Moderate hypofractionated radiotherapy for localised prostate cancer: A new standard of care.

Radical radiotherapy is an effective treatment and standard of care for localised prostate cancer. Most conventionally treatment has been given with 1.8 to 2.0Gy fractions to a dose of 74 to 79.2Gy. Radiobiology insights have encouraged the exploration of hypofractionated schedules. Over 7000 patients have contributed to eight randomised controlled trials comparing conventional fractionation with modest hypofractionation using 2.4 to 3.4Gy fractions. These studies have used both non-inferiority statistical designs aiming to define isoeffective and isotoxic moderate hypofractionated radiotherap

Use of Quadruple Therapy in the Management of Hypertension: A Systematic Review of Randomized Controlled Trials.

Background and objectives: Hypertension remains a leading global cause of cardiovascular morbidity and mortality, with suboptimal blood pressure (BP) control despite available treatments. Monotherapy often fails to achieve target BP, necessitating combination therapies. Quadruple low-dose combination therapy (quadpill) has emerged as a potential strategy to enhance efficacy while minimizing side effects. This systematic review evaluates the effectiveness and safety of quadpill therapy compared to standard monotherapy and dual therapy. Methods: A systematic search was conducted in PubMed, Web o

Reporting quality and evidence support in randomized controlled trials of herbal medicine formulas for vestibular migraine.

BACKGROUND: Vestibular migraine (VM) is a prevalent condition characterized by recurrent episodes of dizziness, vertigo, and/or headache, often posing diagnostic and therapeutic challenges. Numerous randomized controlled trials (RCTs) have highlighted potential benefits of herbal medicine formulas (HMFs) for treating VM. However, current evidence remains insufficient to recommend their routine clinical use. The validity of trial results relies heavily on the completeness of the reporting. Therefore, this study aimed to comprehensively evaluate the reporting quality and evidence support of RCTs

Adverse effects of Eye Movement Desensitization and Reprocessing therapy: A neglected but urgent area of inquiry.

While Eye Movement Desensitization and Reprocessing (EMDR) is widely recognized as an effective treatment for posttraumatic stress disorder (PTSD), its safety profile remains underexamined. This review critically evaluates the extent to which adverse effects are reported and monitored in EMDR research. We analyzed 51 randomized controlled trials from recent meta-analyses on EMDR for PTSD and found that only nine studies mentioned adverse effects, with just one employing systematic assessment protocols. In five of the nine studies, patients did report adverse effects, typically mild and tempora

Recent advances in understanding the role of antidepressants to manage breathlessness in supportive and palliative care.

PURPOSE OF REVIEW: Breathlessness is a prevalent and distressing symptom in palliative and supportive care, with limited licensed pharmacological options once disease-directed therapies are no longer effective. Antidepressants have been proposed as a potential treatment, even in the absence of comorbid mood disorders, due to their modulation of neural circuits and serotonin pathways involved in breathlessness perception. Despite their off-label use in clinical practice for managing refractory or chronic breathlessness, robust evidence supporting their efficacy is needed. This review critically
