Hindfoot Valgus following Interlocking Nail Treatment for Tibial Diaphysis Fractures: Can the Fibula Be Neglected?

Purpose. We evaluated whether intramedullary nail fixation for tibial diaphysis fractures with concomitant fibula fractures (except at the distal one-third level) managed conservatively with an associated fibula fracture resulted in ankle deformity and assessed the impact of the ankle deformity on lower extremity function. Methods. Sixty middle one-third tibial shaft fractures with associated fibular fractures, except the distal one-third level, were included in this study. All tibial shaft fractures were anatomically reduced and fixed with interlocking intramedullary nails. Fibular fractures …

Which approach is best for subungual glomus tumors? Transungual with microsurgical dissection of the nail bed or periungual?

Glomus tumors are rare, accounting for 1% to 5% of all hand tumors. The treatment of subungual glomus tumors consists of complete surgical excision, either by the periungual or transungual approach. Tumor recurrence and nail dystrophy are the main complications. The purpose of this study was to evaluate the outcome as a function of the surgical approach used - transungual or periungual. Fifty-three patients presenting with a subungual glomus tumor underwent surgical excision through the periungual approach if the tumor was peripheral, or the transungual approach if the tumor was centrally loca

Nail bed defect covered with labial mucosa graft after subungal glomus tumor removal.

The nail bed is an important part of the nail unit. It supports the nail plate, but does not regenerate itself. The labial mucosa and the nail bed both lack a granular layer. Because the keratinization of the nail bed can cause poor nail adhesion, harvesting graft from nonkeratinized sites is preferred. Furthermore, harvesting graft from labial mucosa is convenient in surgical approach. Therefore, the labial mucosa is more eligible for covering the nail bed defect than other graft methods. In this report, a 46-year-old female patient suffered from a subungal glomus tumor of the right fourth fi

Functional results of displaced proximal humerus fractures in children treated by elastic stable intramedullary nail.

PURPOSE: Displaced proximal humerus fractures within the pediatric population can be treated by elastic stable intramedullary nailing (ESIN). The main objective of our study is to evaluate functional outcome of the displaced proximal humeral fractures treated by ESIN within the pediatric group using a standardized evaluation scale. The secondary goal is to compare functional outcome of epiphyseal and metaphyseal injuries and functional outcome of children younger and older than 10 years of age. MATERIALS AND METHODS: From March 2010 to December 2011, 27 children had been treated surgically us

Freehand 'figure 4' technique for tibial intramedullary nailing: introduction of technique and review of 87 cases.

Intramedullary nailing of tibial fractures is commonplace, and freehand operative techniques are increasingly popular. The standard freehand method has the knee of the injured leg flexed over a radiolucent bolster. This requires the theatre fluoroscope to swing from antero-posterior to lateral position several times. Furthermore, guide wire placement, reaming and nail insertion are all performed well above most surgeons' shoulder height. Alternatively the leg is hung over the edge of the table, and the assistant must crouch and hold the leg until the nail is passed beyond the fracture. We desc

Complications and outcomes of diaphyseal forearm fracture intramedullary nailing: a comparison of pediatric and adolescent age groups.

BACKGROUND: Flexible intramedullary nailing (IMN) has become a popular technique for the management of unstable or open forearm fractures. Recent publications have suggested an increased incidence of delayed union and poor outcomes in older children and adolescents. The objective of this study was to review forearm fractures treated with IMN, comparing the rate of complications and outcomes between the 2 age groups. Our hypothesis was that IMN is an effective technique with a similar rate of complications in both age groups. METHODS: An Institutional Review Board-approved retrospective review

[Dependence of a recurrence rate of a growing in nail from the choice of the operative intervention method and preoperative preparation].

The investigation results on dependence of the growing in nail recurrence rate from the choice of the operative intervention method and preoperative preparation are adduced. There were operated on 73 patients. The 1/3 nail plate resection, excision of pathologic granulations and the nail bed, the nail bed plasty were performed in 51 patients. In preoperative preparation there were used the baths, containing a decoction of a Wormwood bitter and antiseptic preparation--1% spirituous solution of chlorophyllypte--locally in a form of bandages.

Reducing chemical exposures in nail salons through owner and worker trainings: an exploratory intervention study.

BACKGROUND: Nail salons represent a rapidly growing industry with mainly Vietnamese immigrant workers. Workers routinely handle nail products containing hazardous compounds, yet have limited accessible information to minimize workplace exposures. METHODS: We conducted a culturally appropriate pilot intervention on workplace chemical exposure reduction strategies. We trained eight Vietnamese owners, who then trained Vietnamese workers in their salons. We conducted pre-, mid-, and post-intervention assessments with workers, including an in-person survey and personal air monitoring of volatile c

Removal of a femoral nail with osseous overgrowth at the end-cap: A navigated and cannulated minimally invasive technique.

Intramedullary nail removal can be demanding, especially in cases of implant breakage or bony overgrowth at the end-cap, if the exact insertion depth of the nail is neglected in the index surgery. In the presented case, two challenging nail removals were necessary. The first was performed in a re-nailing procedure due to a pseudarthrosis with implant breakage, and the second was performed during hardware removal after fracture healing in a situation where there was deep intramedullary placement of the exchange nail. For the second implant removal a minimally invasive approach based on instrume

CT scanogram for limb length discrepancy in comminuted femoral shaft fractures following IM nailing.

INTRODUCTION: Leg length discrepancy (LLD) following intramedullary nailing of femoral fractures is not uncommon. We designed a prospective study to evaluate the efficacy of routine postoperative computed tomography (CT) scanograms for evaluation of limb length discrepancy in patients with comminuted Winquist III or IV femoral shaft fractures treated with intramedullary nailing. METHODS: The study consisted of 15 patients with Winquist III and 13 with a Winquist IV femoral shaft fracture pattern with an average age of 37 years. The mechanisms of injury were motor vehicle collision (13), gunsh

Penetrating injury to the superior sagittal sinus by a nail in a 4-year-old child: a case report.

Penetrating head injuries are rare in children, with most injuries being accidental as a result of unsupervised use of sharp objects by young children. We present the case of a 4-year-old boy brought to our emergency department with a nail embedded through a wooden board and into his skull. The nail was determined to be entering the superior sagittal sinus through radiographic imaging. Thus, surgical removal of the nail was determined to be necessary. Inspection revealed a likely through-and-through injury to the sinus, and therefore, because sinus reconstruction was not deemed possible, the s

Percutaneous plating for comminuted midshaft fractures of the clavicle: a surgical technique to aid the reduction with nail assistance.

INTRODUCTION: Due to the complex anatomy of the clavicle, percutaneous plating with indirect reduction for comminuted midshaft fracture of the clavicle is challenging. The aim of this series was to report on a novel technique of nail-assisted percutaneous plating and to evaluate the radiographic and clinical outcomes in comminuted midshaft fractures of the clavicle. MATERIALS AND METHODS: Between 2009 and 2010, with nail assistance for the reduction and its maintenance, percutaneous plating was performed in 14 patients with comminuted midshaft fracture of the clavicle. The mean follow-up peri

[Fungal nail infections--an update: Part 1--Prevalence, epidemiology, predisposing conditions, and differential diagnosis].

Onychomycosis describes a chronic fungal infection of the nails most frequently caused by dermatophytes, primarily Trichophyton rubrum. In addition, yeasts (e. g. Candida parapsilosis), more rarely molds (Scopulariopsis brevicaulis), play a role as causative agents of onychomycosis. However, in every case it has to be decided if these yeasts and molds are contaminants, or if they are growing secondarily on pathological altered nails. The point prevalence of onychomycosis in Germany is 12.4%, as demonstrated within the "Foot-Check-Study", which was a part of the European Achilles project. Altho…
