Middle ear effusion and sinusitis.

The present investigation comprises a retrospective and a prospective study on children with middle ear effusion (MEE). It is shown that, in the majority of patients, middle ear effusion was found in combination with an infective disease of other parts of the upper respiratory system, especially in the maxillary sinuses. Lavage of the infected sinuses resulted in resolution of the middle ear effusion in about 85 per cent of the cases, while adenoidectomy, in patients in whom adenoid enlargement was found together with middle ear effusion, had only a minor effect.

Hypoventilation during sleep in children who have lymphoid airway obstruction treated by nasopharyngeal tube and T and A.

Three children with sleep apnea, alveolar hypoventilation, apparent mental retardation, and poor growth associated with chronically enlarged tonsils and adenoids were treated with the use of a nasopharyngeal tube followed by tonsillectomy and adenoidectomy. The effectiveness of this therapy was documented by polygraphic recording of sleep stages and respirations, and by correlation with serial arterial blood gases and pH. The nasopharyngeal tube was well tolerated, easy to use, and effective in diagnosis and treatment. We suggest that its use be further evaluated in patients with obstructive a…

The management of residual OSA post-adenotonsillectomy in children with down syndrome: The experience of a large tertiary sleep service.

INTRODUCTION: Obstructive Sleep Apnoea (OSA) is common in children with Down Syndrome (DS). Adenotonsillectomy is regarded as first line treatment for OSA but does not always lead to resolution of symptoms in this group of children. Management of residual OSA is variable with no existing recommendations to guide clinical practice. AIM: To describe the experience of a large tertiary sleep service in managing residual OSA in children with DS following upper airway surgery (adenotonsillectomy, adenoidectomy or tonsillectomy). METHODS: A retrospective study of children who were under evaluation …

MODERN APPROACHES TO CHOOSING SHAVER BLADE FOR ENDOSCOPIC SURGERY OF THE NASOPHARYNX AND PARANASAL SINUSES USING 3D MODELING.

OBJECTIVE: The aim: To determine the effectiveness of the use of 3D printed templates of shaver blades for choosing the optimal blade shape for endoscopic surgery of the nasopharynx and paranasal sinuses. PATIENTS AND METHODS: Materials and methods: The shaver blade templates with bending angles of 40º, 60º, 90º and 120° for powered endoscopic sinus surgery were made according to the manufacturer catalog using the Asiga 3D printer and Dental TOOTH material. There were examined 100 patients who underwent endoscopic powered sinus interventions (50 - adenoidectomy, and 50 - removal of the maxill…

Oxymetazoline vs ofloxacin vs ciprofloxacin/dexamethasone- effects of drops on tympanostomy tube postoperative otorrhea.

OBJECTIVE: To assess for differences in postoperative otorrhea rates after tympanostomy with tube placement surgery comparing use of oxymetazoline, ofloxacin, or ciprofloxacin/dexamethasone drops prescribed in the postoperative period. METHODS: A retrospective review was conducted of 516 pediatric patients who had either bilateral or unilateral myringotomy with tube placement performed during the year 2018. Information collected from each surgery included whether there was effusion at time of surgery, type of effusion, whether an adenoidectomy was performed the same time or prior, prior histo…

Assessment of Central Sleep Apnea Events in Children with Sleep-Disordered Breathing.

PURPOSE: To determine the prevalence of central apnea (CA) events and central sleep apnea (CSA) in children with sleep-disordered breathing (SDB) and to assess the effect of tonsillectomy and adenoidectomy (TA) on CSA in children with obstructive sleep apnea (OSA). Material and Methods. The medical charts of children with SDB were reviewed to obtain information on past medical history, polysomnography (PSG) findings, and surgical management. Counts and indexes of obstructive apnea, obstructive hypopnea, and central apnea were evaluated before and after TA. The prevalence of CSA and the effect …

Effect of adenoids and tonsil tissue on pediatric obstructive sleep apnea severity determined by computational fluid dynamics.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is a respiratory disorder caused by the obstruction of the upper airway during sleep. The most common cause of pediatric OSA is adenotonsillar hypertrophy. Adenotonsillectomy is the first-line treatment for pediatric OSA; however, OSA persists in a significant number of patients due, in part, to the method of evaluating enlarged adenoids and tonsil tissue. The reason for these effects on OSA severity is not clear. This study aimed to establish a method to diagnose the need for adenoidectomy or tonsillectomy. METHODS: Twenty-seven Japanese childr…

Were collected data about proceedings PET placement, age of the patient at each PET, adenoidectomy, tonsillectomy and results for audiometry and tympanometry. RESULTS: Among the 90 patients with DS, 49 (54.4%) were male, median age of 58 months (15-143 months). In thi…

OBJECTIVES: To evaluate the myofunctional status in children submitted to adenoidectomy or adenotonsillectomy, correlating the pre and post-surgical patterns throughout a 24-month-period. To correlate the myofunctional alterations to the sort of surgery performed (adenoidectomy versus adenotonsillectomy) and to the predominant post-surgical breathing pattern (predominantly nasal versus allergic rhinitis). OBJECTIVES: To investigate the relationship between obstructive sleep apnea (OSA) syndrome and nocturnal enuresis (NE) in patients who required tonsillectomy or adenoidectomy. STUDY DESIGN: Retrospective chart review with prospective collection of data. METHODS: All charts of patients ages 2 to 18 years that had tonsillectomy or adenoidectomy over a 44 month period were reviewed for presence of NE and indication for surgery. MR imaging of Tornwaldt's cysts.

OBJECTIVE: The purpose of this study was to assess the characteristics of Tornwaldt's cysts as revealed by routine MR studies. MATERIALS AND METHODS: We retrospectively reviewed MR images of the brain in 1208 consecutive subjects who ranged in age from 3 weeks to 93 years (mean, 57.1 years). The signal intensity, shape, and size of Tornwaldt's cysts were assessed. Patients with Tornwaldt's cysts were then questioned about the presence of persistent nasal discharge, occipital headaches, and halitosis and an unpleasant taste in the mouth and about a history of adenoidectomy. RESULTS: Tornwaldt…

Video-Based Assessment of Tonsil Size in Children With Sleep-Disordered Breathing.

OBJECTIVE: To evaluate whether parent-captured oropharyngeal videos obtained using a handheld telemedicine device can assess tonsil size in children with sleep-disordered breathing compared with in-person clinical examination. METHODS: We conducted a prospective diagnostic accuracy study at a single tertiary pediatric referral center. A consecutive sample of 169 children scheduled for tonsillectomy, adenoidectomy, or adenotonsillectomy was enrolled. Current concepts in pediatric adenotonsillar disease.

More objective thinking regarding the indications for adenotonsillar surgery has gained new respect for these operations. Better understanding of the implications of adenoid disease in chronic otitis and sinusitis, along with appreciation of obstructive airway problems secondary to adenotonsillar hypertrophy, has changed the scope of the operation. Research is ongoing to determine the etiology of chronic antigenic stimulation as a cause for hypertrophy of these organs.
