The effect of timolol and acetazolamide on transient intraocular pressure elevation following cataract extraction with alpha-chymotrypsin.

Elevated intraocular pressure (IOP) early after cataract extraction with alpha-chymotrypsin is a well-described, common occurrence. To study the incidence of pressure increase and the efficacy of medical therapy, IOP was monitored every 12 hours beginning on the first postoperative day in 68 patients after otherwise uncomplicated intracapsular cataract extraction performed with one milliliter of alpha-chymotrypsin and wound closure with multiple sutures. Patients were randomly assigned to one of three groups: treatment with oral acetazolamide, treatment with topical timolol, or no treatment. W

[Experience in therapy and prophylaxis of epidemic keratoconjunctivitis (author's transl)].

101 patients with epidemic keratoconjunctivitis were treated with different eye drops: cortisone, antibiotics and P.V.P.-Iodine. The treatment of 19 patients with P.V.P.-Iodine showed that inflammatory symptoms disappeared rapidly; corneal complications however such as superficial keratitis could not be prevented. After the outbreak of epidemic keratoconjunctivitis, severe hygienic measures had been taken at the eye-clinic. On account of the hygienic prophylactic measures further infections could be prevented at the clinic almost completely.

Management of Dry Eye Toxicity After Treatment With 177Lu-PSMA-617 Radioligand Therapy.

Treatment options for patients with metastatic castration-resistant prostate cancer include use of radioligand therapy with 177Lu-PSMA-617. 177Lu-PSMA-617 is used to target prostate cancer cells selectively by targeting prostate specific membrane antigen (PSMA); however, PSMA is also expressed on lacrimal glands among other tissues. Herein, we report on a case of a Common Terminology Criteria for Adverse Events version 5 grade 3 dry eye event with concomitant blepharitis after administration of 177Lu-PSMA-617. The patient was managed with neomycin-polymyxin-dexamethasone 3.5-10000-0.1 ophthalm

Clinical review: topical ophthalmic use of cyclosporin A.

Cyclosporine A (CsA) is an immunomodulatory agent that primarily inhibits the proliferation and action of T cells. Systemic CsA has been used successfully in solid organ transplantation and noninfectious uveitis. Topical CsA with various formulations has been used in the field of ocular surface diseases since early 1980s. An ophthalmic emulsion 0.05% (Restasis, Allergan, Irvine, CA), was approved by the United States Food and Drug Administration in 2003 to treat dry eye syndrome. This article aims to evaluate the peer-reviewed published scientific literature and to define well-established uses

Abducent nerve paralysis during interferon alpha-2a therapy in a case of chronic active hepatitis C.

A 59-year-old woman with chronic active hepatitis C was treated with recombinant human interferon alpha-2a. After three days of administration, the patient complained of diplopia with dizziness and head heaviness. Ophthalmic examinations revealed a disturbance of the movement of left eye ball to the outer side without any other neurological signs. The diplopia, which was diagnosed as abducent nerve paralysis, improved rapidly and reversed at about 6 weeks after discontinuation of interferon and during infusion of hydrocortisone. To our knowledge, this is the first report of abducent nerve para

Idiopathic hypertrophic cranial pachymeningitis.

We evaluated 3 patients with biopsy-proven hypertrophic cranial pachymeningitis apparently unrelated to any systemic disease. Each patient had chronic headache, cranial neuropathy, an elevated ESR, and a mild CSF pleocytosis. Neuro-ophthalmic findings included bilateral sixth nerve palsies in two patients and the third had bilateral optic neuropathies. MR imaging revealed thickened dura that enhanced with Gd-DTPA administration. Histologic examination showed thickened, fibrotic dura with a sterile, chronic, nongranulomatous inflammation. The response to treatment was variable with corticostero

Preparation and in vivo and ex vivo studies of sirolimus nano-in-situ gel ophthalmic formulation.

Sirolimus (SR) is a macrolide with antifungal and antitumor immunosuppressant properties, classified as a selective inhibitor of mammalian target of rapamycin (mTOR). In this study, an ionic in situ gel of SR (SR-SUS-ISG) was formulated using gellan gum, exhibiting stability regardless of temperature and pH variations, causing minimal irritation. Harnessing the physiological conditions of the eye, SR-SUS-ISG underwent gelation upon contact with ions, increasing drug viscosity and prolonging retention on the ocular surface. Concurrently, SR-SUS-ISG displayed favorable shear dilution properties,

Ocular Drug Delivery Systems Using Contact Lenses.

The use of contact lenses as ocular drug delivery systems has been considered intuitive for decades. However, at this time, there are no approved products using such systems. In this article, we review the challenges with current therapies, pharmacokinetics, and pharmacodynamics of different drug classes and the patient population. In addition, we note the relative lack of clinical studies, and list potential products in active development at this time. In particular, we address the alignment of time course of the therapeutic need, the pharmacokinetics of the molecule, and the delivery charact

Prevention of macular edema in patients with diabetes after cataract surgery.

To evaluate the efficacy of topical steroids ± nonsteroidal antiinflammatory drugs (NSAIDs), depot steroids, and antivascular endothelial growth factors (anti-VEGFs) in preventing pseudophakic cystoid macular edema (PCME) after cataract surgery in patients with diabetes, a systematic literature search for randomized controlled trials published after 1990 was carried out in Cochrane, EMBASE, and PubMed databases. A meta-analysis was performed using risk ratios for PCME as the primary outcome and visual acuity, macular thickness, and adverse events as the secondary outcomes. Topical steroids in

Brimonidine suppresses loss of retinal neurons and visual function in a murine model of optic neuritis.

Optic neuritis is inflammation of the optic nerve and is strongly associated with multiple sclerosis (MS), an inflammatory demyelinating syndrome of the central nervous system. It leads to retinal ganglion cell (RGC) death and can cause severe vision loss. Brimonidine (BMD) is a selective α2-adrenergic receptor agonist that is used clinically for the treatment of glaucoma. BMD lowers intraocular pressure, but recent evidence suggests that its therapeutic efficacy may also mediate through mechanisms independent of modulation of intraocular pressure. In this study, we examined the effects of top

Giant Cell Arteritis Presenting with Ptosis and Diplopia.

Giant cell arteritis (GCA) is vasculitis of large-sized vessels that can lead to vision loss. We herein report a rare case of GCA accompanied by ptosis and diplopia as early symptoms, which were caused by third nerve palsy. A 78-year-old man presented with fever, right temporal headache, right eyelid ptosis, and diplopia. GCA was confirmed by a temporal artery biopsy. The symptoms disappeared after a slight delay following the administration of prednisolone. Unlike vision loss, ptosis and diplopia are considered to be reversible and responsive to treatment. GCA should not be ruled out if patie

Using Historical Data to Support Approval of Drug Products for Ophthalmia Neonatorum: Innovative Approach Toward Evidence-Based Medicine.

IMPORTANCE: Off-label treatment was common for ophthalmia neonatorum because only erythromycin ointment had been approved by the US Food and Drug Administration (FDA) for this indication. Ophthalmia neonatorum was previously considered a different indication from bacterial conjunctivitis in older children and adults because of uncertain similarities in the cause of disease and the treatment course between the 2 populations. Prospective therapeutic clinical studies were required to demonstrate the effectiveness of treatment for ophthalmia neonatorum. OBJECTIVE: To review the therapeutic clinic

Treatment of postoperative pain after ophthalmic surgery.

For ophthalmic surgery we have to deal with a wide range of different patient characteristics. We treat young healthy children, in some cases even neonates, but on the other hand we have debilitated aging patients with multiple concomitant diseases. Treatment of postoperative pain is imperative for inpatients, but is even more important for patients who are treated on an outpatient basis. There also is a wide range of different types of ophthalmic surgical procedures. The postoperative care after a cataract extraction is only seldom complicated by severe pain and is completely different of tha

Zeolitic imidazolate frameworks (ZIF-8) as a carrier in a topical ocular delivery system for the treatment of ocular diseases.

Current systems for topical ocular drug delivery enable non-invasive administration of therapeutics to the eye but suffer from poor permeability and bioavailability, limiting therapeutic efficacy. Metal-organic frameworks are porous, crystalline materials that can serve as drug delivery systems and may improve these shortcomings. In this study, we explore the use of ZIF-8 as a representative metal-organic framework for topical ocular drug delivery. ZIF-8 particles were prepared via a simplified aqueous synthetic method and loaded with ovalbumin, a model molecule to simulate anti-angiogenic pro
