Headache and Neurological Deficits with Cerebrospinal Fluid Lymphocytosis in a Helicopter Pilot.

BACKGROUND: The syndrome of transient headache and neurological deficits with cerebrospinal fluid lymphocytosis (HaNDL syndrome) consists of migraine-like headache episodes with >4 h of hemiparesthesia, dysphasia, and/or hemiparesis plus cerebrospinal fluid lymphocytic pleocytosis. While the rarity of HaNDL syndrome often precludes reassurance of the nature of this syndrome, it has consistently been identified as a benign condition that lasts no longer than 3 mo. CASE REPORT: In the first week of a viral illness, a 29-yr-old male helicopter pilot experienced acute-onset "stumbling" when walki

Remote sensing techniques for automated marine mammals detection: a review of methods and current challenges.

Marine mammals are under pressure from multiple threats, such as global climate change, bycatch, and vessel collisions. In this context, more frequent and spatially extensive surveys for abundance and distribution studies are necessary to inform conservation efforts. Marine mammal surveys have been performed visually from land, ships, and aircraft. These methods can be costly, logistically challenging in remote locations, dangerous to researchers, and disturbing to the animals. The growing use of imagery from satellite and unoccupied aerial systems (UAS) can help address some of these challeng…

Helicopter emergency medical services use of thoracic point of care ultrasound for pneumothorax: a systematic review and meta-analysis.

BACKGROUND: Auscultating for breath sounds to assess for pneumothorax in the helicopter emergency medical services (HEMS) settings can be extremely challenging. Thoracic point of care ultrasound (POCUS) offers a seemingly more useful visual (rather than audible) alternative. This review critically and quantitatively evaluates the use of thoracic POCUS for pneumothorax in the HEMS setting. METHODS: A systematic literature review with meta-analysis was conducted. Only papers reporting on patients undergoing POCUS for pneumothorax in the helicopter or pre-hospital setting were included. Primary

Prevalence of Low Back and Cervical Back Pain in Military Helicopter Crews: An Underestimated Italian Problem.

BACKGROUND: Studies have highlighted the incidence and prevalence of chronic pain, which is an epidemic problem in all career sectors, as well as estimated the economic loss that follows its pathology. Several studies have indicated a high incidence of chronic osteoarticular pain in military service members, particularly in flight personnel. To date, no studies have estimated the incidence of pain pathology in the Italian military population, despite the implications related to flight qualification. METHODS: A survey was conducted on helicopter flight personnel undergoing periodic annual eval

Investigations on the buckling and dynamics of diving-inspired systems when entering water.

This work provides comparative modeling approaches to determine the velocities and natural frequencies of plunge-diving bird and bioinspired drone systems when entering water. These systems are chosen to further explain the survival of diving birds as they impact water and to provide insight into the design geometry and material choice of bioinspired diving drones. A nonlinear reduced-order model is developed and utilized to analyze the dive at impact considering both Timoshenko and Euler-Bernoulli beam theories. Using Hamilton's principle, the equations of motion are first derived. Then, stat

Diagnostic performance of prehospital ultrasound diagnosis for traumatic pneumothorax by a UK Helicopter Emergency Medical Service.

OBJECTIVE: Up to 20% of major trauma patients may sustain a pneumothorax. Traumatic pneumothoraces can be difficult to diagnose on scene. Although the use of handheld ultrasound (HHUS) is becoming increasingly widespread, there remains uncertainty about its efficacy as a diagnostic tool in the prehospital setting. The aim of this study was to determine the diagnostic performance of prehospital chest HHUS in trauma patients. METHOD: Retrospective review of trauma patients who received a prehospital chest HHUS and subsequently conveyed to the Royal Sussex County Hospital (RSCH) between 1 July 2

Skull fracture with effacement of the superior sagittal sinus following drone impact: a case report.

BACKGROUND: The popularity of unmanned aerial vehicles, or drones, raises safety concerns as they become increasingly common for commercial, personal, and recreational use. Collisions between drones and people may result in serious injuries. CASE REPORT: A 13-year-old male presented with a comminuted depressed skull fracture causing effacement of the superior sagittal sinus secondary to a racing drone impact. The patient experienced a brief loss of consciousness and reported lower extremity numbness and weakness after the accident. Imaging studies revealed bone fragments crossing the superior

Air travel-associated venous thromboembolism.

Long-distance air travel is increasing and cases of venous thromboembolism (VTE) following air travel have attracted both considerable public attention and legal claims against airlines. VTE is a common disorder worldwide with a notably high incidence in older individuals. Many biochemical factors that lead to, or accentuate, thrombus formation are associated with increased risk of VTE. These factors include thrombophilia, activated protein C resistance and factor V Leiden, prothrombin gene mutation, antiphospholipid antibodies, protein S and protein C deficiencies, and methylene tetrahydrofol

[Fear of flying].

All reliable data and statistics point to the safety of flying. Nevertheless many opinion surveys indicate that every third in the population is affected by aviophobia in one way or the other. Aviophobia is simply a specific phobia from which men and women suffer the same degree. It can be found in all social strata and all occupational groups. Aviophobia is felt in very subjective manners and different ways. The structure of reasons of aviophobia can be very complex, whereas it is somewhat easier to identify immediately the causes. Depending upon the degree of aviophobia different strategies

[Is there a physician on board?].

Physicians responding to emergency calls on board airliners in intercontinental traffic may not be aware of certain legal complications which may arise. For instance, the medical practitioner may hold a license valid in one country, the air carrier may be registered in another, and the patient may be a third state national. Legislation varies between nations, as do court decisions. Physicians may be aware neither of the laws and regulations which apply nor the subtle differences between terms and interpretations used in formal language. This article contains a scenario description from a comme

Sickle cell anemia trait in the military aircrew population: a report from the Military Aviation Safety Subcommittee of the Aviation Safety Committee, AsMA.

The question of whether sickle cell trait (SCT) is potentially dangerous to military aircrew personnel who have it and, consequently, whether such individuals should be allowed to fly in military aircraft is a very emotional issue. This article traces the evolution of how the U.S. military has dealt with the problem, and the present status of individuals with SCT in the U.S. military aviation community. Extensive studies and means for subjectively evaluating the problem were instituted by the Department of Defense in 1981, after making the decision not to restrict aircrew with the trait from a…

In-Flight Assessment of Simulated Hemorrhage During Aerial Patient Evacuation.

INTRODUCTION: Understanding how flight trajectories impact hemodynamically unstable patients is critical for maintaining high survival rates during aerial evacuation. We integrated an experimental model of hemorrhage in an aircraft during evasive maneuvers to capture the physiological response in healthy humans vs. simulated hemorrhage. METHODS: Lower body negative pressure (LBNP), a validated method to simulate hemorrhage, and continuous physiological monitoring were integrated into a C-12J aircraft. Nine volunteers underwent incremental LBNP during wind-up turns, roller coasters, and level …

Transcranial Direct Current Stimulation and Aviator Performance During Simulated Flight.

INTRODUCTION: Transcranial direct current stimulation (tDCS) is a promising method for maintaining cognitive performance. Anticipated changes in rotary-wing aircraft are expected to alter aviator performance.METHODS: A single-blind, randomized, sham-controlled study evaluated effects of 2-mA anodal tDCS to the right posterior parietal cortex on aviator performance within a Black Hawk simulator. A mixed design with one between-subjects factor was assessed: stimulation prior to flight (20 constant min) and during flight (two timepoints for 10 min each). The within-subjects factor included active…
