Acute appendicitis and its treatment: a historical overview.

PURPOSE: Acute appendicitis (AA) is the leading cause of acute abdomen worldwide, with an incidence of 90-100 cases per 100,000 individuals annually and a lifetime risk of 7-12%. Despite its prevalence, historical accounts of AA are limited, particularly when compared to conditions like haemorrhoids, likely due to the appendix's internal location. This article traces the historical evolution of AA treatment from ancient times to the present, highlighting key contributions. METHODS: A review of common research databases and relevant literature on AA was conducted. RESULTS: Evidence from ancie

Eliminating Use of Home Oral Antibiotics in Pediatric Complicated Appendicitis.

BACKGROUND: Postoperative oral antibiotic management at discharge for perforated appendicitis varies by institution. A prior study at our institution led to a decrease in antibiotic therapy in patients without leukocytosis. A subsequent protocol change eliminated the white blood cell count check and oral antibiotics if discharge criteria were met by postoperative day seven. We hypothesized this change could be made without an increase in abscess or readmission rates. METHODS: We conducted a retrospective review of patients with perforated appendicitis over two 1-year periods after institution

A simple intervention to improve the use of postoperative antibiotics and intra-abdominal drains in appendicectomy patients.

INTRODUCTION: Appendicitis remains the most common intra-abdominal surgical emergency, yet many patients receive postoperative antibiotics and intra-abdominal drains against recommended guidelines. This study assesses the impact of an educational poster on the appropriate use of postoperative antibiotics and intra-abdominal drains in appendicectomy patients. METHODS: Patients who underwent an appendicectomy between January and April 2019 ('Poster absent' group) as well as between November 2019 and February 2020 ('Poster present' group) were identified. Data were collected on patient demograph

EMR of laterally spreading lesions around or involving the appendiceal orifice: technique, risk factors for failure, and outcomes of a tertiary referral cohort (with video).

BACKGROUND AND AIMS: EMR of sessile periappendiceal laterally spreading lesions (PA-LSLs) is technically demanding because of poor endoscopic access to the appendiceal lumen and the thin colonic wall at the base of the cecum. We aimed to assess the feasibility and safety of EMR for PA-LSLs. METHODS: Consecutive LSLs ≥20 mm and PA-LSLs ≥10 mm detected at 3 academic endoscopy centers from September 2008 until January 2017 were eligible. Prospective patient, procedural, and lesion data were collected. PA-LSLs were compared with LSLs in other colonic locations. RESULTS: Thirty-eight PA-LSLs were

Management and outcomes of appendicular neuroendocrine tumours: Retrospective review with 5-year follow-up.

BACKGROUND: Neuroendocrine (NEN) tumours are the commonest type of tumours affecting the appendix. The majority of cases are diagnosed incidentally on post-operative histopathological examination of the resected appendicectomy specimen. Preoperative diagnosis remains a challenge, unless the patient presents with obvious features of carcinoid syndrome or signs of metastatic disease. Hence, the authors present our five-year experience in diagnosing and managing NEN tumours of the appendix. METHODS: Retrospective review of all patients underwent an emergency appendicectomy with intention to trea

The effect of non-obstetric invasive procedures during pregnancy on perinatal outcomes.

PURPOSE: To evaluate the effect of non-obstetric invasive procedure during pregnancy on perinatal outcome. METHODS: The present retrospective study investigated perinatal outcome in women that underwent an invasive procedure during one of their pregnancies (n = 61); perinatal outcome was compared to other pregnancies (without an invasive procedure) of the same patients (n = 122). RESULTS: Women with a non-obstetric invasive procedure during pregnancy delivered earlier than those in the comparison group (38.5 vs. 40.0 weeks; p = 0.01) and had a significantly higher rate of cesarean sections (

Bringing surgeons together across the world: diagnosis and management of acute appendicitis.

INTRODUCTION: Despite appendicitis being one of the most common surgical emergencies in the pediatric population, there is still a great deal of debate among pediatric surgeons regarding the workup and treatment. MATERIALS AND METHODS: In an interactive Web symposium consisting of 130 pediatric surgeons from various regions of the world, questions about diagnosis and management of appendicitis were displayed on the screen. The audience was asked to respond to the poll questions. The questions asked to the participants pertained to an example case of a 12-year-old boy presenting to the emergen

[Endometrial and endocervical lesions associated with pseudomyxoma peritonei: a case report].

The association of a pseudomyxoma peritonei with a mucinous tumor of the appendix and/or the ovary is regularly reported in the literature. However, in this context, endometrial or endocervical lesions remain exceptional. We report the case of a 57-year-old patient, with pseudomyxoma peritonei associated with a low-grade mucinous neoplasia of appendix and both ovaries. The systematic sampling of endometrium and endocervix showed small segments of intestinal epithelium admixed with native epithelium. Histological and immunohistochemical characteristics of this epithelium were quite comparable t

Neonatal appendicitis: a new look at an old zebra.

Acute neonatal appendicitis is a rare condition associated with significant morbidity and mortality. The severity of this disease is caused by its tendency to occur more frequently in premature infants, an increased perforation rate with rapid progression to peritonitis, and delay in diagnosis and intervention. Although appendicitis in the perinatal period may occur as an isolated event, in many cases it occurs in association with other pathologic states, including prematurity, inguinal hernia, and others. The presentation of neonatal appendicitis can be identical to necrotizing enterocolitis,

Trocar injury of the retroperitoneal vessels followed by life-threatening postischemic compartment syndrome of both lower extremities.

BACKGROUND: Lesions of the intra-abdominal organs and vessels caused by trocars and Verres needles are rare but serious complications during laparoscopic surgery. REPORT: We report an unusual case of left common iliac artery and inferior vena cava injury during laparoscopy. This lesion was followed by a bilateral postischemic compartment syndrome of the lower extremities. The patient sustained massive rhabdomyolysis, renal failure, peroneal nerve palsy, and functional loss of the lower extremities. CONCLUSION: Postischemic tissue swelling should be recognized as early as possible because it

Laparoscopic robot-assisted appendicovesicostomy: an initial experience.

PURPOSE: To describe our initial experience with laparoscopic robot-assisted appendicovesicostomy (LRAA). MATERIALS AND METHODS: We have performed the LRAA in three patients, aged 9, 17, and 45 years. The underlying medical conditions were myelomeningocele, prune-belly syndrome, and multiple sclerosis. Adjuvant procedures consisted of extensive lysis of adhesions and a bladder-neck fascial sling. RESULTS: All procedures were completed intracorporeally. The mean blood loss was 50 mL. The mean operative time was 301 minutes (range 203-362 minutes). The mean hospital stay was 3 days (range 2-4

The utility of laparoscopy in the diagnosis of acute appendicitis in women of reproductive age.

AIM: To see whether laparoscopy improves the accuracy of a clinical diagnosis of acute appendicitis in women of reproductive age, and to determine what the long-term sequelae are of not removing an appendix deemed at laparoscopy to be normal. METHOD: The initial part of the study was undertaken during 1991-1992. Female patients between 16 and 45 years were eligible for inclusion once a clinical decision had been made to perform an appendicectomy for suspected acute appendicitis. Following consent, patients were randomized into two groups. One group had open appendicectomy, as planned. The oth

Prospective randomized trial of two wound management strategies for dirty abdominal wounds.

OBJECTIVE: To determine the optimal method of wound closure for dirty abdominal wounds. SUMMARY BACKGROUND DATA: The rate of wound infection for dirty abdominal wounds is approximately 40%, but the optimal method of wound closure remains controversial. Three randomized studies comparing delayed primary closure (DPC) with primary closure (PC) have not conclusively shown any advantage of one method over the other in terms of wound infection. METHODS: Fifty-one patients with dirty abdominal wounds related to perforated appendicitis, other perforated viscus, traumatic injuries more than 4 hours

National trends in the presentation of surgically resected appendiceal adenocarcinoma over a decade.

BACKGROUND: Rates of nonoperative management of acute appendicitis and appendiceal adenocarcinoma have increased over a decade, but the presentation and outcomes of appendiceal adenocarcinoma over this period is not well-characterized. METHODS: Patients with surgically resected Stage I-III appendiceal adenocarcinoma were identified from the 2006 to 2015 National Cancer Data Base and classified into two cohorts, 2006-2010 and 2011-2015, based on year of diagnosis. Three-year overall survival (OS) was analyzed using Cox proportional hazards regression and Kaplan-Meier survival estimates. RESUL
