OBJECTIVE: To test the hypothesis that the decline in femoral bone mass associated with healthy aging is partially accounted for by deficiency of the growth hormone-insulin-like growth factor-I (IGF-I) axis.
DESIGN: Cross-sectional study.
STUDY PARTICIPANTS: A sample of 245 community-dwelling healthy women aged 70 and older. Exclusion criteria were diseases or medications known to affect the musculoskeletal system or the somatotrophic axis.
MEASUREMENTS: Serum levels of IGF-I, calcitriol, and osteocalcin were determined by radioimmunoassay; serum calcidiol was measured by competitive bindin…

Historically, arthritis was the main symptom which led to the description of the disease called Lyme borreliosis. However, a relatively high awareness of doctors and patients of tick-borne diseases seems to cause a trend to frequently diagnose this antibiotic-sensitive disease. A case can be defined as borreliosis only if either the typical erythema migrans is reliably identified by a physician or if a characteristic late manifestation of Lyme disease is accompanied by unequivocal serological and/or bacteriological evidence of Borrelia infection. Within the musculoskeletal system, the only rel…

The adeno-associated virus (AAV) has become the vector of choice for gene therapy and experimental gene delivery, owing to its non-pathogenic nature and ability to achieve persistent gene expression across diverse tissues. AAV has emerged as a key platform in cellular reprogramming and regenerative medicine, with applications spanning transcription factor delivery for in vivo lineage conversion and tissue repair across the CNS, heart, and musculoskeletal systems. However, significant limitations remain, particularly in the context of induced pluripotent stem cell (iPSC) engineering. We assess …

Cellular senescence is a context-dependent cellular state characterised by persistent cell-cycle arrest, epigenetic remodelling, metabolic reprogramming and acquisition of a senescence-associated secretory phenotype. Transient senescence contributes to embryogenesis, tissue repair and tumour suppression, whereas persistent senescent cell populations accumulate with advancing age across multiple tissues, in part owing to declining immune-mediated clearance and intrinsic resistance to apoptosis, thereby promoting chronic systemic inflammation, tissue fibrosis, stem-cell dysfunction and propagati

Background, Clinical Significance: Anterior cruciate ligament (ACL) injuries are common, however the occurrence of concurrent gastrocnemius muscle tears is exceptionally rare. Given this, the diagnosis and management of this pattern of injury is poorly characterised, with lack of current clinically relevant classification systems and evidence-based guidelines to guide treatment. Early recognition is essential. with advanced imaging critical to guiding the diagnosis and management of patients with this pattern of injury. Case presentation: A 39 year old man presented with acute right knee swell

Survivors of bilateral retinoblastoma are at significantly increased lifetime risk of secondary malignancies, particularly osteosarcoma. While radiotherapy further amplifies this risk, osteosarcoma may develop in non-irradiated patients due to underlying genetic susceptibility. A 16-year-old female was referred for evaluation of progressive left knee swelling following minor trauma. Her past medical history was significant for bilateral retinoblastoma diagnosed in infancy, managed with right eye enucleation and systemic chemotherapy, without exposure to radiotherapy. She had remained in comple

Frailty in chronic kidney disease (CKD) is increasingly recognized as a manifestation of accelerated biological aging, characterized by systemic impairments that extend beyond the musculoskeletal domain. A distinctive yet underappreciated feature of CKD-associated frailty is its triple burden-the co-existence of sarcopenia, bone fragility, and cognitive impairment. Rather than isolated phenomena, these deficits share convergent pathophysiological drivers, including uremic toxicity, oxidative stress, hormonal dysregulation, and chronic inflammation. Accumulating evidence highlights the role of

Arthritis is one of the most prevalent chronic musculoskeletal disorders worldwide, affecting more than 300 million individuals and representing a leading cause of pain, disability, and reduced quality of life. The global burden of osteoarthritis and rheumatoid arthritis continues to rise due to population aging, sedentary lifestyles, and increasing metabolic comorbidities. Liposomal drug carriers hold great promise in maximizing dexamethasone's therapeutic utility while minimizing its first-pass effect and systemic toxicity. Due to their biocompatibility, slow-release capability, and potentia

INTRODUCTION: Osteoarthritis (OA) and cardiometabolic diseases (CMD), including metabolic syndrome, type 2 diabetes, and cardiovascular disease, are common chronic disorders sharing risk factors, mechanisms, and societal burdens. Obesity, insulin resistance, dyslipidaemia, hypertension, and inflammation link joint damage with vascular and metabolic issues. Factors such as advanced glycation end-products, adipokines, and cytokines including interleukin-6 and C-reactive protein drive systemic and local inflammation, accelerating joint deterioration and cardiovascular problems.

BACKGROUND: Neurogenic hip dysplasia (NHD) is a clinically significant musculoskeletal complication in children with neurological disorders, particularly infantile cerebral palsy (CP). It contributes substantially to pain, functional limitations, and reduced quality of life. Muscle imbalance and impaired dynamic hip stability lead to progressive deformity of the femoral head and acetabulum, increasing the risk of subluxation and dislocation. Risk strongly correlates with GMFCS level and is highest in non-ambulatory children (levels IV-V).
DIAGNOSTICS: Diagnosis and monitoring rely on radiogra

As wearables are becoming an increasingly important part of wellness and everyday life for many people, their potential in healthcare is also expanding, particularly in personalized and remote healthcare. However, many wearables lack sophistication, relying on simple sensors such as accelerometers and pulse meters to measure heart rate, body composition, and daily activity. Such basic metrics are insufficient for musculoskeletal disease diagnosis, which requires more detailed, multimodal neuromusculoskeletal monitoring. A major challenge in wearables development is the need for precise electro

Patellofemoral osteoarthritis is a prevalent musculoskeletal disorder characterized by knee pain during physically demanding activities like stair climbing and sit-to-stand transitions. These movements require high knee extension torques, leading to increased quadriceps activation and patellofemoral joint compression, which aggravates pain. While external torque assistance at the knee joint could theoretically reduce joint loads, traditional exoskeletons have not proven effective in managing osteoarthritis due to their rigid actuation, cumber-some attachments, and inadequate control systems. The global aging population faces a growing prevalence of sarcopenia and musculoskeletal (MSK) pain, two interrelated conditions that diminish physical function, quality of life, and independence in older adults. Sarcopenia, characterized by the loss of muscle strength, mass, and function, often coexists with MSK pain, with emerging evidence suggesting that each condition may contribute to the progression of the other. This perspective explores the bidirectional relationship between sarcopenia and MSK pain, highlighting shared mechanisms, including inactivity, cellular aging, chronic inflammat

BACKGROUND: Systemic lupus Erythematous (SLE) with its multisystem manifestations can sometimes throw the treating team into scientific oblivion. Bone marrow suppression increases the susceptibility to infection and life-threatening bleeding manifestations and hence timely diagnosis and treatment can be lifesaving in these patients. CASE PRESENTATION: Here we report such a young case of SLE who presented with anasarca, polyarthritis and other musculoskeletal manifestations. She was initially managed with steroids along with the evaluation of subnephrotic proteinuria with renal biopsy. Thereaft

Sarcopenia, defined by the progressive decline of skeletal muscle mass, strength, and performance, is an important, yet often overlooked complication of rheumatic and musculoskeletal diseases (RMDs). It arises from a multifactorial interplay of chronic systemic inflammation, oxidative stress, hormonal dysregulation, mitochondrial dysfunction, and prolonged glucocorticoid therapy. Elevated pro-inflammatory cytokines, such as IL-6, TNF-α, and IL-1β, promote catabolic pathways, impair regeneration, and accelerate muscle degradation. The prevalence of sarcopenia in RMDs may reach 40%, particularly

Musculoskeletal disorders (MSDs) encompass conditions that affect bones, joints, and muscles, leading to substantial pain and functional impairment and representing a major global health concern. In MSDs, key metabolic processes-such as glycolysis and lactate accumulation under hypoxic conditions-disrupt cellular energy homeostasis. Lactylation, a post-translational modification arising from excessive lactate, modifies both histone and non-histone proteins and has been implicated in inflammation and MSD pathogenesis. Preclinical studies indicate that targeting lactylation holds promise for the

Marfan's syndrome is a rare connective tissue disorder which mainly affects cardiovascular, skeletal, and ocular systems. Common cardiovascular manifestation of Marfan's includes aortic root dilatation, aortic regurgitation, and mitral valve prolapse. However, presence of congenital heart defects like atrial septal defect (ASD) is very rare. Here, we report the case of a 22-year-old male with Marfan's syndrome who had an uncommon coexistence of ASD and severe aortic pathologies. The patient had clinical features like tall stature, long arm span, and musculoskeletal abnormalities such as pectus
