Propensity matched professional baseball pitchers with early trunk rotation demonstrate increased shoulder and elbow distractive forces with equivalent ball velocity.

BACKGROUND: Prior mixed cohort pitching biomechanic analyses have noted favorable pitching biomechanics and decreased throwing arm kinetics in pitchers with delayed trunk rotation. METHODS: Three hundred thirty-nine professional pitchers instructed to throw 8 to 12 fastball pitches were evaluated with 3-dimensional motion capture (480 Hz). Pitchers were identified as initiating trunk rotation before the moment of foot contact (EARLY, n = 33) and were propensity matched on the basis of height, age, and weight to pitchers who initiated trunk rotation after foot contact (LATE, n = 132) resulting

[A complex forearm injury caused by skateboarding].

Conservative treatment of forearm fractures in children is preferred. However, in a few cases an open reduction and internal fixation are required. The case of a 15-year-old boy with a complex forearm injury following a skateboard accident is reported. The surgical treatment of these injuries is presented and the indications for open reduction and internal fixation of forearm fractures in children are discussed.

Are the Pathologic Features of Enthesopathy, Tendinopathy, and Labral and Articular Disc Disease Related to Mucoid Degeneration? A Systematic Review.

BACKGROUND: Tendinopathy, enthesopathy, labral degeneration, and pathologic conditions of the articular disc (knee meniscus and ulnocarpal) are sometimes described in terms of inflammation or damage, while the histopathologic findings are often consistent with mucoid degeneration. A systematic review of the histopathology of these structures at diverse locations might reconceptualize these diseases as expected aspects of human aging. The potential benefits of this evolution might include healthier patient and clinician mindsets as well as a reduced likelihood of overdiagnosis and overtreatment

Screw Diameter and Use of a Washer Affect Symptomatic Implant Removal Rates in Pediatric Medial Epicondyle Fractures.

OBJECTIVE: Medial epicondyle fractures are a common pediatric injury. When operative, cannulated partially threaded screws, with or without a washer, are commonly utilized. These implants may need to be removed after full healing if symptomatic. There is mixed evidence regarding the influence of a washer on rates of implant removal, and the influence of screw size has not been studied. We aim to determine the rate of symptomatic deep implant removal for each fixation type and identify factors associated with the need for removal. METHODS: This was an IRB-reviewed, retrospective, case-cohort s

Below- vs above-elbow cast for distal radius fractures: is elbow immobilization really effective for reduction maintenance?

PURPOSE: The choice of the cast length in conservative management of distal radius fractures still represents a debated controversy. Historically, the elbow is immobilized to reduce the risk of secondary displacement; however, short-arm casts are currently felt to be equally effective with less complications and better patient comfort. This paper investigates whether immobilization of the elbow is actually effective in reducing the risk of loss of reduction in conservatively manipulated distal radius fractures. METHODS: We retrospectively studied 297 consecutive patients with distal radius fr

[Post-traumatic pure dislocation of the elbow: what is the outcome?].

Elbow dislocations are very frequent. We conducted a retrospective study of 40 patients with dislocations of the elbow whose data were collected between 2013 and 2015. All patients underwent emergency reduction under sedation or under general anesthesia. Treatment was completed with antalgic immobilization for about ten days followed by active mobilization. After a mean follow-up period of 24 months, flexion and extension mobility greater than 100ο was obtained in more than 80% of patients and only two cases had severe stiffness. Overall outcomes assessed using Mayo Elbow Performance Score ran

Reference Values of Flexion and Supination in the Elbow Joint of a Cohort without Shoulder Pathologies.

BACKGROUND: After surgery of the long head of the biceps tendon, the examination of the biceps brachii muscle function and strength is common clinical practice. The muscle strength is usually compared with the uninjured contralateral side or with a matched pair group assuming that the uninjured side can be used as an appropriate reference. HYPOTHESIS/PURPOSE: The purpose of this study was to define reference values of the supination and flexion strength in the elbow joint and to investigate the influence of the arm positions and various anthropometric factors. METHODS: 105 participants witho

Reduction of radial-head subluxation in children by triage nurses in the emergency department: a cluster-randomized controlled trial.

BACKGROUND: Radial-head subluxation is an easily identified and treated injury. We investigated whether triage nurses in the emergency department can safely reduce radial-head subluxation at rates that are not substantially lower than those of emergency department physicians. METHODS: We performed an open, noninferiority, cluster-randomized control trial. Children aged 6 years and younger who presented to the emergency department with a presentation consistent with radial-head subluxation and who had sustained a known injury in the previous 12 hours were assigned to either nurse-initiated or

Posterior elbow coverage using whole and split flexor carpi ulnaris flaps: a cadaveric study.

PURPOSE: The purpose of this study is to evaluate the coverage patterns of whole and split flexor carpi ulnaris (FCU) pedicle muscle flaps for posterior elbow soft-tissue defects. METHODS: Seventeen fresh-frozen cadaveric upper extremities were used. The whole FCU was raised to the dominant vascular pedicle and transposed proximally over the olecranon. The widths of coverage at 2-cm distances about the posterior elbow were measured. Widths were also measured after making 3 longitudinal cuts in the fascia and after suturing the muscle to adjacent soft tissue under tension. The FCU was also spl

Strain on the ulnar nerve at the elbow and wrist during throwing motion.

BACKGROUND: It is well known that cubital tunnel syndrome frequently occurs in throwing athletes. The cause of cubital tunnel syndrome is considered to be mechanical stimuli on the ulnar nerve in the cubital tunnel. The hypothesis of the present cadaveric study was that the ulnar nerve is subjected to longitudinal strain in the cubital tunnel during the throwing motion. METHODS: Four phases of throwing (stance, wind-up, middle cock-up, and early acceleration) were passively simulated in seven fresh-frozen transthoracic cadaveric specimens that were fixed in an upright position to allow free a

[Nerve affection in connection with bloodletting II. Four case reports].

Four blood donors with persistent pain and dysfunction of the upper extremity after venepuncture are presented. Physical examinations identified median nerve-affections in all and additionally a varying involvement of other nerves in the elbow region. Such complications appear to have developed following to a vascular lesion. They are rare but potentially disabling and should be known in blood banks and clinical departments to which these patients are referred.

Evaluation of intercostal to musculocutaneous nerve transfer in reconstructive brachial plexus surgery.

OBJECT: Direct coaptation of intercostal nerves (ICNs) to the musculocutaneous (MC) nerve was performed to restore elbow flexion in 25 patients with brachial plexus root avulsions. METHODS: Seventy-five ICNs were transected as close as possible to the sternum to obtain sufficient length and then tunneled to the axilla and coapted to the MC nerve. Direct coaptation was achieved in 95% of ICNs, and functional elbow flexion was regained in 64% of the patients. The results were compared with several reported transfer techniques in which either an ICN or other donor nerves were used. CONCLUSIONS:

The latissimus dorsi flap for reconstruction of the brachium and shoulder.

The latissimus dorsi was transferred on its neurovascular pedicle to reconstruct the shoulder or brachium in nineteen patients. Group I consisted of seven patients in whom transfer of the latissimus dorsi was used only to obtain active flexion of the elbow. Although there was complete necrosis of the transferred muscle in one patient, six patients achieved an average of 111 degrees of active flexion and full extension of the elbow. There was only a modest gain in active supination because of pre-existing pronation contractures. The three patients in Group II had sustained loss of the flexor mu

Electrical stimulation over the human vertebral column: which neural elements are excited?

Supramaximal percutaneous electrical stimuli applied over the human cervical vertebral column produce maximal compound muscle action potentials (CMAPs) in abductor digiti minimi. It is important to know which neural elements are excited by these stimuli and experiments were performed to answer this question. With stimulating electrodes placed progressively lateral to the midline, submaximal CMAPs with the same latency are produced. With shocks over the cervical vertebrae in the midline, the threshold for excitation of arm muscles is much lower than for excitation of leg muscles. Comparison of

Elbow Arthroscopy for Treatment of Osteochondritis Dissecans of the Capitellum.

BACKGROUND: Osteochondritis dissecans (OCD) of the elbow is an idiopathic lesion of subchondral bone resulting in delamination and sequestration with or without articular cartilage involvement; for patients with persistent symptoms, surgical treatment is often recommended. While traditionally performed in the supine or prone position, arthroscopic debridement and microfracture of OCD of the capitellum in the lateral decubitus position permits strategic arm positioning and placement of portals to optimize visualization and reduce the risk of damage to neurovascular structures. INDICATIONS: Art

Comparison of lateral approach versus anterolateral approach with Herbert screw fixation for isolated coronal shear fractures of humeral capitellum.

BACKGROUND: For coronal shear fractures of humeral capitellum, the lateral approach is the most commonly used surgical approach. However, exposure range of the anterior aspect of the distal humerus is inadequate. The anterolateral approach has also been adopted to overcome this disadvantage. However, this approach seems anatomically complex due to the risk of iatrogenic injury to the radial nerve. So far, the optimal approach for the treatment of capitellar shear fractures remains inconclusive. The purpose of this study is to prospectively review and compare the early clinical and radiographic
