Myocardial work abnormalities in rheumatoid arthritis with preserved left ventricular ejection fraction are more closely related to disease activity than to disease duration: a prospective cross-sectional study.

OBJECTIVE: To investigate subclinical left ventricular (LV) dysfunction in rheumatoid arthritis (RA) patients with preserved left ventricular ejection fraction (pLVEF) using myocardial work (MW) analysis, and to determine if myocardial damage is closely associated with disease duration. PATIENTS AND METHODS: We prospectively enrolled 59 RA patients with preserved LVEF between January 2021 and September 2023 at Wujin Hospital Affiliated to Jiangsu University. The RA cohort was stratified by disease duration into a short-duration group (≤10 years, n = 28) and a long-duration group (>10 years, n

Left ventricular myocardial strain analysis by feature tracking cardiac magnetic resonance in patients with autoimmune rheumatic diseases: A case-control study.

BACKGROUND: Cardiovascular involvement is a major contributor to morbidity and mortality in autoimmune rheumatic diseases (ARDs). Subclinical myocardial dysfunction may occur despite preserved conventional cardiac function. Cardiac magnetic resonance feature tracking (CMR-FT) enables sensitive assessment of myocardial deformation and may aid in early detection of cardiac involvement. OBJECTIVE: To evaluate left ventricular myocardial strain using CMR-FT in patients with ARDs for detection of subclinical myocardial dysfunction. METHODS: This case-control study included 30 patients with ARDs a

Angiotensin-Converting Enzyme Inhibitors Mitigate Development of Chronic Persistent Cardiac Dysfunction Following Fulminant Myocarditis: A Multicenter Retrospective Study in China.

BACKGROUND: Although temporary mechanical circulatory supports (tMCS) combined with immunoregulatory therapy (IT) can reduce the mortality of patients with fulminant myocarditis (FM), a considerable proportion still progress to chronic persistent cardiac dysfunction. It is unclear if angiotensin-converting enzyme (ACE) inhibitors can further prevent such dysfunction under tMCS combined with IT. METHODS: This multicenter, retrospective, observational study included 124 FM patients with a left ventricular ejection fraction (LVEF) ≤ 40%. Among them, 90 (72.58%) received ACE inhibitors and 34 (27

Endocardial fibroelastosis in a dog with congestive heart failure.

In a 6-month-old, intact female, Japanese spitz presenting with severe dyspnea, lung ultrasonography revealed confluent B lines associated with severe echocardiographic left sided volume overload and systolic dysfunction. A congenital shunt or valvular dysplasia was not demonstrable. On electrocardiogram, there was a constant sinus rhythm, respectively sinus tachycardia. Cardiac troponin I was normal. Within 2 days of admission, the dog died of heart failure. On macroscopic postmortem examination, the left ventricle and atrium were markedly dilated, and the left ventricular endocardium had a m

Combined complement and coagulation activation in ST-elevation myocardial infarction: associations with myocardial injury and dysfunction.

INTRODUCTION: Preclinical data indicates reciprocal activation of the complement system and the coagulation cascade. The magnitude of this interaction in patients with acute myocardial infarction is unknown. We aimed to determine associations between circulating markers of complement and coagulation activation in patients with acute ST-elevation myocardial infarction (STEMI), and explore a possible link to myocardial injury and left ventricular dysfunction. MATERIALS AND METHODS: We included 864 patients with STEMI. Blood was drawn at a median of 18 hours after percutaneous coronary intervent

The role of cGMP in the physiological and molecular responses of the right ventricle to pressure overload.

Pulmonary arterial hypertension (PAH) is a progressive disease that is associated with a poor prognosis and results in right heart dysfunction. While pulmonary vascular disease is the obvious primary pathological focus, right ventricular hypertrophy (RVH) and right ventricular (RV) dysfunction are major determinants of prognosis in PAH. Our knowledge about the molecular physiology and pathophysiology of RV hypertrophy and failure in response to pressure overload is still limited, and most data are derived from left heart research. However, the molecular mechanisms of left ventricular remodelli

The impact of new tetralogy drugs on ventricular remodeling in heart failure patients: a systematic review and network meta-analysis.

BACKGROUND: Heart failure, particularly heart failure with reduced ejection fraction (HFrEF), represents a major global health challenge due to its high prevalence, rapid progression, and substantial mortality rates. While the “New Tetralogy” drugs (angiotensin receptor-neprilysin inhibitors, sodium-glucose cotransporter 2 inhibitors, beta-blockers, and mineralocorticoid receptor antagonists) have significantly advanced HFrEF management, their comparative effects on ventricular remodeling remain unclear. This network meta-analysis systematically evaluates these therapeutic agents to inform opt

Mild aortic valve disease and the diastolic pressure-volume relationship in heart failure with preserved ejection fraction.

OBJECTIVE: Mild aortic valve stenosis (AS) and aortic valve (AV) sclerosis are associated with diastolic dysfunction and increased mortality in the general population. This study specifically investigated the impact of mild AV disease in heart failure with preserved ejection fraction (HFpEF). METHODS: Consecutive patients hospitalised with HFpEF (n=370) underwent assessment of cardiac structure and function and long-term clinical follow-up. RESULTS: In the study cohort, 111 had mild AS (30%), 104 AV sclerosis (28%) and 155 a non-calcified AV (42%). Mild-to-moderate AV regurgitation (AR) was

What Is the Optimal Setting for a Continuous-Flow Left Ventricular Assist Device in Severe Mitral Regurgitation?

Excessive left ventricular (LV) volume unloading can affect right ventricular (RV) function by causing a leftward shift of the interventricular septum in patients with mitral regurgitation (MR) receiving left ventricular assist device (LVAD) support. Optimal settings for the LVAD should be chosen to appropriately control the MR without causing RV dysfunction. In this study, we assessed the utility of our electrocardiogram-synchronized rotational speed (RS) modulation system along with a continuous-flow LVAD in a goat model of MR. We implanted EVAHEART devices after left thoracotomy in six adul

Clinical and prognostic impact of left atrial strain in non-ischemic cardiomyopathies.

Over the last decade, left atrial (LA) strain has gained attention as a clinical diagnostic and prognostic non-invasive imaging parameter. LA strain measures the myocardial deformation of the LA in three phases; the reservoir (LA filling during ventricular systole), the conduit (passive filling of the left ventricle [LV]) and the booster phase (late-diastolic active atrial contraction). LA strain reflects both diastolic and systolic myocardial function and is presumed to be an early marker of atrial and ventricular dysfunction. The aim of this review article is to discuss the (patho)physiology

Cardiac Magnetic Resonance Imaging in Diagnostics and Cardiovascular Risk Assessment.

Cardiac magnetic resonance (CMR) allows for analysis of cardiac function and myocardial tissue characterization. Increased left ventricular mass (LVM) is an independent predictor of cardiovascular events; however, the diagnosis of left ventricular hypertrophy and its prognostic value strongly depend on the LVM indexation method. Evaluation of the quantity and distribution of late gadolinium enhancement assists in clinical decisions on diagnosis, cardiovascular assessment, and interventions, including the placement of cardiac implantable electronic devices and the choice of an optimal procedura

Systolic time intervals as a measure of left ventricular function in viscose rayon workers exposed to carbon disulfide.

In a group of viscose rayon workers exposed to carbon disulfide the systolic time intervals were calculated. They were determined from the simultaneous recording of an ECG, a phonocardiogram, and arterial pulse tracings. Results indicate a shortened left ventricular ejection time (lvet) and a prolonged isovolumetric contraction time (ICT), together with an increase in the ratio of ICT to LVET. These alterations, which occurred in the absence of any clinical signs or symptoms of heart disease, are interpreted as a consequence of an impaired left ventricular contractility, comparable to mild cor

Stem Cell Therapy for Hypoplastic Left Heart Syndrome: Mechanism, Clinical Application, and Future Directions.

Hypoplastic left heart syndrome is a type of congenital heart disease characterized by underdevelopment of the left ventricle, outflow tract, and aorta. The condition is fatal if aggressive palliative operations are not undertaken, but even after the complete 3-staged surgical palliation, there is significant morbidity because of progressive and ultimately intractable right ventricular failure. For this reason, there is interest in developing novel therapies for the management of right ventricular dysfunction in patients with hypoplastic left heart syndrome. Stem cell therapy may represent one

Repair of an anomalous left coronary artery from the pulmonary artery in a premature neonate.

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital heart defect. It is typically diagnosed at a few months of age. The infant presents with cardiac failure secondary to left ventricular dysfunction from the coronary steal that develops as pulmonary vascular resistance (PVR) falls. With the diagnosis made, surgical repair is typically performed expeditiously in order to try and restore left ventricular perfusion and stop the ongoing coronary steal. We present an unusual case of a preterm infant undergoing a routine echocardiogram, who was incide
