Diagnosing physeal separation of the distal humerus in a newborn can be challenging. Skeletal immaturity and absence of secondary ossification centers make radiographic diagnosis difficult for inexperienced providers. This fracture is seen in the setting of complicated deliveries with excessive traction and rotation applied to the forearm. We report a case of physeal separation of the distal humerus in a newborn as a complication of cesarean section and describe the intervention used and the short-term results.

The elbow is a complex joint that supports forearm movement and consequently is at risk for various injuries and disorders. Elbow disorders can range from chronic to acute problems, many of which can be debilitating. This article explains the functional anatomy of the elbow joint and discusses the most common elbow disorders and injuries. It also presents the most common diagnostic imaging choices, along with typical acquisition methods.

Presented in this paper is a case of a young Filipino man presenting with recurrent acute gouty flares leading to chronic tophaceous gout and unusual cutaneous manifestations (miliary gout, panniculitis, ulcerations). Unusual sites within the body (torso, shoulder, forearms, thigh) revealed multiple urate dermal deposition. Self-medication with steroids eventually led to secondary Cushing's syndrome and eventual emergence of metabolic complications.

A 69 year old female presented to the Emergency Department with swelling of her right arm and elbow, one week following a fall which resulted in a fracture of the middle third of her right clavicle (Figure 1). The forearm was hot and tender and the arm and hand function were somewhat impaired by swelling, but sensation was intact. Initial bloods were unremarkable, apart from a mild neutrophilia.

A 13-year-old black boy had pruritic papular and nodular lesions on his forearms associated to edema of the lower lip, photophobia, conjunctivitis and pterygium. Skin biopsy of the lower lip revealed acanthosis, spongiosis with dermal perivascular mononuclear cell infiltration composed by lymphocytes, plasma cells and eosinophils consistent with actinic prurigo. Lesions improved considerably with the use of thalidomide 100mg/ day.

The superficial radial vein at the lateral edge of the inferior third of the forearm and of the wrist has strong anatomical relationship with the sensory superficial branch of the radial nerve. At this level, any venous puncture may be responsible for a lesion of this superficial part of the radial nerve. We report two cases of dysesthesia in the radial territory after this kind of puncture. This risk should not be neglected and venous puncture should be avoided in this area.

A 48-year-old Japanese man developed coalesced erythema multiforme-like lesions on the forearms and the dorsa of hands that were exposed to a cutting oil product. Subsequently, multiple, round erythematous lesions with iris formation occurred on the trunk and four extremities that had no direct contact with the oil. It is suggested that erythema multiforme can be induced via some allergic processes following a contact reaction.

A 49-y-old male, who had travelled in Central Africa, was admitted to hospital with oedema of the right forearm. He was diagnosed as having thrombosis of the ulnar veins. Subsequently eosinophilia and positive serology for filariae established the aetiologic agent as Loa loa. The patient received antithrombotic and antiparasitic therapy. This is, to our knowledge, the first reported case of thrombosis of the ulnar veins associated with Loa loa.

We report a case of pseudotumor cerebri (PC) in a maintenance hemodialysis patient that was associated with thrombosis of the right and left brachiocephalic veins caused by repeated subclavian and jugular vein catheterizations. Symptoms and signs of PC appeared when a right forearm Gortex graft was inserted. PC secondary to intracranial venous outflow obstruction is an important diagnostic consideration for hemodialysis patients with headache, visual disturbance, and papilledema.

A case of gunshot wound of the brain associated with a scalp defect is presented. Repair of the scalp defect was primarily achieved by multiple rotation flaps. Successive complications occurred in the postoperative course related to wound breakdown and infection. The patient underwent a secondary reconstruction by a free forearm flap. Despite an infected recipient bed and poor general condition, the postoperative course was uneventful and the healing was complete within 1 month.

The authors recall the principles and possibilities of this method and give details about its utilisation for each indication, and for their preferred indications: lengthening for marked disparity, complex trauma, progressive distraction of soft tissues, and for very young children.

This is a report of an unexpected laboratory diagnosis of Histoplasma capsulatum. The fungus was isolated from an acute cellulitic lesion on the forearm of an elderly male patient with a functioning renal transplant. The patient resides within the environs of Brisbane and has not travelled outside Australia. We consider the isolation of H. capsulatum from a rare site in a patient resident in a non-endemic area indicative of a latent opportunistic infection in an immunocompromised patient.

Twenty-eight unstable distal fractures of the radius have been treated with pins incorporated into a forearm plaster. The wrist is maintained in a neutral position permitting early use of the hand. Secondary displacement has been decreased. It is a safe and effective method of treating unstable distal fractures of the radius suitable for regular use in a busy district general hospital without the need of expensive specialized equipment.

A 51 year old man with an innominate artery aneurysm presented with claudication and ischemia of the right forearm and hand. Two-dimensional echocardiography visualized the saccular aneurysm and a pedunculated mobile thrombus within it that were not seen during aortic arch angiography. Two-dimensional echocardiography, shown to be useful in identifying intracardiac masses and aortic aneurysms, may be important in selecting patients with increased risk of embolization.

Superior dislocation of the fibular head associated with a fractured tibia occurred in four cases with a direct, severe trauma to the tibia. In one case there was permanent peroneal nerve damage. This injury is somewhat analogous to the fracture-dislocation of the Monteggia type in the forearm. To diagnose secondary dislocations of the fibula and treat the condition early, the knee joint should be carefully examined before and after X-ray of all fractures of the tibia.

In a double blind study, seven volunteers were given lidocaine 0.75 per cent and bupivacaine 0.25 per cent in two trials to compare the agents for use in intravenous regional analgesia. There was no significant difference between the two agents in the duration of analgesia after removal of the tourniquet. Bupivacaine produced fewer side effects. It also consistently produced persistent analgesia on the posterolateral aspect of the forearm.

A woman is described who has a severe bone dysplasia confined to the limbs. The degree of involvement is more marked in the distal bones. Her forearms and legs are short; the fibulae are represented by distal triangular remnants; the metacarpals are short, several phalanges in the fingers are absent; and the toes are represented by "ball-like" remnants containing a single bone. The findings in our patient are compared to previously reported cases of acromesomelic dwarfism.

Fibroadipose vascular anomaly (FAVA) is an intramuscular vascular anomaly where venous ectasia and muscle is replaced by fibroadipose tissue. It typically affects adolescents and young adults, who experience pain and swelling, primarily in distal muscles, such as the calf and forearm. We present a case of FAVA affecting the orbital region, posing diagnostic and management challenges. Surgical intervention was successful, with no recurrence over 2 years' follow-up.

CASE: An adult woman with anti-aminoacyl-t RNA synthetase syndrome developed pain and swelling of both hands and her left forearm, initially diagnosed as seronegative rheumatoid arthritis. Surgical exploration revealed multiple "rice bodies," and the specimen grew Mycobacterium intracellurale. She subsequently received antibiotic therapy.
CONCLUSION: In the diagnosis of rice body formation in musculoskeletal tissues, it is necessary to consider not only rheumatic diseases but also mycobacterial infection.

A 12-year-old female with oligoarticular juvenile inflammatory arthritis developed an atrophic linear plaque involving the left medial forearm and proximal arm 7 months after intra-articular triamcinolone injection for arthritis. The plaque spontaneously resolved without treatment over approximately one year. It is important to recognize this rare complication of intra-articular steroid injection in order to avoid potential misdiagnosis as linear scleroderma and subsequent immunosuppressive treatment.
