Association of Autosomal Dominant Snowflake Vitreoretinal Degeneration with Retinoschisis.

OBJECTIVE: To define and characterize the clinical association of autosomal dominant snowflake vitreoretinal degeneration (SVD) with retinochisis arising from a heterozygous KCNJ13 variant in unrelated multigenerational families and to investigate its genetic, molecular, and functional consequences. DESIGN: Affected individuals from 4 unrelated families underwent detailed ophthalmic examinations, multimodal retinal imaging, and electroretinography. Genetic testing using targeted next-generation sequencing panels and Sanger sequencing identified a heterozygous KCNJ13 c.341T > C variant (p.[L11

The world according to girls: a qualitative study of school, work and identity among adolescent girls and young women living with HIV in Ghana.

BACKGROUND: Adolescent girls and young women (AGYW) living with HIV in Ghana face multiple intersecting forms of marginalisation. Beyond the clinical management of HIV, little is known about how they construct meaning, navigate identity and imagine their futures within structural contexts shaped by stigma, gender inequity, economic precarity and colonial legacies. OBJECTIVE: To explore how AGYW living with HIV in Ghana understand and negotiate their social identities in work and school. We then aimed to understand how their lived experiences at school and work are shaped by broader systems of

Advancing Early Detection of Alzheimer Disease in the Primary Care Setting in the United States.

BACKGROUND AND OBJECTIVES: As evidence supporting the robustness of blood-based biomarker (BBM) testing for Alzheimer disease (AD) continues to emerge, understanding the perceptions, drivers, and barriers to the adoption of these tests among primary care physicians (PCPs) in the United States is crucial for improving patient outcomes and advancing clinical management of AD. In this study, we perform a survey of PCPs in the United States to better understand current practices for detecting and diagnosing mild cognitive impairment (MCI) or mild dementia due to AD and to identify drivers and barr

Navigating Adulthood with Maple Syrup Urine Disease: Patient and Caregiver Perspectives on Healthcare Transition and Independent Living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

Rationale of Basic and Cellular Mechanisms Considered in Updating the Staging System for Diabetic Retinal Disease.

PURPOSE: Hyperglycemia is a major risk factor for early lesions of diabetic retinal disease (DRD). Updating the DRD staging system to incorporate relevant basic and cellular mechanisms pertinent to DRD is necessary to better address early disease, disease progression, the use of therapeutic interventions, and treatment effectiveness. DESIGN: We sought to review preclinical and clinical evidence on basic and cellular mechanisms potentially pertinent to DRD that might eventually be relevant to update the DRD staging system. PARTICIPANTS: Not applicable. METHODS: The Basic and Cellular Mechani

Costs of community-wide mass drug administration and school-based deworming for soil-transmitted helminths: evidence from a randomised controlled trial in Benin, India and Malawi.

OBJECTIVES: Current guidelines for the control of soil-transmitted helminths (STH) recommend deworming children and other high-risk groups, primarily using school-based deworming (SBD) programmes. However, targeting individuals of all ages through community-wide mass drug administration (cMDA) may interrupt STH transmission in some settings. We compared the costs of cMDA to SBD to inform decision-making about future updates to STH policy. DESIGN: We conducted activity-based microcosting of cMDA and SBD for 2 years in Benin, India and Malawi within an ongoing cMDA trial. SETTING: Field sites

Food insecurity during COVID-19: An Australian university experience.

The aim of this study was to describe the prevalence, severity, coping strategies and precipitating factors of food insecurity in university students in a large multi-campus Australian university during the COVID-19 lockdown in 2020, in context of providing information to inform institutional support. This was a cross-sectional analysis which was part of a larger university-led research project, the Thrive@Home study conducted at Monash University during the COVID-19 pandemic. The main outcome measures included: Sociodemographic characteristics, PROMIS anxiety, PROMIS depression and the six-it

Anxiety and Depression After a Cardiac Event: Prevalence and Predictors.

INTRODUCTION: Patients who are anxious or depressed after an acute cardiac event are at increased risk of a subsequent event and premature death. It is therefore important to identify these patients early in order to initiate supportive or even preventive measures. In the present study, we report on the prevalence of anxiety and depression during the first 12 months after an acute cardiac event, and the patient characteristics predictive of increased anxiety and depression risk in early and late convalescence. METHODS: We recruited a sample of 911 patients with acute myocardial infarction (AM

Towards understanding the drivers of policy change: a case study of infection control policies for multi-drug resistant tuberculosis in South Africa.

BACKGROUND: Explaining policy change is one of the central tasks of contemporary policy analysis. In this article, we examine the changes in infection control policies for multi-drug resistant tuberculosis (MDR-TB) in South Africa from the time the country made the transition to democracy in 1994, until 2015. We focus on MDR-TB infection control and refer to decentralised management as a form of infection control. Using Kingdon's theoretical framework of policy streams, we explore the temporal ordering of policy framework changes. We also consider the role of research in motivating policy chan

Family economic strengthening and mental health functioning of caregivers for AIDS-affected children in rural Uganda.

In sub-Saharan Africa, many extended families assume the role of caregivers for children orphaned by AIDS (AIDS-affected children). The economic and psychological stress ensued from caregiving duties often predispose caregivers to poor mental health outcomes. Yet, very few studies exist on effective interventions to support these caregivers. Using data from a randomized controlled trial called Suubi-Maka (N = 346), this paper examines whether a family economic strengthening intervention among families caring for AIDS-affected children (ages 12-14) in Uganda would improve the primary caregivers

Defining the cost of the Egyptian lymphatic filariasis elimination programme.

BACKGROUND: Lymphatic filariasis (LF) is targeted for global elimination. LF elimination programmes in different countries, including Egypt, are supported financially by national and international agencies. The national programme in Egypt is based on mass drug administration (MDA) of an annual dose of a combination of 2 drugs (DEC and albendazole) to all endemic villages. This study aimed primarily to estimate the Total and Government costs of two rounds of MDA conducted in Egypt in 2000 and 2001, the average cost per person treated, and the cost share of the different programme partners. MET

Barriers and facilitators of cervical cancer screening among female healthcare professionals: a systematic review.

BACKGROUND: Cervical cancer remains as one of the leading causes of cancer-related deaths among women, due to the significant disparities with regard to the implementation of cervical cancer screening programs between the high and low—and middle-income countries. Studies have shown that cervical cancer screening uptake among female healthcare professionals (HCPs) has also been remarkably low. This systematic review aimed to identify the barriers and facilitators that affect the uptake of cervical cancer screening among female HCPs. METHOD: In this systematic review, we selected a total of 456

Why healthy eating is not equally attainable for all: a participatory systems perspective on dietary behaviour of people living on a low income in the Netherlands, and recommendations for systemic change.

BACKGROUND: Living on a low income is associated with multiple interacting factors and mechanisms that contribute to less healthy diets. Yet research has rarely examined these systemic drivers in depth among people on low incomes, including the perspectives of both citizens with experiential knowledge as well as professionals. Therefore, this study aimed to gain insights into the complex system dynamics underlying dietary behaviour of people on a low income, from the perspective of both citizens with experiential knowledge and professionals. METHODS: This study applied a participatory, system-

AWhen screams go unheard: covert violence, women victims, and social denial.

BACKGROUND: Covert violence-defined as a pattern of subtle, intentionally hidden, and socially camouflaged abusive behaviors that systematically erode a victim's reality, autonomy, and self-worth-remains a critically understudied dimension of gender-based violence. Distinct from overt psychological aggression, covert violence operates through tactics such as gaslighting, micro-invalidations, coercive control, and economic manipulation, often evading detection while inflicting profound harm. This scoping review systematically maps the evidence on its manifestations, psychosocial impacts, and th

Patterns and predictors of classes of sexual desire trajectories in couples with medical infertility undergoing medically assisted reproduction.

BACKGROUND: Sexual desire is important for sustaining relationships during periods of vulnerability such as when undergoing medically assisted reproduction (MAR). Desire problems are the most common sexual issue experienced by couples during MAR and while there is evidence of variability, distinct classes and predictors of dyadic trajectories have not been established. AIMS: To establish distinct classes of sexual desire trajectories among couples undergoing MAR over 2-years and to examine biopsychosocial predictors of classes. METHODS: Couples (N = 217) seeking MAR completed an online surve

Cardiac Rehabilitation Adherence and Long-Term Outcomes Post Myocardial Infarction: A Mixed-Methods Study of Patient Motivation, Barriers, and Program Satisfaction.

Background Participation in a cardiac rehabilitation (CR) program is a key component of secondary prevention following myocardial infarction (MI). However, adherence to CR varies considerably and may influence functional recovery and short-term clinical outcomes. Understanding the factors that facilitate or hinder participation may help optimize rehabilitation programs and improve patient outcomes. This prospective mixed-methods study assessed the association between adherence to CR and clinical outcomes four months after MI and explored the motivational factors, perceived barriers, and patien
