[The iatrogenic complications of tracheostomy].

The objective of the present study was to analyze the complications of tracheostomy associated with bleeding from the brachiocephalic trunk. A total of 13 protocols of the autopsy study of the patients who had died in the intensive care unit were available for the analysis. These patients had experienced heavy external bleeding from the tracheostomy defect shortly before death. The study has demonstrated that all the victims had the tracheostomy hole localized below the level of the standard dissection of the tracheal rings. Nine patients presented with a damage to the brachiocephalic trunk wh

Clinical history, prognostic factors, and management of facial nerve in malignant tumors of the parotid gland.

OBJECTIVES: We analyzed the outcomes following clinical management of parotid masses that were determined to be malignant tumors after parotidectomy. METHODS: We evaluated data from 70 patients with parotid malignancies between November 1994 and December 2005. RESULTS: Among salivary histotypes (n=49), the most significant prognostic parameter was cT4 stage at diagnosis (P=0.0055, log-rank) both for clinical involvement of the facial nerve and for invasion of other structures. The main cause of cancer-related death was a distant metastasis. CONCLUSION: The present series confirms that the m

Purpura fulminans and late onset group B streptococcal sepsis in a premature twin.

Objective To report a rare case of unruptured internal carotid-posterior communicating artery (IC-PC) aneurysm splitting the oculomotor nerve treated by clipping and to review the previously published cases. Case Presentation A 42-year-old man suddenly presented with left oculomotor paresis. Three-dimensional digital subtraction angiography (3D DSA) demonstrated a left IC-PC aneurysm with a bulging part. During surgery, it was confirmed that the bulging part split the oculomotor nerve. After the fenestrated oculomotor nerve was dissected from the bulging part with a careful microsurgical techn

Is it necessary to remove submandibular glands in squamous cell carcinomas of the oral cavity?

The aim of this study was to determine the frequency and the mechanism of submandibular gland (SMG) involvement in oral cavity squamous cell carcinomas (OCSCC), and to discuss the necessity of extirpation of the gland. The authors investigated and analyzed the retrospective charts of 236 patients who underwent surgery for OCSCC over a 10-year period and the pathology reports of 294 neck dissections with SMG removal. SMG involvement was evident in 13 cases (4%). Eight cases were due to direct invasion, which was the most common mechanism. Four cases had infiltration from a metastatic periglandu…

Surgical treatment of esophageal cancer: Tokyo experience of the three-field technique.

Esophageal cancer can metastasize to the lymph nodes at a very early stage of the disease, and spread occurs both upwards and downwards. We have developed the 'three-field lymphadenectomy' (3-FD) technique, in which more than 100 lymph nodes are completely dissected from the lower neck, mediastinum, and upper abdomen. More than 700 patients have undergone 3-FD since 1984. Three-field lymphadenectomy is associated with considerable morbidity, although efforts have been made to reduce this by preserving tracheobronchial circulation and innervation. The mortality associated with 3-FD is acceptabl

Collateralization of an occluded internal carotid artery via a vas vasorum.

BACKGROUND: Reopening of an occluded internal carotid artery (ICA) is often seen in dissections but only rarely occurs in atherothrombotic occlusion of the internal carotid artery. CASE DESCRIPTION: A 60-year-old man suffered a minor stroke with dysphasia in March 1995. Color-coded duplex ultrasonography of his neck arteries revealed a left ICA occlusion. He was placed on a regimen of aspirin and followed up clinically and with ultrasonography. At follow-up 18 months later, the patient was asymptomatic. On duplex ultrasonography his left occluded ICA was found to be reopened, with a residual,

Lateral thigh free flap in head and neck reconstruction.

To understand the role of the lateral thigh flap in head and neck reconstruction, we performed injection studies and anatomic dissections in 2 fresh and 61 preserved cadaver extremities followed by 10 clinical cases involving defects after tumor ablation. The flap, based on the third perforator of the profunda femoris artery, is designed on the posterolateral aspect of the distal thigh. The pedicle courses through the biceps short head, allowing optional transfer of this muscle. Defects of the oral cavity, pharynx, cervical esophagus, and external skin were successfully restored. One flap was

Airway compromise after carotid surgery in patients with cervical irradiation.

Symptomatic carotid atherosclerotic disease occurring as a result of cervical irradiation often requires surgical intervention. Airway obstruction is an uncommon problem after most carotid surgery and has not been described for patients with cervical irradiation. Airway obstruction developed after two of five carotid endarterectomy procedures in previously irradiated necks requiring emergency tracheostomy or reintubation. Mechanisms of obstruction included endotracheal tube trauma to the fixed irradiated vocal cords and laryngeal edema caused by surgical dissection in an irradiated field. In a

Preoperative predictors of occult central lymph node metastasis in clinically node-negative papillary thyroid carcinoma: a common data model-based retrospective cohort study.

PURPOSE: In papillary thyroid carcinoma (PTC), central lymph node metastasis (CLNM) is frequently detected only after surgery, even when preoperative ultrasonography (USG) and CT show no suspicious lymph nodes. Identifying readily available clinical factors associated with occult CLNM may guide the decision for prophylactic central neck dissection in clinically node-negative (cN0) PTC. We investigated factors associated with CLNM using a Common Data Model (CDM)-based cohort. METHODS: Patients who underwent thyroid surgery for PTC between 2013 and 2022 and had no suspected nodal metastasis on …

Minimizing tissue damage through a modified muscle-sparing technique of retro-sigmoid exposure using linear incision.

BACKGROUND: Conventional retrosigmoid craniotomy involves cutting of suboccipital muscles and neurovasculature, often leading to chronic postoperative pain and neck discomfort. We evaluated a modified, muscle-sparing linear incision technique designed to preserve these structures. METHODS: This observational cohort study compared 34 cases using the muscle-sparing technique (Group A) with 34 cases using conventional muscle cutting techniques (Group B). In Group A, a linear incision was followed by sharp subfascial dissection, allowing the suboccipital muscle group and occipital neurovascular b

Vascular Eagle syndrome presenting with expanding cervical hematoma and acute ischemic stroke causing impending airway compromise.

INTRODUCTION: Eagle syndrome is a rare condition resulting from elongation of the styloid process or ossification of the stylohyoid ligament. While the classical variant typically presents with cervicofacial pain and dysphagia, the vascular variant may lead to compression or injury of adjacent vascular structures, resulting in transient ischemic attack, arterial dissection, or ischemic stroke. Hemorrhagic vascular complications associated with Eagle syndrome are extremely uncommon. CASE PRESENTATION: A 26-year-old female presented to the emergency department with rapidly progressive swelling

Assessing violent mechanical asphyxia in forensic pathology: State-of-the-art and unanswered questions.

Mechanical asphyxiation has been a common method used to commit homicide, including femicide, throughout history. A recent report by the United Nations has shed light on the issue of misidentification and concealment of gender-related killings, which makes it difficult to effectively fight against it. Forensic pathologists are frequently asked to examine cases involving suspected asphyxia to determine whether other persons have been involved. Therefore, medicolegal experts must look for signs of occlusion of the oral/nasal orifices, compression of the neck, or specific signs such as the "facie
