Bloodstream infections caused by Streptococcus anginosus group bacteria: A retrospective analysis of 78 cases at a Japanese tertiary hospital.

OBJECTIVES: To investigate the characteristics of Streptococcus anginosus group (SAG) bacteremia in recent years, we conducted a retrospective cohort study and compared its findings with the data from previous studies. METHODS: All patients with positive blood cultures from May 2005 to September 2014 in a tertiary care center with 925 beds were included. RESULTS: There were 78 cases of SAG bacteremia (51 cases men; median age, 68 years) during the study period. The most common comorbidities were solid tumors in 32.1% of the patients. The most common infection source was hepatobiliary in one-

Objective outcome evaluation using inertial sensors in subacromial impingement syndrome: a five-year follow-up study.

Shoulder-related dysfunction is the second most common musculoskeletal disorder and is an increasing burden on health-care systems. Commonly used clinical questionnaires suffer from subjectivity, pain dominance and a ceiling effect. Objective functional measurement has been identified as a relevant issue in clinical rehabilitation. Inertia based motion analysis (IMA) is a new generation of objective outcome assessment tool; it can produce objective movement parameters while being fast, cheap and easy to operate. In this prospective study, an inertial sensor comprising a three-dimensional accel

Role of ultrasound in assessing remission in rheumatoid arthritis.

INTRODUCTION: Remission is the ultimate goal of the treatment of rheumatoid arthritis (RA). However, the diagnosis of remission might still be vague. Musculoskeletal ultrasound (US) seems to effectively assess synovitis, effusion and bone damage. Thus, its role could be relevant for the diagnosis, monitoring or detection of relapse in the follow-up of RA in remission. The goal of this review of the literature was to clarify the added value of ultrasonography during remission. METHODS: A systemic search of the literature was performed on Medline and Scopus. The following key words were used: r

Subchondral insufficiency fractures and spontaneous osteonecrosis of the knee may not be related to osteoporosis.

UNLABELLED: The term insufficiency fracture implies inadequate bone and is applied to some subchondral knee magnetic resonance images. We reviewed bone mineral density, body mass index, meniscal extrusion, comorbidities, and demographics in 32 knee insufficiency fracture patients. Only five were osteoporotic. Meniscal extrusion was predominant. PURPOSE: The literature supports systemic osteoporosis as a risk fracture for spontaneous osteonecrosis of the knee (SONK). SONK is also called a subchondral insufficiency fracture. Recognizing that insufficiency fracture and SONK are related, we desig

Long-term damage assessment in patients with microscopic polyangiitis and renal-limited vasculitis using the Vasculitis Damage Index.

OBJECTIVES: The Vasculitis Damage Index (VDI) is used to define the degree of damage occurring in patients with systemic vasculitis. We conducted a retrospective study of 30 patients with microscopic polyangiitis (MPA) and renal-limited vasculitis (RLV). METHODS: The clinical data and VDI of the 30 patients enrolled in the study were collected and assessed for a period of 5 years. RESULT: The VDI score, which was 2.5 at 1 year after the initial diagnosis, increased gradually to 4.3 at 5 years post-diagnosis. The degrees of musculoskeletal and ocular damage significantly increased during the

Bread mold osteomyelitis in the femur.

Rhizopus osteomyelitis is an uncommon and often fatal infection that usually occurs in immunocompromised patients. The infection is commonly referred to as "bread mold." The usual course of treatment is Amphotericin B, debridement, and, if needed, amputation of the affected limb. This article details a rare case of postoperative Rhizopus osteomyelitis in an otherwise healthy patient. The patient originally presented at another institution for anterior cruciate ligament repair after a ski injury. Postoperatively, he developed clinical evidence of infection. He was referred to our institution wi

Musculoskeletal complications encountered in the lysosomal storage disorders.

The lysosomal storage disorders are a heterogeneous group of inherited metabolic diseases resulting from defects in the degradation or transport of several distinct by-products of cellular turnover. The various subtypes are characterized by multi-systemic involvement; the wide range in patient ages at symptom onset is only partly explained by the underlying mutation(s). Neurodegenerative features and musculoskeletal complications are often seen in the most severe variants, and are features of the disease that have the most significant impact on patients' physical and functional well-being. Sarcoid and erythema nodosum arthropathies.

Sarcoidosis is a systemic granulomatous disease of unknown origin, characterized in affected organs by an accumulation of activated T lymphocytes and macrophages. Musculoskeletal manifestations of sarcoidosis include acute and chronic arthritis and muscular and osseous sarcoidosis. In certain populations, acute sarcoidosis often presents with constitutional symptoms, polyarthritis and erythema nodosum (Löfgren's syndrome). Erythema nodosum, often with joint symptoms, also occurs in association with several other conditions including infections, medications and other underlying diseases. The di

Diagnostic Performance of Articular Ultrasound Versus Magnetic Resonance Imaging in the Approach to Rotator Cuff Lesions at a Referral Hospital.

Introduction Rotator cuff injury is a leading cause of musculoskeletal pain and the most common shoulder pathology. It is characterized by pain and limited mobility, particularly during overhead activities, and is associated with factors such as age, trauma, occupation, limb dominance, and cardiovascular risk. Diagnosis requires clinical evaluation and imaging, with ultrasound being a cost-effective and accessible option, whereas MRI is preferred for persistent or ambiguous cases. However, MRI access may be restricted by geographical and economic constraints. Methodology A retrospective census

Are Accelerometer-based Functional Outcome Assessments Feasible and Valid After Treatment for Lower Extremity Sarcomas?

BACKGROUND: Aspects of physical functioning, including balance and gait, are affected after surgery for lower limb musculoskeletal tumors. These are not routinely measured but likely are related to how well patients function after resection or amputation for a bone or soft tissue sarcoma. Small, inexpensive portable accelerometers are available that might be clinically useful to assess balance and gait in these patients, but they have not been well studied. QUESTIONS/PURPOSES: In patients treated for lower extremity musculoskeletal tumors, we asked: (1) Are accelerometer-based body-worn monit

Beyond the Operating Room: Orthopaedic Advocacy, Identity, and Wider Impact.

➢ Orthopaedic identity is traditionally anchored in the operating room, where technical skill, decisive intervention, and operative outcomes remain central to professional validation. Yet orthopaedic surgeons also encounter realities that no implant can fully address: motor vehicle collisions, unsafe work, poverty, delayed access, disability, rehabilitation gaps, and loss of livelihood. We argue that orthopaedic responsibility may, in selected circumstances, extend beyond direct operative care into advocacy, education, systems improvement, disability inclusion, public engagement, and health-po

Neurofibromatosis Type 1: An Imaging Review of Multisystem Manifestations in Children.

Neurofibromatosis type 1 (NF1) is a common inherited tumor predisposition syndrome with multisystem involvement of the central and peripheral nervous systems, vasculature, skeleton, skin, and internal organs. Many clinically relevant manifestations are internal, age-dependent, progressive, or initially asymptomatic, making imaging central to diagnosis, surveillance, and treatment. This review provides a radiology-centered overview of the major imaging manifestations of NF1 and emphasizes pattern recognition, modality selection, and imaging features that should prompt closer follow-up, function

Quantifying locomotor instability in healthy adults: Insights from treadmill-induced slips and trips using a dynamic criterion.

BACKGROUND: Understanding how the musculoskeletal and nervous systems maintain balance during walking remains challenging. Instability is difficult to quantify in real-life settings and no fully developed wearable system is currently available to measure it outside the laboratory. Treadmills provide a safe, compact, standardized solution to generate surrogate slips and trips, enabling precise and repeatable measurements. Accurate assessment is critical for identifying at-risk of falls populations and evaluating rehabilitation. OBJECTIVE: To this aim, we tested a 3D dynamic criterion - the dyn

Systemic Lupus Erythematosus: What Every Clinician Needs to Know.

Systemic lupus erythematosus (SLE) is a chronic, multisystem autoimmune disease that disproportionately affects women of reproductive age and is associated with substantial morbidity, premature mortality, and racial and ethnic disparities. Advances in diagnosis and immunosuppressive therapy have improved survival, yet patients remain at increased risk for kidney failure, cardiovascular disease, infection, pregnancy complications, and treatment related damage. Primary care and hospital-based clinicians are often the first point of contact and are central to early recognition, evaluation, and co
