Victimhood Without Personhood.

New York's Reproductive Health Act 2019 (RHA) reshaped the legal treatment of pregnancy-loss violence by confining homicide to the death of a "person" and removing earlier provisions that integrated certain late-term abortion and abortive offences into the homicide framework. This article traces New York's statutory development from the 1965 Penal Law to the RHA and uses the federal Unborn Victims of Violence Act 2004 (UVVA) as a comparator. It argues that the apparent tension between abortion permissibility and fetal-protection liability is better explained by jurisdictional triggers and cons

Maternal Kidney Adaptation and Function in Pregnancy: Review and Perspectives.

Pregnancy imposes considerable physiological demands on the maternal kidney, requiring coordinated structural, hemodynamic, and molecular adaptations to maintain homeostasis and support fetal development. This review synthesizes renal changes throughout normal and abnormal gestation, emphasizing the dynamic interactions among maternal, placental, and fetal systems. These adaptations are driven by integrated hormonal pathways, including the renin-angiotensin system and vasodilatory mediators, as well as emerging mechanotransductive processes that enable the kidney to sense and respond to increa…

Retinoic acid regulates fetoplacental vascularization via notch signaling and a SEMA3E/F-PLEXIND1 axis.

The placenta is vital for fetal development, and altered placental vascularization, the most common placental pathology, underlies prevalent disorders, including fetal growth restriction, prematurity, and pregnancy complications. Impaired placental vascularization is associated with Vitamin A deficiency, but the mechanisms are undefined. To investigate this, we used retinoic acid (RA)-deficient Raldh2-/- embryos, and found they exhibit allantoic and placental endothelial hyperproliferation and impaired arterial-venous remodeling, which were rescued by providing all-trans-RA (ATRA) via maternal…

Two cases of Leukemoid reaction in premature infants caused by fetal inflammatory response syndrome.

BACKGROUND: Fetal inflammatory response syndrome (FIRS) is a systemic inflammatory response caused by the activation of the fetal immune system. The serological diagnostic criterion for fetal inflammatory response syndrome is a cord blood interleukin-6 concentration that exceeds 11 pg/mL, while pathologic evidence indicates the presence of funisitis or chorionic vasculitis. It can affect all systems of the fetus. Alterations in patients' hematopoietic system are primarily reflected by changes in peripheral blood leukocyte and neutrophil counts. CASE PRESENTATION: We performed placental pathol

A systematic review of prenatally diagnosed vein of Galen malformations: prenatal predictive markers and management from fetal life to childhood.

INTRODUCTION: Vein of Galen malformations (VGMs) account for less than 1% of all intracranial vascular malformations. However, in fetal and pediatric populations, they represent the most common vascular malformation of the brain. For the effective management of this condition, an optimal knowledge of its prenatal and postnatal clinical features is mandatory. METHODS: Articles published between 1 January 2003 and 31 January 2024, reported in PubMed and EMBASE, were evaluated for a systematic review analyzing the prenatal and postnatal features and management of fetal VGMs. RESULTS: Thirty-one

Fetal premature excess vertebral linear calcification: a case series.

During embryogenesis, the vertebrae begin development during the 6th week of gestation via two lateral chondrification centers per segment. It was assumed that when disruptions occur in the process of somitogenesis during membranous vertebral body formation, chondrification and ossification will follow the anomalous membranous vertebral body scaffolding, resulting in an anomalous vertebral formation, such as a hemivertebra. Another hypothesis is that hemivertebra may result from anomalous distribution of intersegmental arteries of the vertebral column. There is no description in the medical li

Fetal endothelial colony-forming cells: Possible targets for prevention of the fetal origins of adult diseases.

Endothelial colony-forming cells (ECFCs), a subset of circulating and resident endothelial progenitor cells, are capable of self-renewal and de novo vessel formation, and are known key regulators of vascular integrity and homeostasis. Numerous studies have found that exposure to hostile environment during the fetal development exerts a profound influence on the level and function of ECFCs, which may be the underlying factor linking endothelial dysfunction to cardiovascular disease of the offspring in later life. Herein, we focus on the latest findings regarding the effects of pregnancy-related…

Is the expression of placental epithelial and lymphoid markers associated with the perinatal outcomes in preeclampsia?

OBJECTIVES: This study aimed to investigate the association of the epithelial and lymphoid immune markers with the adverse perinatal conditions such as early-onset preeclampsia (EOPE), fetal growth restriction (FGR) and intrauterine fetal death (IUFD) in preeclampsia in the placentae of preeclamptic patients. MATERIAL AND METHODS: A total of 60 pregnant patients were included in this study. The immunohistochemistry method was used to determine the expression levels of CD4, CD8, CD4 / CD8, CD68, P53, MDM2, CK18, CK19, E-cadherin, and β catenin. RESULTS: In our study, the increase in E-cadheri

Circulating biomarkers associated with placental dysfunction and their utility for predicting fetal growth restriction.

Fetal growth restriction (FGR) leading to low birth weight (LBW) is a major cause of neonatal morbidity and mortality worldwide. Normal placental development involves a series of highly regulated processes involving a multitude of hormones, transcription factors, and cell lineages. Failure to achieve this leads to placental dysfunction and related placental diseases such as pre-clampsia and FGR. Early recognition of at-risk pregnancies is important because careful maternal and fetal surveillance can potentially prevent adverse maternal and perinatal outcomes by judicious pregnancy surveillance

DECONSTRUCTING EXTRAPLACENTAL MEMBRANES TO UNDERSTAND BACTERIAL CHORIOAMNIONITIS.

Bacterial chorioamnionitis is an intrauterine infection that occurs during pregnancy and involves the membranes that extend from the placenta to form the sac encasing the amniotic fluid and developing fetus. Chorioamnionitis is most commonly caused by bacteria that likely ascend from the vagina into the gravid uterus and can result in devastating complications such as preterm labor, membrane rupture, fetal stillbirth, or severe infection of the newborn. Surviving babies exposed to chorioamnionitis in utero have an increased risk of lifelong disability. Unfortunately, most chorioamnionitis is c

Electrophoresis features and genotypes of Hb bart's hydrops fetalis.

Hydrops fetalis syndrome (Hb Bart's disease) is one of the common fetal development abnormalities and the most severe form of α-thalassemia. It causes fetal death during the third trimester or shortly after birth. The aim of this study is to investigate the feature of Hb Bart's disease by Capillary electrophoresis (CE) and its genotypes. A total of 38 cases of suspected fetal Hb Bart's disease were analyzed by B-ultrasound and CE, including 17 with 24-28 weeks of pregnancy, 9 with 29-32 weeks of pregnancy, and 12 with 33-38 weeks of pregnancy. All cases were then identified by DNA analysis. Th…

ACOG Practice Bulletin No. 192: Management of Alloimmunization During Pregnancy.

When any fetal blood group factor inherited from the father is not possessed by the mother, antepartum or intrapartum fetal-maternal bleeding may stimulate an immune reaction in the mother. Maternal immune reactions also can occur from blood product transfusion. The formation of maternal antibodies, or "alloimmunization," may lead to various degrees of transplacental passage of these antibodies into the fetal circulation. Depending on the degree of antigenicity and the amount and type of antibodies involved, this transplacental passage may lead to hemolytic disease in the fetus and neonate. Un

Maternal di-(2-ethylhexyl) phthalate exposure during pregnancy causes fetal growth restriction in a stage-specific but gender-independent manner.

Di (2-ethylhexyl) phthalate (DEHP) is male developmental toxicant that impairs testis development with reduced anogenital distance. The present study aimed to investigate whether maternal DEHP exposure during pregnancy causes intrauterine growth restriction (IUGR) in a gender-specific manner and to identify the critical window of DEHP-induced fetal IUGR. Pregnant mice were administered with DEHP (0, 50 or 200mg/kg) by gavage. Fetal IUGR was observed not only in males but also in females when litters were exposed to DEHP on gestational day (GD)0-GD17. Interestingly, fetal weight and crown-rump

Syncytiotrophoblast Functions and Fetal Growth Restriction during Placental Malaria: Updates and Implication for Future Interventions.

Syncytiotrophoblast lines the intervillous space of the placenta and plays important roles in fetus growth throughout gestation. However, perturbations at the maternal-fetal interface during placental malaria may possibly alter the physiological functions of syncytiotrophoblast and therefore growth and development of the embryo in utero. An understanding of the influence of placental malaria on syncytiotrophoblast function is paramount in developing novel interventions for the control of placental pathology associated with placental malaria. In this review, we discuss how malaria changes syncy

Metabolic disorders of fetal life: glycogenoses and mitochondrial defects of the mitochondrial respiratory chain.

Two major groups of inborn errors of energy metabolism are reviewed -glycogenoses and defects of the mitochondrial respiratory chain - to see how often these disorders present in fetal life or neonatally. After some general considerations on energy metabolism in the pre- and postnatal development of the human infant, different glycogen storage diseases and mitochondrial encephalomyopathies are surveyed. General conclusions are that: (i) disorders of glycogen metabolism are more likely to cause 'fetal disease' than defects of the respiratory chain; (ii) mitochondrial encephalomyopathies, especi

Fetal alcohol spectrum disorders: flying under the radar.

Alcohol exposure during pregnancy has been shown to result in a spectrum of birth defects known as Fetal Alcohol Spectrum Disorders (FASD) that can negatively impact a child's growth, development, cognition, behavior and physical appearance over his or her entire lifespan. FASD is not a diagnostic term, unlike Fetal Alcohol Syndrome (FAS), which is the most serious disorder within the spectrum. Despite warnings by the U.S. Surgeon General and others, childbearing age women continue to drink at high levels, even in pregnancy. As there is no cure for an FASD, preventive activities are currently

Mirror syndrome associated with sacrococcygeal teratoma: a case report.

BACKGROUND: Mirror syndrome is associated with both nonimmune and immune hydrops fetalis. The clinical manifestations are quite varied, and the pathophysiology is poorly understood. We describe a case of mirror syndrome associated with afetus that had a rapidly growing sacrococcygeal teratoma (SCT) without overt hydrops. CASE: At 30 weeks' gestational age a fetus with SCT began to show early sonographic evidence of right heart failure, placentomegaly and polyhydramnios without overt fetal hydrops. Shortly after these findings were noted, the mother began to develop hypertension, epigastric pa

Disturbances in placental immunology: ready for therapeutic interventions?

Recent studies have provided new insight into aberrations in the immunological interplay between mother and fetus and their potential role in the development of recurrent fetal loss and preeclampsia. The action of anti-phospholipid antibodies in recurrent fetal loss is now proposed to involve the complement system, neutrophil activation and the production of TNFalpha by immune bystander cells. A clear involvement of the immune system is emerging in preeclampsia, involving mainly the innate arm, especially neutrophils. The activation of peripheral neutrophils by placentally released inflammator
