Radiation-induced sarcoma in a cat following hypofractionated, palliative intent radiation therapy for large-cell lymphoma.

CASE SUMMARY: A 5-year-old female spayed domestic shorthair cat was presented with a 4.5 × 3 cm ulcerated cutaneous mass on the nasal bridge … he right eye within the previous radiation field. CT scan revealed a mass associated with the right frontal sinus with extension throughout the nasal cavity and facial bones. Histopathology was consistent with a moderately differentiated sarcoma. Seventy-one months post-radiation therapy, the cat developed neurologic clinical signs and was humanely euthanized. …

Frontolateral pins for halo ring placement: reassessment of a common neurosurgical procedure with CT measurements of skull thickness.

OBJECT: Halo orthosis placement is a common neurosurgical procedure for the treatment of cervical spine injuries. Frontal sinus puncture by the anterior pins may occur using standard techniques, and up to 30% are dissatisfied with forehead scarring, especially women and African Americans. METHODS: The authors describe a frontolateral (FL) anterior pin site placement supported by high-resolution CT scan skull thickness measurements. The standard supraorbital (SO) pin site is several centimeters above the lateral orbit, whereas the FL pin site is 2-3 cm posterolateral to the SO site. Frontolate…

Bilateral interhemispheric subdural hematoma after inadvertent lumbar puncture in a parturient.

PURPOSE: Interhemispheric subdural hematomas (ISH) are rare in adults and occur most often after cranial trauma. We describe a parturient who developed bilateral acute ISH after inadvertent dural puncture associated with placement of an epidural catheter for labour analgesia. We discuss the features, pathophysiology, and management of this type of subdural hematoma. CLINICAL FEATURES: A 38-yr-old woman requested epidural analgesia for relief of labour pain. An inadvertent dural puncture occurred during placement of a 17G Tuohy needle. After labour and delivery, the patient developed symptoms

Canine sino-nasal aspergillosis: parallels with human disease.

Canine sino-nasal aspergillosis (SNA) is characterized by the formation of a superficial mucosal fungal plaque within the nasal cavity and/or frontal sinus of systemically healthy dogs. The most common causative agent is Aspergillus fumigatus. The fungus does not invade beneath the level of mucosal epithelium but incites a severe chronic inflammatory response that leads to local destruction of nasal bone. These clinicopathological features are equivalent to those of human chronic erosive non-invasive fungal sinusitis. The clinical diagnosis of canine SNA relies on multiple modalities but local…

[Pituitary macroadenoma associated with pulmonary and cutaneous sarcoidosis--a clinical case].

Sarcoidosis is a granulomatous disease of unknown etiology that can affect all organs and systems, including the central nervous system in approximately 5% of patients. Although the hypothalamo-hypophyseal system may be one of the areas affected by neurosarcoidosis, pseudotumoral lesions are very rare. This paper reports the case of a 42-year-old male Caucasian patient followed by the Pneumology and Dermatology services for pulmonary and cutaneous sarcoidosis. Corticotherapy was initiated five months from diagnosis and the patient showed clear clinical and imagiological signs of improvement. I

Computer assisted surgery and endoscopic endonasal approach in 32 procedures.

UNLABELLED: Computer aided surgery (CAS) is now routinely introduced in the ENT surgical field especially in endoscopic endonasal surgery. OBJECTIVE: Using a frameless computer aided surgery for endoscopic endonasal approach (SurgiGATE ORL TM), we tried to evaluate the practical use of such a system, to calculate supplementary installation time of the procedure, to determine the number of matching attempt (referencing) before starting optonavigation and to determine its clinical accuracy. PATIENTS AND METHODS: Thirty two patients underwent endoscopic endonasal surgery with the help of CAS; 1

Management of 100 consecutive direct carotid-cavernous fistulas: results of treatment with detachable balloons.

Direct carotid-cavernous fistulas are high-flow shunts with a direct connection between the internal carotid artery and the cavernous sinus. The goals of treatment are to eliminate the fistula and preserve carotid artery patency. The authors reviewed the outcome of 98 patients with 100 consecutive direct carotid-cavernous fistulas initially treated by transarterial embolization with detachable balloons (1979-1992) at the University of Cincinnati Medical Center to evaluate the merits of this technique and to provide a standard for comparison with future treatment alternatives. Among 100 fistula

Orbital fractures in children.

OBJECTIVE: To determine if the pattern of orbital fractures may be influenced by the changing craniofacial ratio of the growing child, as th … primarily in younger children as a consequence of the proportionally larger cranium and the lack of frontal sinus pneumatization. Lower orbital fractures are a type of facial fracture that occur primarily in older children as a consequence of the increased vulnerability of the face due to growth and the pneumatization of the paranasal sinuses.

[Intracranial chordoma--report of two cases with cytochemical study].

Intracranial chordoma is very uncommon. We report two cases of intracranial chordoma with cytochemical studies. Case 1: A 66-year-old housewife was admitted on February 10, 1981, complaining of disturbance of visual acuity and bitemporal hemianopsia. A plain skull film showed normal finding with no calcification. CT showed a spherical ring contrast enhancement with intraluminal low density area in the suprasellar cistern. Pneumoencephalogram showed a suprasellar mass with filling defect of the anterior part of the third ventricle. On February 28, 1981, the tumor was intracapsually removed by t

The endoscopic supraorbital approach to tumors of the middle cranial base.

BACKGROUND: Access to tumors of the middle cranial base has traditionally required wide surgical exposures via open craniotomies. These open techniques often require the use of potentially disfiguring skin incisions and are often associated with a significant degree of brain retraction and potential morbidity. We report our experience with the use of a minimally invasive supraorbital endoscopic approach through the eyebrow for excision of middle cranial base tumors in 2 cases. METHODS: We describe 2 patients with large-sized middle cranial fossa tumors (a medial sphenoid wing meningioma measu

Surgical treatment of intracavernous trigeminal schwannomas via a fronto-temporal epidural approach.

The purpose of this study is to evaluate the efficacy of the fronto-temporal epidural approach in managing intracavernous trigeminal schwannomas. Five unselected patients harboring an intracavernous trigeminal schwannoma were operated on. Each of them was cured by a single craniotomy. In all cases the skull base approach described by Dolenc [Acta Neurochir. (Wien) 130 (1994) 55] was performed. The complete resection of the tumor and its capsule was gained in all five cases. There was no surgical mortality. No patient developed postoperative major complications. The fronto-temporal epidural app

Unusual location of sinonasal ossifying fibroma of the posterior ethmoid sinus: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Ossifying fibroma is a benign, well-demarcated fibro-osseous neoplasm that most commonly affects the mandible and maxilla. Involvement of the paranasal sinuses is rare, and ethmoid sinus localization is particularly uncommon. Because sinonasal ossifying fibroma may mimic other fibro-osseous and osseous lesions radiologically, histopathologic confirmation and complete excision are important for diagnosis and disease control. CASE PRESENTATION: A 43-year-old Saudi man with well-controlled type 2 diabetes mellitus presented with a 6-month history of progressive right

Migratory Preseptal Cellulitis Secondary to Rhinosinusitis Complicated by Left Frontal Subdural Empyema in a Child: A Case Report.

Preseptal cellulitis is a common pediatric infection involving the eyelids and surrounding periocular tissues. It is typically benign and rarely progresses to intracranial complications, unlike orbital cellulitis. However, when secondary to rhinosinusitis, the infection may behave unpredictably and extend beyond expected anatomical boundaries. We report a rare case of migratory preseptal cellulitis in a 12-year-old boy, initially presenting with unilateral eyelid swelling, erythema, and fever. Despite prompt initiation of empirical intravenous antibiotics, the condition failed to improve and i
