injury remained, consisting primarily in weakness of shoulder and elbow flexion and sensory involvement of the ventral side of the forearm. Secondary sequelae included muscle atrophy with a concomitant frozen shoulder. The clinical examination and electromyography revealed a lesion of the musculocutaneous nerve, which is a very rare complication in shoulder dislocation.

Acromesomelic dwarfism: manifestations in childhood.

Acromesomelic dwarfism is a distinct condition characterized by short stature of the short limb type, with the hands and feet showing the most obvious deviations from normal. The forearm bones are usually disproportionately shorter than the other long tubular bones of the limbs. The intelligence is normal. Available data suggest autosomal recessive transmission. Characteristic clinical and radiographic features permit establishment of a confident diagnosis in the first year of life.

An Analysis of 1000 Patients With the "Big 5" Orthopaedic Surgery Procedures and the Impact of Residents on Outcome.

BACKGROUND: The study is intended to show that the operative quality of a resident in orthopedic trauma surgery is comparable to that of a senior physician in the most common orthopaedic trauma surgeries (Plate osteosynthesis in ankle fractures and distal radius fractures, ESIN in pediatric forearm fractures, implantation of a proximal femoral nail in pertrochanteric femur fractures and hemiarthroplasty in femoral neck fractures) with appropriate supervision by a senior physician. With only minimal deviations in the operating time, which is becoming increasingly relevant in everyday clinical p…

The Reversed Palmaris Longus: Sonographic Findings and Anatomical Correlation With Implications for Carpal Tunnel Syndrome Diagnosis and Management.

Anomalous muscles of the forearm, including a reversed palmaris longus, may present as masses or median nerve compression syndromes, such as carpal tunnel syndrome. In both cases, ultrasound may be used as a first-line diagnostic test. This report presents a case of a reversed palmaris longus with sonographic findings and anatomic correlation. The diagnosis of these muscular anomalies, their relationship to compression syndromes, and implications for surgical planning are discussed.

Anomalous Flexor Digitorum Superficialis Muscle Belly Associated with Carpal Tunnel Syndrome.

Anomalous muscle bellies in the forearm generally are asymptomatic and appreciated in an academic sense during cadaveric dissections. Few prior anatomic variations in muscle bellies have been described with symptoms, and are associated even more rarely with carpal tunnel syndrome (CTS). We discuss the evaluation and management of a case of CTS associated with a muscle belly of the flexor digitorum superficialis to the index within the carpal tunnel.

Concomitant Fracture Radial Head and Shaft With Dislocation of the Distal Radioulnar Joint: A Rare Variant of the Essex-Lopresti Lesion.

The Essex-Lopresti lesion is a challenging injury both for diagnosis and management. These often tend to be missed, resulting in incapacitating pain and joint stiffness despite treatment. In rare circumstances, they may occur in association with other injuries of the forearm or elbow. We describe an atypical Essex-Lopresti variant comprising a radial head and shaft fracture with an associated distal radioulnar joint dislocation in a 36-year-old man.

A curious finding of skin blebs: A case report.

Mannitol has been the cornerstone of osmotherapy in the treatment of raised intracranial pressure for the past several decades. We discuss here a case of subcutaneous mannitol extravasation, leading to bullous eruptions and swelling in the forearm of a postoperative patient of arteriovenous malformation. We emphasize the importance of careful selection of peripheral intravenous catheter site, especially when infusing hypertonic solutions with propensity for subcutaneous leaks and tissue damage.

Home-made explosive found inside injured Afghan.

There is extensive literature on metal fragments from improvised explosive devices being embedded in patients but there are no reports describing the clinical and radiological appearances of embedded home-made explosive (HME). We present a case of partially detonated HME being found inside a patient's forearm. We discuss the medical management of the injury, the ongoing risk to the patient and surgical team associated with the explosive and the safe disposal of the substance.
