Update on autoimmune polyendocrine syndromes (APS).

Autoimmune Polyendocrine Syndromes (APS) were initially defined as a multiple endocrine gland insufficiency associated to an autoimmune disease in a patient. Neufeld & Blizzard (1980) suggested a classification of APS, based on clinical criteria only, describing four main types. APS-1 is characterized by presence of chronic candidiasis, chronic hypoparathyroidism, Addison's disease. It is a very rare syndrome interesting young subjects correlating to different mutations of AIRE (AutoImmuneRegulator) gene on chromosome 21. APS-2 is characterized by presence of Addison's disease (always present)

An endocrinological emergency masquerading as an overdose.

Primary adrenocortical insufficiency (Addison's disease) is a potentially life-threatening endocrinological disorder caused by destruction of the adrenal cortex with consequent reduction in glucocorticoid and mineralocorticoid function. Although this is a rare condition, misdiagnosis can result in fatal consequences. Previous case reports have described the neuropsychiatric manifestations of Addison's disease which can result in misdiagnosis. Here the authors describe how a psychiatric history combined with atypical biochemical results can point towards a mistaken diagnosis of poisoning and he

Serious complications of tuberculous epididymitis.

Tuberculous epididymitis is a rare entity associated with minor complications. We present two cases of tuberculous epididymitis associated with serious complications (bilateral psoas abscesses and Addison's disease with psoas abscess). A review of the literature disclosed six additional cases associated with serious complications (Addison's disease, inappropriate antidiuretic hormone secretion, central nervous system involvement) which are discussed and compared to these cases. We conclude that tuberculous epididymitis represents a grave sequela of genital tract involvement and may be associat

Changes in cortisol metabolism following rifampicin therapy.

A patient with Addison's disease required increased corticosteroid dosage whilst receiving rifampicin. The pharmacological half-life of cortisol was reduced, but returned to normal when rifampicin was stopped. Cortisolproduction rates in four patients with pulmonary tuberculosis rose during treatment with rifampicin, as did urinary D-glucaric-acid excretion, an index of liver microsomal-enzyme activity. The alteration of the corticosteroid requirement in the patient with Addison's disease and the elevation of the cortisol-production rates were attributed to increased cortisol catabolism follow

Autoimmune-associated lymphoid tyrosine phosphatase is a gain-of-function variant.

A SNP in the gene PTPN22 is associated with type 1 diabetes, rheumatoid arthritis, lupus, Graves thyroiditis, Addison disease and other autoimmune disorders. T cells from carriers of the predisposing allele produce less interleukin-2 upon TCR stimulation, and the encoded phosphatase has higher catalytic activity and is a more potent negative regulator of T lymphocyte activation. We conclude that the autoimmune-predisposing allele is a gain-of-function mutant.

[Pernicious anemia: diagnosis and course in Burkina Faso].

Pernicious anemia (also known as Biermer disease or anemia, Addison or Addisonian anemia, and Addison-Biermer anemia) is an autoimmune atrophic gastritis responsible for vitamin B12 malabsorption due to a deficiency of intrinsic factor. We report eight cases of pernicious anemia in Burkina Faso, collected over a 44-month period. The three criteria for diagnosis of pernicious anemia were: vitamin B12 deficiency, gastric disease (gastric histology) with presence of anti-intrinsic factor, and/or anti-gastric parietal cell antibodies in serum. All patients had anemia, with a mean hemoglobin level

[Autoimmune polyglandular syndrome].

Among many etiologies for hypoparathyroidism, one of the inheritable forms of hypoparathyroidism, called as autoimmune polyglandular syndrome (APS), appears as a complex of hypofunction of several endocrine glands, candidiasis, pernicious anemia and vitiligo. Idiopathic hypoparathyroidism in APS typically presents by 20 years of age. Among the three most components of APS I, candidiasis is usually the first manifestation. Hypoparathyroidism almost invariably precedes the onset of Addison's disease. One should remember that Addison's disease can mask the presence of hypoparathyroidism and that

[Autoimmune diseases in type 1A diabetes mellitus].

Type 1A diabetes (DM1A) is an autoimmune disease that comprises 10% of patients with diabetes mellitus. Its frequency is gradually increasing in countries like Mexico. Patients with DM1A commonly have hypothyroidism, Addison disease, celiac disease and less common diseases such as polyglandular syndrome. These diseases are related to susceptibility genes such as HLA, CTLA-4 and PTPN22, which induce central and peripheral immunologic tolerance. This review article emphasizes the importance of searching other autoimmune diseases in patients with DM1A, to improve their prognosis and quality of li…

Common hyperpigmentation disorders in adults: Part I. Diagnostic approach, café au lait macules, diffuse hyperpigmentation, sun exposure, and phototoxic reactions.

The cause of hyperpigmentation usually is traced to the activity and presence of melanocytes. Café au lait macules may be solitary benign findings or may indicate the presence of neurofibromatosis with its associated complications. Diffuse hyperpigmentation should prompt a search for offending medications or systemic diseases such as hemochromatosis, hyperthyroidism, and Addison's disease. In these instances, the hyperpigmentation may be ameliorated by discontinuing offending medications, performing serial phlebotomy in patients with hemochromatosis, instituting cause-specific treatments in pa

The incidence of parathyroid and other antibodies in the sera of patients with idiopathic hypoparathyroidism.

Parathyroid antibodies were demonstrated in 38% of seventy-four patients with idiopathic hypoparathyroidism, 26% of ninety-two patients with idiopathic Addison's disease, 12% of forty-nine patients with Hashimoto's thyroiditis, and in 6% of 245 control patients. A significant percentage of patients with idiopathic hypoparathyroidism also had adrenal, thyroid and/or gastric parietal cell antibodies, even in the absence of diagnosed Addison's disease, thyroid disease, or pernicious anaemia. The antibody(s) against parathyroid tissue are not against parathyroid hormone, and do not cross-react in

Sparse hair and multiple endocrine disorders in two women heterozygous for adrenoleukodystrophy.

We describe two sisters (40 and 42 years old) heterozygous for adrenoleukodystrophy who have multiple endocrine disorders. In addition to the characteristic neurological symptoms, the younger patient has Addison disease and primary hypothyroidism attributable to autoimmune thyroiditis, and the older one has Graves disease. Both patients have loss of body hair and sparse scalp hair, which have not been reported previously in women heterozygous for adrenoleukodystrophy. After the institution of glucocorticoid replacement therapy, the younger sister, who has adrenal insufficiency, has shown unexp…

Endocrine disorders: causes of hyponatremia not to neglect.

Hyponatremia is a common electrolyte abnormality with the potential for significant morbidity and mortality. Endocrine disorders, including adrenal deficiency and hypothyroidism, are uncommon causes of hyponatremia. Primary adrenal insufficiency (i.e. Addison's disease) may well be recognized by clear hall-marks of the disease, such as pigmentation, salt craving, hypotension, and concomitant hyperkalemia. Addison's disease is an important diagnosis not to be missed since the consequences can be grave. On the other hand, hypothyroidism and secondary adrenocortical insufficiency originating from

[Thyrotropic function of the hypophysis in endocrine diseases].

The content of thyrotropic hormone of the hypophysis (by radioimmunological method) and the level of thyrotropic hormones in the blood plasma were studied in 178 patients with various endocrine pathology (thyrotoxicosis, hypothyroidism, diabetes mellitus, Itsenko-Cushing disease, Addison's disease, adiposity). Healthy persons (33) served as control. Thyrotropic function of the hypophysis proved to be decreased in thyrotoxicosis, diabetes mellitus, adiposity, Itsenko-Cushing disease. Comparison of the plasma level of thyrotropin and thyroid hormones led to a supposition on a different mechanism

Acquired primary endocrine failure in adult Ethiopian patients.

Over a 4 year period, February 1986 to January 1990, 18 patients with acquired primary endocrine failure were seen in the weekly endocrine clinic of Tikur Anbessa teaching and referral hospital in Addis Abeba. Eight patients had hypothyroidism and 4 Addison's disease; 3 of the 5 hypothyroid and 2 of the 3 Addison's cases on whom autoantibodies were determined were positive. One patient had polyglandular failure, involving the adrenal and thyroid glands and the gonads. In one patient tuberculosis was the cause of Addison's disease and 2 patients each had hypothyroidism and hypoparathyroidism, r

Primary adrenal lymphoma associated with adrenal insufficiency: a distinct clinical entity.

We describe a case of a 42 year old male who presented with Addison's disease resulting from primary lymphoma of the adrenals. Our case and a review of the literature indicates that this distinct entity has some unique clinical and radiologic features. In this entity, the lymphoma tends to be extranodal and have a poor prognosis. In addition, the computed tomography (CT) images have the unique appearance of enlargement of the adrenal gland with maintenance of the adreniform shape. We suggest that primary adrenal lymphoma is a distinct clinical entity and should be considered in patients with a

A Rare Presentation of a Common Disease: A Case Report.

Addison's disease (AD), or primary adrenal insufficiency, was first described by Thomas Addison in patients with adrenal tuberculosis (TB). Over the past several decades, along with the introduction of antituberculous treatment (ATT), the incidence of both has declined. The most common symptoms are nonspecific, leading to delayed diagnosis; patients may first present with a life-threatening crisis. Here, we report a case of AD that, upon further workup, was found to be due to one of the most common infections in India-TB.

Multiple Disease Associations in Autoimmune Polyglandular Syndrome Type II.

OBJECTIVE: Autoimmune polyglandular syndrome type II (APS II) is characterized by adrenal insufficiency (Addison's disease), autoimmune thyroid disease, and/or type 1 diabetes mellitus (DM1). Multiple other autoimmune diseases have been associated with APS II. Here we report a case of a patient with APS II who over the course of 10 years developed Addison's disease, hypothyroidism, DM1, Hashimoto's encephalopathy, vitiligo, celiac disease, seronegative arthritis, and ulcerative colitis. This is a particularly aggressive course of APS II, and this combination of autoimmune diseases has not been

End-Stage Renal Disease in a Patient With Schmidt's Syndrome: A Case Report.

Schmidt's syndrome constitutes Addison's disease in conjunction with autoimmune hypothyroidism or type 1 diabetes mellitus. It has misleading symptomology and an unclear order of presentation of symptoms. This often results in missed and late diagnosis. Chronic kidney disease is a rarely reported phenomenon in Schmidt's syndrome. Multiple factors may have the potential to cause renal failure, such as Addison's disease and/or hypothyroidism, the understanding of which is still evolving. A 45-year-old gentleman who is a known case of Schmidt's syndrome presented to us with fatigue, anorexia, and

Autoimmune polyglandular syndrome type III in monozygotic twins: a case report.

Autoimmune polyglandular syndrome is characterized by the coexistence of several autoimmune diseases, affecting predominantly the endocrine glands. Autoimmune polyglandular syndrome type III, as a subdivision of autoimmune polyglandular syndrome type II, is the co-occurrence of autoimmune thyroid disease with other autoimmune disorders without Addison disease. We present a rare case of autoimmune polyglandular syndrome type III in monozygotic twins. One of the twins also had autoimmune leukopenia. To our knowledge, leukopenia is the first mentioned coexistence in the literature of autoimmune p…
