Case series on photobiomodulation in wound care: hard-to-heal leg ulcers and radiation-induced fibrosis.

Photobiomodulation (PBM) uses red and near-infrared light to stimulate tissue repair, reduce inflammation and provide analgesia. This case series presents four patients receiving adjunctive PBM for hard-to-heal wounds across community nursing and radiotherapy late-effects settings. Cases 1 and 2 involved patients with hard-to-heal leg ulcers and significant comorbidity; both received biweekly PBM over 6 weeks, with progressive reductions in wound area and pain, and complete closure in case 2 at 8 weeks. Cases 3 and 4 involved radiation-induced fibrosis with non-healing ulceration following che

Differential contributions of lower leg muscle activation to plantar flexor function during single-leg hopping one year after Achilles tendon rupture: A shift from monoarticular to biarticular muscle work.

Loss of plantarflexion function persists indefinitely after an Achilles tendon rupture, detrimentally affecting quality of life. This study aimed to quantify lower leg muscle activation and its contribution to side differences in plantarflexion work during single-leg hopping one year after Achilles tendon rupture. Thirty-seven participants of mean (SD) age 48 (8.9) years participated in the study. Ankle kinetics and electromyograms of the soleus, tibialis anterior, lateral and medial gastrocnemius muscles were recorded during single-leg hopping. Side-to-side differences in plantarflexion power

Lower leg muscle activation during the ebbets foot drills.

Lateral ankle sprains (LAS) often lead to chronic ankle instability (CAI). The Ebbets foot drills were created to strengthen the lower leg muscles and reduce the risk of LAS. The current study aimed to explore the activation of the lower leg muscles during the Ebbets foot drills. Twenty-two (22) college students without LAS participated in the study. Surface electromyography (sEMG) of the tibialis anterior (TA), tibialis posterior (TP), and peroneus longus (PL) was collected during each of the Ebbets foot drills and a normal walking trial. The sEMG mean root mean square (RMS) was calculated fo

Assessing Risk of Iatrogenic Peroneal Nerve Injury in All-Inside Lateral Meniscal Repair Between Standard Vs. Arthroscopic MRI.

BACKGROUND: Various studies have examined the risk of peroneal nerve injury with use of standard magnetic resonance imaging (MRI) of the knee. However, the position of the knee during standard MRI is different from that during an actual arthroscopic lateral meniscal repair. The purpose of the present study was to evaluate and compare the risk of peroneal nerve injury during simulated all-inside lateral meniscal repairs on the basis of MRI scans made with the knee in both positions. METHODS: With use of axial MRI scans made with the knee in the standard position (i.e., in 30° of flexion) and i

Patellofemoral Angle, Pelvis Diameter, Foot Posture Index, and Single Leg Hop in Post-Operative ACL Reconstruction.

Background and Objectives: Anterior cruciate ligament (ACL) injuries occur as a result of the deterioration of the static and dynamic stability of the knee. One of the structures involved in providing static stability is the patellofemoral angle (Q angle). The aim of this study was to investigate the relationships between Q angle, pelvis diameter, lower extremity length, and foot posture index (FPI) in patients who had undergone ACL reconstruction (ACLR) with the semitendinosus/gracilis (ST/G) technique on both the operated and non-operated sides. Materials and Methods: Twenty-five male recrea

Reconstruction of Major Burns With Stevens-Johnson Syndrome: A Case Report and Literature Review.

Stevens-Johnson syndrome (SJS) is a severe type of pleomorphic erythema and a rare disorder of the skin and mucous membranes, which can lead to serious infections, pulmonary embolism, acute respiratory distress syndrome, multiple organ dysfunction syndrome, and other serious consequences. Patients with SJS are usually treated in burn centers. SJS complicated by severe burns is very rare, and this is associated with a high risk of infection and other more serious complications. With SJS, the availability of donor sites is compromised given the lack of healthy epidermis, and this makes it more d

Cyclic eccentric stretching induces more damage and improved subsequent protection than stretched isometric contractions in the lower limb.

PURPOSE: Controversy remains about whether exercise-induced muscle damage (EIMD) and the subsequent repeated bout effect (RBE) are caused by the stretching of an activated muscle, or the production of high force at long, but constant, muscle lengths. The aim of this study was to determine the influence of muscle fascicle stretch elicited during different muscle contraction types on the magnitude of EIMD and the RBE. METHODS: Fourteen participants performed an initial bout of lower limb exercise of the triceps surae. One leg performed sustained static contractions at a constant long muscle len

Reduction of Morbidity With a Reverse-Flow Sural Flap: A Two-Stage Technique.

The reverse sural flap has often been used for cutaneous coverage of the distal region of the leg and ankle. When the flap is performed in 2 stages, the vascular pedicle is exteriorized and later resected. Our goal was to assess the reverse sural flap performed in 2 stages regarding its viability and low morbidity along the flap-donor area. Eleven patients with cutaneous coverage loss found in the area between the distal third of the leg and ankle underwent cutaneous coverage surgery with a reverse-flow sural flap with an exteriorized pedicle, without violation of the skin between the base of

PTEN hamartoma of the soft tissue: the initial manifestation of an underlying PTEN hamartoma tumor syndrome in a 4-year-old female.

A 4-year-old female was referred to pediatric orthopedic surgery for left leg pain and limping for 3 months following a motor vehicle collision. Recently, the patient's mother had noted left knee swelling and dragging of the left leg when walking. Past medical history was significant for hip dysplasia with slight leg length discrepancy. The patient was otherwise healthy. Physical examination was remarkable for left pre-patellar soft tissue fullness with normal range of motion. There was no warmth or tenderness. Subsequent ultrasound revealed a heterogeneous soft tissue mass superior and medial

SENSORIMOTOR REGULATION OF MOVEMENTS: NOVEL STRATEGIES FOR THE RECOVERY OF MOBILITY.

A series of observations have provided important insight into properties of the spinal as well as supraspinal circuitries that control posture and movement. We have demonstrated that spinal rats can regain full weight-bearing standing and stepping over a range of speeds and directions with the aid of electrically enabling motor control (eEmc), pharmacological modulation (fEmc), and training. Also, we have reported that voluntary control movements of individual joints and limbs can be regained after complete paralysis in humans. However, the ability to generate significant levels of voluntary w

Regional Anesthesia for Painful Injuries after Disasters (RAPID): study protocol for a randomized controlled trial.

BACKGROUND: Lower extremity trauma during earthquakes accounts for the largest burden of disaster-related injuries. Insufficient pain management is common in resource-limited disaster settings, and regional anesthesia (RA) may reduce pain in injured patients beyond current standards of care. To date, no controlled trials have been conducted to evaluate the use of RA for pain management in a disaster setting. METHODS/DESIGN: The Regional Anesthesia for Painful Injuries after Disasters (RAPID) study aims to evaluate whether regional anesthesia (RA), either with or without ultrasound (US) guidan

Rehabilitation assessment for lower limb disability based on multi-disciplinary approaches.

Low limb rehabilitation training is recognized as a very effective technique to facilitate body recovery. To make rehabilitation more efficient, we need to monitor the whole progress and detect how well the patient improves. The physician could make an optimal treatment plan according to the patient's improvement only when the patient's condition is correctly evaluated. Also, it is essential to provide a rehabilitation assessment system which would enable more accurate tracking of patient's status and minimize the requirement of time-consuming manual evaluations conducted by skilled person.
