Catheter versus surgical ablation of atrial fibrillation after a failed initial pulmonary vein isolation procedure: a randomized controlled trial.

INTRODUCTION: The aim of this prospective randomized study was to compare the efficacy and safety of catheter ablation (CA) versus surgical ablation (SA) in the treatment of paroxysmal and persistent AF after failed initial pulmonary vein isolation procedure. METHODS AND RESULTS: Patients with a history of symptomatic AF after a previous failed first ablation procedure were eligible for this study. Patients were randomized to CA (n = 32) or SA (n = 32) redo ablation. The primary endpoint was recurrence of atrial tachyarrhythmia at 1 year of follow-up. At the 12-month follow-up, 26 (81%) of th

Superiority of video-assisted to standard mediastinoscopy in non-small-cell lung cancer staging.

BACKGROUND: We compared the efficiency of videomediastinoscopy (VM) and standard mediastinoscopy (SM) in detecting mediastinal lymph node (MLN) metastasis in non-small-cell lung cancer (NSCLC) patients. METHODS: By SM method a surgeon sampled bilateral paratracheal and subcarinal lymph node stations and then by VM method, another surgeon resampled all lymph node stations once more through the same incision in the same operative setting. Results of the pathologic examinations of two methods were compared. RESULTS: Twenty-seven consecutive mediastinoscopies were included. The numbers of nodal

Is VAMLA/TEMLA the new standard of preresection staging of non-small cell lung cancer?

Accurate mediastinal staging is the hallmark of a good thoracic oncology program. Mediastinal lymph node staging is important for prognostication and to guide the administration of neoadjuvant and adjuvant therapy. In addition, accurate mediastinal staging is necessary for a fair comparison of different clinical studies. The most important surgical advance in mediastinal lymph node staging in the past few years is transcervical staging, by either sternal elevation or video-assisted mediastinoscopy. The present report summarizes the existing published data evaluating such an approach.

Endoscopic repair for left ventricular pseudoaneurysm with right minithoracotomy.

Left ventricular pseudoaneurysm (LVPA) is a known serious complication of myocardial infarction or mitral valve replacement. As an alternative option, transmitral patch repair for LVPA has been reported. However, it is very difficult to complete the procedure for LVPA with a large defect. A 68-year old man with a history of inferior myocardial infarction had undergone mitral valve repair. At 4 months after surgery, the presence of a giant LVPA in the posteroinferior left ventricular wall was revealed. We performed transmitral patch repair through minithoracotomy under a three-dimensional video

Prospective study of percutaneous radiofrequency sympathicolysis in severe hyperhidrosis and facial blushing: efficacy and safety findings.

OBJECTIVE: Hyperhidrosis (Hh) and facial blushing (Fb) are conditions caused by hyperactivity of the sympathetic system; they affect around 2% of the population. Severe cases have been typically treated with thoracic sympathectomy through a minimally invasive approach. We sought to determine if severe Hh and/or Fb patients, who are reluctant to go through an operation with general anaesthesia, could benefit from receiving percutaneous radiofrequency ablation of the sympathetic chain. METHODS: Prospectively collected data obtained from May 2007 to May 2010 were analysed to compare the treatmen

[A new possibility for determining the size and position of the tricortical bone graft in ventral spine stabilisation].

AIM: Ventral stabilisation of thoracolumbar fractures is often done with autogenous iliac crest grafts as an alternative to other filling and stabilisation systems. Today the golden standard for this kind of reconstruction is the minimally invasive thoracoscopic stabilization. Often there was a problem with the determination of the size and the position of the graft, especially on video-assisted minimally invasive stabilisation. The aim of this study was now to develop a new instrument that would be able to minimise this kind of intraoperative problems. METHOD: For measuring and for checking

Clinical results of thoracoscopic Heller's myotomy in the treatment of achalasia.

OBJECTIVES: Ideal treatment for achalasia permanently eliminates the dysfunctional lower oesophageal sphincter, relieving dysphagia and regurgitation. The aim of this study was to review the results in a series of patients undergoing video-imaged thoracoscopic Heller's myotomy (THM). METHODS: Records of all patients undergoing THM by a single surgeon at one institution were analysed. Follow-up was conducted using a structured questionnaire together with oesophageal manometry and/or 24 h pH monitoring when clinically indicated. RESULTS: Twenty-five consecutive patients (13 males, 12 females,

Interruption of patent ductus arteriosus in children: robotically assisted versus videothoracoscopic surgery.

OBJECTIVES: If robotic surgery is to be widely used, the risks must be equivalent to those of standard techniques. This study analyzes the feasibility, safety, and efficiency of a robotically assisted technique for patent ductus arteriosus closure and compares the results with those of the videothoracoscopic technique. METHODS: During 2000, 56 children weighing 2.3 to 57 kg (mean, 12 kg) underwent surgical closure of a patent ductus arteriosus. They were distributed into 2 groups: 28 patients (group 1) underwent the videothoracoscopic technique, and 28 (group 2) underwent a robotically assist

Segmental Bowel Resection for Rectal Endometriosis Using the da Vinci SP.

OBJECTIVE: The da Vinci SP Surgical System (SP) received regulatory approval for use in gynecological surgeries in Japan in 2023. Given the advantages of the precision of a robot, less pain, and the cosmesis of single-port surgery, the da Vinci SP is expected to be further used for minimally invasive surgeries. To the best of our knowledge, this is the first report of the use of SP for the treatment of rectal endometriosis with segmental bowel resection. SETTING: An urban general hospital. Stepwise demonstration of the technique with narrated video footage. PARTICIPANTS: The patient was a 46

The British technique of purely robotic Whipple's pancreatoduodenectomy a systematic and stepwise ('clockwise') approach.

The growing trend towards minimally invasive hepato-biliary and pancreatic (HPB) surgery has led to the evolution of multiple techniques in the performance of the pancreaticoduodenectomy (PD), either Whipple's procedure or pylorus-preserving pancreaticoduodenectomy (PPPD), for lesions in the head of the pancreas and other indications. Recent trials have highlighted the limitations of the laparoscopic technique when it comes to PD. The use of robotic assistance allows surgeons to circumvent these limitations and carry out robotic pancreaticoduodenectomy (RPD) with good outcomes. The technique d

Fully Endoscopic Retrosigmoid Approach for Cerebellopontine Angle Tumors.

BACKGROUND: Fully endoscopic or endoscope-controlled approaches are essentially keyhole approaches in which rigid endoscopes are the sole visualization tools used during the whole procedure. At the early attempts of endoscope-assisted cranial surgery, it was noted that rigid endoscopes enabled overcoming the problem of suboptimal visualization when small exposures are used. The technical specifications and design of the currently available rigid endoscopes are associated with a group of unique features that define the endoscopic view and lay the basis for its superiority over the microscopic v

ConsisTNet: a spatio-temporal approach for consistent anatomical localization in endoscopic pituitary surgery.

PURPOSE: Automated localization of critical anatomical structures in endoscopic pituitary surgery is crucial for enhancing patient safety and surgical outcomes. While deep learning models have shown promise in this task, their predictions often suffer from frame-to-frame inconsistency. This study addresses this issue by proposing ConsisTNet, a novel spatio-temporal model designed to improve prediction stability. METHODS: ConsisTNet leverages spatio-temporal features extracted from consecutive frames to provide both temporally and spatially consistent predictions, addressing the limitations of

Giant solitary fibrous tumour of the pleura. Case report and review of the literature.

BACKGROUND: Solitary fibrous tumours of the pleura (SFTP) are rare tumours. They are mostly benign. Only around 12% of them are malign ant. In the initial stage they are mostly asymptomatic and by growing they cause chest pain, irritating cough and dyspnoea on account of the pressure created on the surrounding structures. Rare giant tumours have compression symptoms on the mediastinal structures. The condition requires tiered diagnostic radiology. Preoperative biopsy is not successful in most cases. The therapy of choice is radical surgical tumour removal. Malignant or non-radically removed be

Efficient segmentation of intraoperative anatomical landmarks in laparoscopic cholecystectomy based on deep learning.

BACKGROUND: Laparoscopic cholecystectomy is the gold standard procedure for the treatment of benign gallbladder diseases, but there is the risk of intraoperative bile duct injury, which can lead to surgical failure and cause significant social and economic burden. When surgeons rely on visual inspection to identify tissue structures during laparoscopic cholecystectomy, subjective factors such as experience, psychological factors, and fatigue can compromise the intraoperative recognition of anatomic landmarks. The positioning of anatomical landmarks by the surgeon in the pre-dissection phase of
