OBJECTIVE: To investigate the efficacy of surgical method with the soleus muscle flap for repairing the soft-tissue defects in the middle and distal thirds of the leg, combined with open tibial fracture and/or osteomyelitis.

Burn contractures are common after deep burns of the limbs. When rehabilitation fails to restore joint function and normal range of motion, surgical contracture release should be performed. Different plastic surgery techniques can be used, from Z-plasty to skin grafts, artificial dermis and flaps. Surgery can also be used in case of esthetic concerns. Nineteen patients with complex injuries of the lower extremities were treated with emergency free flap reconstruction after radical débridement, bone stabilisation, and primary repair of injured structures (nerves, tendons, vessels, and muscles). Six long-term nursing home residents who experienced fractures of the long bones in the absence of obvious trauma are described. All patients had been essentially non-weight-bearing for at least 2 years before the episode. Four patients experienced fractures on more than one occasion.
