Perforator-plus flaps: a new concept in traditional flap design.

BACKGROUND: Conventional fasciocutaneous flaps in reconstructive surgery, especially in the lower extremities, have limited utility. Traditional flaps are essentially random pattern, often require delays, and are limited in mobility and reach. Islanded fasciocutaneous flaps pedicled on perforators can be raised anywhere on the body and have a reliable blood supply and greater freedom of movement. However, venous compromise is a common problem. METHODS: A new approach to raising conventional fasciocutaneous flaps while including and retaining perforators in their substance was used to offset t

MDCT in the evaluation of skeletal trauma: principles, protocols, and clinical applications.

Multidetector computed tomography (MDCT) scanners have made volume imaging possible and are used extensively to study polytrauma patients, especially in the evaluation of the spine and peripheral skeleton. An MDCT scanner coupled with a modern workstation has become an essential diagnostic tool for any emergency department. Familiarity with the basic physical principles of MDCT such as projection data, section collimation, and beam collimation is important to achieve high-quality imaging while keeping unnecessary radiation to a minimum. After a polytrauma MDCT examination, images can be recons

Movement and muscle activity pattern in wheelchair ambulation by persons with para-and tetraplegia.

The patterns of movement and muscle activation in wheelchair ambulation have been studied in two groups: subjects with paraplegia (n = 4) and tetraplegia (n = 3). All subjects were physically active and experienced wheelchair users. The tests were done in the subjects' own wheelchairs and under free-wheeling conditions. The tasks studied were: self-chosen normal velocity, maximal velocity and maximally accelerated start. Muscle activation was registered by surface electromyography performed on several arm and shoulder muscles. The movement pattern was studied by goniometry of the shoulder and

Evaluation of the lower motor neuron integrity of upper extremity muscles in high level spinal cord injury.

PURPOSE: To evaluate the lower motor neuron (LMN) integrity of upper extremity muscles of persons with high tetraplegia (C1-C4) in order to determine muscles available for stimulation. METHODS: Fourteen subjects (23 arms) were evaluated for LMN integrity. Muscles that elicited a functional response (grade 3 or better) to surface electrical stimulation were considered to have intact LMN and good candidates for FES. Strength-duration (S-D) curves were generated on muscles that showed weak (less than grade 3) or no response to surface stimulation. Muscles were considered denervated if S-D curves

[Cutaneous pigmentation induced by minocycline: 2 cases].

Two new cases of cutaneous pigmentation induced by minocycline are reported, in addition to the 38 cases collected in the literature. Our first case was a 50-year old man with a history of multiple orthopaedic operations for injuries sustained in road accidents. Arthritis of the elbow, probably of bacterial origin, was treated with cephalexin and gentamicin, followed by minocycline 300 mg/day. After a total dose of 60 g of minocycline had been reached, a bluish-grey pigmentation was observed on the internal aspect of the left tibia and on the scars left by the orthopaedic operations. Subsequen

Ischaemia and the pink, pulseless hand complicating supracondylar fractures of the humerus in childhood: long-term follow-up.

A series of 26 children was referred to our specialist unit with a 'pink pulseless hand' following a supracondylar fracture of the distal humerus after a mean period of three months (4 days to 12 months) except for one referred after almost three years. They were followed up for a mean of 15.5 years (4 to 26). The neurovascular injuries and resulting impairment in function and salvage procedures were recorded. The mean age at presentation was 8.6 years (2 to 12). There were eight girls and 18 boys. Only four of the 26 patients had undergone immediate surgical exploration before referral and th

Exploratory Velocity-Adjusted Upper-Extremity Loading Profiles in Fastball Pitching: A Secondary Analysis of OpenBiomechanics Data.

Background: Fastballs delivered at comparable velocities may differ in upper-extremity loading, but cross-sectional profile differences do not establish within-pitcher biomechanical mechanisms. Objective: To construct an exploratory velocity-adjusted elbow-shoulder loading profile and descriptively compare non-definitional kinetic-chain characteristics associated with high-velocity low- and high-adjusted-load profiles. Methods: This cross-sectional secondary analysis included 411 fastballs from 100 pitchers in the OpenBiomechanics Project. Elbow varus and shoulder internal rotation moments wer

Assessing the Elasticity of the Flexor Pronator Muscles After Throwing Using Ultrasound Shear Wave Elastography.

BACKGROUND: The medial ulnar collateral ligament (UCL) of the elbow joint is the primary restraint to valgus stress during the throwing motion. The flexor pronator muscles (FPMs) also stabilize the elbow joint against valgus forces; however, assessment of FPM stiffness in baseball players has been limited. PURPOSE: To use ultrasound shear wave elastography (USWE) to evaluate the change in tissue elasticity of the FPMs due to pitching. STUDY DESIGNS: Descriptive laboratory study. METHODS: Included were 14 healthy male amateur baseball players (mean age, 28.0 ± 2.6 years) who had played inter

Neurotization in microsurgical repair of obstetric brachial plexus palsy: Surgical outcome of different techniques.

BACKGROUND: In the treatment of obstetric brachial plexus palsy (OBPP), neurotization, or nerve transfer, has become a crucial microsurgical procedure, especially when nerve roots are avulsed or non-functional. To reinnervate a denervated target and restore motor function, a functional donor neuron must be redirected. The objective of the study is to assess the outcomes of various neurotization approaches in individuals with brachial plexus injuries sustained during pregnancy. METHODS: Thirty patients with OBPP who received microsurgical treatment using various brachial plexus neurotization p

A Prospective Comparative Study of Clinical Outcomes of Hook Plate and Endobutton Fixation Techniques for Acromioclavicular Joint Dislocation in Jharkhand, Eastern India.

BACKGROUND: Acromioclavicular joint dislocations (ACJDs) are prevalent shoulder injuries that often lead to significant disability, necessitating surgical repair to regain joint stability and function. The evolution of surgical techniques and the development of various implants have introduced multiple treatment modalities, among which the hook plate (HP) and the endobutton (EB) have gained prominence. The HP and EB are prominent implants used for this purpose, each offering distinct advantages and potential complications. AIM AND OBJECTIVE: This study aimed to evaluate the clinical outcomes

A comparison of sugar-tong and volar-dorsal splints for provisional immobilization of distal radius fractures in the adult population.

INTRODUCTION: Distal radius fractures are extremely common injuries affecting a wide range of patient demographics. The purpose of this study was to evaluate the outcomes of distal radius fractures managed initially with closed reduction and immobilization in either a below elbow volar-dorsal splint versus sugar-tong splint prior to conversion into a short arm cast. METHODS: We performed a retrospective study of patients with distal radius fractures placed in a sugar-tong (n = 45) and volar-dorsal splint (n = 36). Anteroposterior and lateral radiographs were evaluated immediately after closed

Massive rotator cuff tears in patients younger than 65 years. What treatment options are available?

Management of massive rotator cuff tears is a therapeutic challenge in patients younger than 65 years, particularly if still working. According to our hypothesis, choice of the most appropriate treatment option mainly depends on the patient's functional status and on two predictive factors: height of the subacromial space and fatty muscle infiltration. This is a retrospective, multicenter study of a series of 296 patients younger than 65 years, including 176 males and 120 females with extensive or massive cuff tear. Patients had loss of elevation or external rotation or both in 162 cases. Four

The use of forearm free fillet flap in traumatic upper extremity amputations.

BACKGROUND: Complete traumatic upper extremity avulsions are an infrequent but devastating injury. These injuries are usually the result of massive blunt trauma to the upper limb. Intact issue from amputated or nonsalvageable limbs may be transferred for reconstruction of complex defects resulting from trauma when the indications for replantation are not met. This strategy allows preservation of stump length or coverage of exposed joints, and provides free flap harvest for reconstruction without additional donor-site morbidity. METHODS: A retrospective review at São João Hospital was performe

[Repetitive strain injury (RSI): occurrence, etiology, therapy and prevention].

In the Netherlands, work related upper-limb disorders are called Repetitive Strain Injuries (RSI). RSI is not a diagnosis but a catch-all term for symptoms and signs located in the neck, upper back, shoulder, arm, elbow, hand, wrist and fingers. These symptoms may include pain, stiffness, tingling, clumsiness, loss of co-ordination, loss of strength, skin discoloration and temperature differences. Each year, 8% of working Dutch citizens take time off work due to RSI symptoms. Although the number of people claiming disability benefit due to RSI is limited, this figure has risen consecutively ov

Traction avulsion amputation of the major upper limb: a proposed new classification, guidelines for acute management, and strategies for secondary reconstruction.

Major replantation of a traction avulsion amputation is undertaken with the goal of not only the reestablishment of circulation, but also functional outcome. This type of amputation is characterized by different levels of soft-tissue divisions involving crushing, traction, and avulsion injuries to various structures. Between 1985 and 1998, 27 cases were referred for secondary reconstruction following amputation of the upper extremity involving both arm and forearm. Replantation was performed by at least 12 qualified plastic surgeons using different approaches and management, resulting in diffe
