Machine Learning and Natural Language Processing to Improve Classification of Atrial Septal Defects in Electronic Health Records.

BACKGROUND: International Classification of Disease (ICD) codes can accurately identify patients with certain congenital heart defects (CHDs). In ICD-defined CHD data sets, the code for secundum atrial septal defect (ASD) is the most common, but it has a low positive predictive value for CHD, potentially resulting in the drawing of erroneous conclusions from such data sets. Methods with reduced false positive rates for CHD among individuals captured with the ASD ICD code are needed for public health surveillance. METHODS: We propose a two-level classification system, which includes a CHD and

Comparative Evaluation of Large Language and Multimodal Models in Detecting Spinal Stabilization Systems on X-Ray Images.

Background/Objectives: Open-source AI models are increasingly applied in medical imaging, yet their effectiveness in detecting and classifying spinal stabilization systems remains underexplored. This study compares ChatGPT-4o (a large language model) and BiomedCLIP (a multimodal model) in their analysis of posturographic X-ray images (AP projection) to assess their accuracy in identifying the presence, type (growing vs. non-growing), and specific system (MCGR vs. PSF). Methods: A dataset of 270 X-ray images (93 without stabilization, 80 with MCGR, and 97 with PSF) was analyzed manually by neur…

Language and Beyond: A Registered Report Examining Single and Multiple Risk Models of Later Reading Comprehension Weaknesses.

Children with poor reading comprehension tend to have oral language weaknesses, suggesting that poor language in the early years is a proximal cause of later reading comprehension difficulties. Yet, longitudinal studies have not succeeded in reliably predicting which children go on to have comprehension weaknesses (CW), and evidence comprises small sample sizes and a narrow focus on language in isolation. In this registered report, we examined early predictors of later outcomes in 879 children with CW identified at 9 years in the Avon Longitudinal Study of Parents and Children. Study 1 showed …

Translating Technology Into Speech-Language Pathology Practice: Qualitative Interviews Using Transcranial Magnetic Stimulation as a Case Study.

BACKGROUND: Technologies such as videofluoroscopy, therapy applications, and telehealth have been translated into the clinical toolkit of many speech-language pathologists (SLPs). Transcranial magnetic stimulation (TMS) is an emerging technology that shows therapeutic promise for communication and swallowing disorders and, once suitable for clinical use, SLPs will likely be responsible for delivering this treatment. As such, it is critical that SLP opinions and attitudes towards TMS, as well as broader translation issues facing the profession, are considered early in the TMS research and trans…

Interdisciplinary approach to identify language markers for post-traumatic stress disorder using machine learning and deep learning.

Post-traumatic stress disorder (PTSD) lacks clear biomarkers in clinical practice. Language as a potential diagnostic biomarker for PTSD is investigated in this study. We analyze an original cohort of 148 individuals exposed to the November 13, 2015, terrorist attacks in Paris. The interviews, conducted 5-11 months after the event, include individuals from similar socioeconomic backgrounds exposed to the same incident, responding to identical questions and using uniform PTSD measures. Using this dataset to collect nuanced insights that might be clinically relevant, we propose a three-step inte…

Extraction of clinical data on major pulmonary diseases from unstructured radiologic reports using a large language model.

Despite significant strides in big data technology, extracting information from unstructured clinical data remains a formidable challenge. This study investigated the utility of large language models (LLMs) for extracting clinical data from unstructured radiological reports without additional training. In this retrospective study, 1800 radiologic reports, 600 from each of the three university hospitals, were collected, with seven pulmonary outcomes defined. Three pulmonology-trained specialists discerned the presence or absence of diseases. Data extraction from the reports was executed using G…

Word learning by children with developmental language disorder: Identifying gaps in our understanding of spaced retrieval effects.

BACKGROUND AND AIMS: Current evidence shows that children with developmental language disorder (DLD) benefit from spaced retrieval during word learning activities. Word recall is quite good relative to recall with alternative word learning procedures. However, recall on an absolute basis can be improved further; many studies report that fewer than two-thirds of the words are learned, even with the assistance of spaced retrieval during the learning activities. In this article we identify details of spaced retrieval that are less well understood in an effort to promote more effective learning th…

Video Remote Sign Language Interpreting in Health Communication for Deaf People: Protocol for a Randomized Controlled Trial.

BACKGROUND: The current standard of interpretation provision is not efficacious or not acceptable to Deaf patients who communicate using sign language. In-person or video relay interpretation (VRI) sign language interpretation is largely unavailable. There are no clear data on the availability of VRI or in-person interpretation. Given the limited number of available sign language interpreters and the cost, VRI may be more available than in-person. Existing evidence tends to focus on assessing personal preferences of Deaf users regarding interpretation and interpreters' preferences. Although re…

Using Large Language Models to Annotate Complex Cases of Social Determinants of Health in Longitudinal Clinical Records.

Social Determinants of Health (SDoH) are an important part of the exposome and are known to have a large impact on variation in health outcomes. In particular, housing stability is known to be intricately linked to a patient's health status, and pregnant women experiencing housing instability (HI) are known to have worse health outcomes. Most SDoH information is stored in electronic health records (EHRs) as free text (unstructured) clinical notes, which traditionally required natural language processing (NLP) for automatic identification of relevant text or keywords. A patient's housing status…
