Application and Challenges of Chimeric Antigen Receptor T Cell Therapy in Systemic Rheumatic Diseases and Autoimmune Disorders.

Chimeric antigen receptor T (CAR-T) cell therapy, originally developed for hematologic malignancies, has emerged as a transformative candidate for systemic rheumatic diseases and autoimmune disorders (AIDs). Its unique efficacy in refractory AIDs relies on depleting autoreactive B cells and driving antigen-naïve immune reconstitution, achieving durable drug-free remission in early-phase trials. Despite promising clinical and serological responses lasting 2-5 years without long-term immunosuppression, the field faces unmet needs: complex manufacturing, limited tissue penetration, antigen escape

Next-Generation Antimicrobial Peptides for Biofilm-Associated Infections: Engineering, Biomaterial Delivery and AI-Assisted Discovery.

Antimicrobial peptides (AMPs) are increasingly regarded as next-generation antimicrobial agents because of their broad-spectrum activity, rapid killing, antibiofilm potential, immunomodulatory properties, and mechanisms of action that differ from those of many conventional antibiotics. Despite these advantages, their clinical translation remains limited by proteolytic instability, hemolysis or cytotoxicity, poor pharmacokinetics, salt and serum sensitivity, production costs, and delivery challenges. The AMP field is therefore shifting from natural peptide discovery toward integrated engineerin

Immunometabolic Reprogramming as a Therapeutic Platform: Integrating Systems Immunology with Precision Biopharmaceutical Engineering.

INTRODUCTION: Immunometabolism integrates energy demands, biosynthesis, redox homeostasis, and immune-cell function, yet metabolic phenotypes remain context dependent. This review conceptualizes immunometabolic reprogramming as a multiscale regulatory network connecting microenvironmental pressures, nutrient sensing, metabolic flux, metabolite-mediated regulation, immune-cell states, systems-level biomarkers, and precision therapeutic engineering. METHODS: A structured narrative review synthesized literature published from January 2022 onward across five domains: microenvironmental metabolic

Spatially Controlled Embedded Bioprinting of Prevascularized Spheroids-Containing Biopatch Enables Functional Skin Regeneration.

Functional vascularization remains a critical challenge in the regeneration of full-thickness skin defects. Here, we present a 3D embedded bioprinting strategy for engineering vascularized biopatches through the precise integration of prevascularized multicellular spheroids within a skin-derived decellularized extracellular matrix (SdECM) bioink. Rheological results reveal that a 1.0% (w/v) SdECM formulation exhibits yield-stress behavior, pronounced shear-thinning, and rapid shear recovery, enabling high-fidelity of spheroids while preserving cell viability and structural integrity. Using thi

Hydrogen releasing biomaterials for bone regeneration: mechanisms, design strategies, and applications.

Complex bone defects remain major clinical challenges due to their multifaceted pathological microenvironment. In recent years, hydrogen (H2) therapy has emerged as a rapidly developing strategy for bone repair, leveraging its redox-regulatory and other microenvironment-modulating effects. However, the rapid diffusion and short retention time of H2 limit its direct clinical application. The development of H2-releasing biomaterials partially addresses these limitations and offers a synergistic approach that integrates gas therapy with bone tissue engineering. Although recent studies on H2-relea…

Advances in RNA delivery to extrahepatic tissues and cells.

Despite their transformative potential in treating genetic, neurological, and oncological disorders, the clinical application of RNA therapeutics remains largely confined to the liver. Current approved platforms rely on two fundamentally distinct hepatic delivery mechanisms. N-acetylgalactosamine (GalNAc) conjugation achieves liver targeting through designed, high-affinity binding to the asialoglycoprotein receptor on hepatocytes, while lipid nanoparticles (LNPs) accumulate in the liver through intrinsic physicochemical tropism driven by apolipoprotein adsorption and the fenestrated hepatic va

Reframing Antimicrobial Peptides beyond Direct Antimicrobial Activity toward Host-Directed Functions and Disease-Specific Therapeutic Applications.

Antimicrobial peptides (AMPs) have been extensively investigated as alternatives to conventional antibiotics to combat multidrug-resistant bacteria. This narrow antibiotic-replacement framework underestimates the biological and translational potential of AMPs. The present review reframes them as disease-context-dependent therapeutic platforms rather than universal antibiotic substitutes. In particular, this review critically examines their microbiological basis, mechanisms of action, emerging applications, engineering strategies, formulation approaches, translational limitations, and criteria

Ionizable Lipid-Dependent Optimization of Steroid Lipid Nanoparticles With Tunable Immunomodulatory Properties.

Lipid nanoparticles (LNPs) are a leading platform for nucleic acid delivery, yet their intrinsic adjuvanticity poses a significant materials design challenge for applications requiring immunological quiescence. Here, we report a modular engineering strategy that incorporates FDA-approved corticosteroids into LNP formulations, creating a new class of steroid LNPs with tunable anti-inflammatory properties. Through systematic screening of steroid and cholesterol substitution ratios, we establish structure-property relationships governing mRNA encapsulation efficiency, physicochemical characterist

Cardiomyocytes in Hypoxia: Cellular Responses and Implications for Cell-Based Cardiac Regenerative Therapies.

Myocardial infarction (MI) is a severe hypoxic event, resulting in the loss of up to one billion cardiomyocytes (CMs). Due to the limited intrinsic regenerative capacity of the heart, cell-based regenerative therapies, which feature the implantation of stem cell-derived cardiomyocytes (SC-CMs) into the infarcted myocardium, are being developed with the goal of restoring lost muscle mass, re-engineering cardiac contractility, and preventing the progression of MI into heart failure (HF). However, such cell-based therapies are challenged by their susceptibility to oxidative stress in the ischemic

Progenitor-derived human endothelial cells evade alloimmunity by CRISPR/Cas9-mediated complete ablation of MHC expression.

Tissue engineering may address organ shortages currently limiting clinical transplantation. Off-the-shelf engineered vascularized organs will likely use allogeneic endothelial cells (ECs) to construct microvessels required for graft perfusion. Vasculogenic ECs can be differentiated from committed progenitors (human endothelial colony-forming cells or HECFCs) without risk of mutation or teratoma formation associated with reprogrammed stem cells. Like other ECs, these cells can express both class I and class II major histocompatibility complex (MHC) molecules, bind donor-specific antibody (DSA),…

Handheld Co-Axial Bioprinting: Application to in situ surgical cartilage repair.

Three-dimensional (3D) bioprinting is driving major innovations in the area of cartilage tissue engineering. Extrusion-based 3D bioprinting necessitates a phase change from a liquid bioink to a semi-solid crosslinked network achieved by a photo-initiated free radical polymerization reaction that is known to be cytotoxic. Therefore, the choice of the photocuring conditions has to be carefully addressed to generate a structure stiff enough to withstand the forces phisiologically applied on articular cartilage, while ensuring adequate cell survival for functional chondral repair. We recently deve…

Antibody-siRNA conjugates: drugging the undruggable for anti-leukemic therapy.

Generating effective RNAi-based therapies with the potential to achieve leukemia remission remains critical unmet need. Despite a growing number of leukemia clinical trials, tissue specific delivery of therapeutic siRNA is a major roadblock in translating its clinical potential. The most recent reports in the antibody-siRNA-conjugates (ARCs) field add new dimensions to leukemic therapy, where a covalently ligated therapeutic antisense-RNA with the potential to repress the oncogenic transcript is selectively delivered into the cancer cells. Despite ARC localization to leukemic cells due to high

Thiamin biofortification of crops.

Thiamin is essential for human health. While plants are the ultimate source of thiamin in most human diets, staple foods like white rice have low thiamin content. Therefore, populations whose diets are mainly based on low-thiamin staple crops suffer from thiamin deficiency. Biofortification of rice grain by engineering the thiamin biosynthesis pathway has recently been attempted, with up to 5-fold increase in thiamin content in unpolished seeds. However, polished seeds that retain only the starchy endosperm had similar thiamin content than that of non-engineered plants. Various factors such as

Alternative Non-Antibody Protein Scaffolds for Molecular Imaging of Cancer.

The development of improved methods for early detection and characterization of cancer presents a major clinical challenge. One approach that has shown excellent potential in preclinical and clinical evaluation is molecular imaging with small-scaffold, non-antibody based, engineered proteins. These novel diagnostic agents produce high contrast images due to their fast clearance from the bloodstream and healthy tissues, can be evolved to bind a multitude of cancer biomarkers, and are easily functionalized by site-specific bioconjugation methods. Several small protein scaffolds have been verifie
