The authors report a serie of five patients with five stumps above the elbow who needed a secondary reconstruction to allow or facilitate a prosthesis. They used in the first case an expansion of the latissimus dorsi flap and of the axillary and prepectoral region in order to free the stump of the humerus sutured on the thorax. In the second case, a free parascapular flap covered an unstable scar of the clavicula after a scapulothoracic amputation. In the third case, the transfer of a free flap of fibula associated with a pedicled latissimus dorsi flap had allowed the elongation of the stump o

The formation of a functional tibial stump after combat injuries with extensive tissue damage is sometimes difficult. We describe a case of reconstruction of the tibial stump after a mine-blast injury. In this case, the fibula was completely removed as a result of fracture, and the tibia was amputated at the border of the upper and middle thirds. To create a stable platform with a larger bearing surface area and reduce the load on the distal fossa, the Ilizarov method was used. For the first time, the area of the bearing surface of the tibia stump was increased by more than 2 times in the case

An exceptional case like our patient who was a 24-year-old man presented 6 weeks after type 4 Allen amputation to his right index finger where primary surgical stump closure was done, presented to us with the amputated distal part warm preserved. The reposition of distal amputated part using the authors described GRF (Graft Reposition on Flap) technique was done and followed. Good consolidation and bone graft union, good nail with near normal pulp and normal sensation with good functional outcome was achieved at the one-year final follow up.

Early, primary amputations are still necessary for certain patients sustaining high impact lower limb trauma. For cases with extensive proximal tibial bone loss a turn-up bone flap technique has been described to achieve a below-knee stump suitable for a prosthesis. However, in certain circumstances, for example if posterior soft tissues are injured, this type of reconstruction is not possible. This case report demonstrates that converting a severe open proximal tibial fracture to a successful below-knee amputation is also possible with acute limb shortening, flap cover and planned subsequent

In order to review major lower extremity amputations at the Toronto East General and Orthopaedic Hospital Inc. between 1979 and 1984, 60 patients with 42 below-knee, 17 mid-thigh and 20 Callander through-knee amputations were studied retrospectively. The three amputation groups demonstrated comparable rates of stump healing. A previous history of vascular surgery on the extremity increased the risk for both delayed healing and failure to heal. The Callander through-knee amputees were rehabilitated earlier and more easily than were the mid-thigh amputees, and once rehabilitated they were more l

There are several treatment modalities for zone 1 or zone 2 fingertip amputations that cannot be replanted by using microsurgical techniques, such as delayed secondary healing, stump revision, skin graft, local flaps, distant flaps, and composite graft. Among these, composite graft of the amputated digit tip is the only possible means of achieving a full-length digit with a normal nail complex. The pocket principle can provide an extra blood supply for survival of the composite graft of the amputated finger by enlarging the area of vascular contact. The surgery was performed in two stages. Spinal segmental myoclonus rarely occurs in peripheral neurological disorders. There are no data about the efficacy of topiramate (TPM) on spinal myoclonus. We describe a patient whose segmental myoclonus in amputation stump was ameliorated markedly by TPM.

Below-knee amputation in adult male rats resulted in characteristic quantitative and qualitative changes of the tibia stump and of the femur in the amputated leg. These changes were followed for periods of 1 to 32 weeks after the amputation. The femur of the amputated leg underwent a significant reduction of its total bone mass and a significant decrease of its percentage ash content, whereas its percentage water and organic contents increased significantly. The bone specific activity was initially decreased and began to increase after the 10th week post surgery with a positive linear regressi

Sixty-six patients with end-stage peripheral vascular disease who had undergone bilateral major amputation of the lower extremities in our institution during the 10-year period January 1980-December 1989 were reviewed. There were 46 males and 20 females with an age range from 34 to 91 years (mean 67.7 years). A 98.5% follow-up was achieved. Of these patients 25% underwent their second amputation in the first, 50% within the second and 75% within the third postoperative year, notwithstanding the fact that prior attempts at revascularisation had been performed in 62% of all patients. The 30-day

BACKGROUND: The treatment of painful neuroma remains challenging. Recently, a nerve-end capping technique using a bioabsorbable nerve conduit was newly introduced to treat amputation neuroma. A collagen-coated polyglycolic acid (PGA) conduit has been commercially available for the reconstruction of peripheral nerve defects, yielding successful clinical outcomes. However, no experimental research has been conducted using this PGA nerve conduit as capping device for treating amputation neuroma. The purpose of this study was to investigate nerve-end capping treatment with the PGA conduit in the r

A 39 year old man presented with recurrent episodes of leukocytoclastic vasculitis and a large ulcer at the distal part of the stump of his right leg. Since birth the patient showed a clinical picture of pseudoainhum as part of the amniotic band syndrome (Simonart syndrome). Radiologic examination of the right foot showed only an hypoplastic talus and calcaneus which had been designated as a "stump":, which have been detected by x-ray. On both upper limbs there were missing phalanges, syndactyly and some constrictive fibrotic bands. Since the ulcer worsened with therapy, the patient underwent

Whether for functional or cosmetic reasons in oncologic patients, following amputation or severe crush injuries of the lower limbs with complete severance of vessels and nerves, microsurgery enables a weight-bearing lower limb or femoral stump to be created if such a result cannot be achieved by transplanting plantar skin with an intact vessel-nerve bundle. When first fitting a prosthesis in a patient with a weight-bearing lower leg stump created by autologous grafting of plantar skin employing the microsurgical method, it is advisable initially to partially relieve the grafted covering tissue

BackgroundMajor lower limb amputations, often performed as lifesaving procedures in patients with advanced peripheral vascular disease (PVD), are associated with high rates of postoperative complications and reoperations. Identification of risk factors associated with reoperation is crucial for targeted perioperative management. The objective of the study is to identify the factors associated with increased risk of reoperation following major amputation and to develop and evaluate a predictive model for the same.MethodsA prospective observational study was conducted at our tertiary care centre

CASE: An 18-year-old woman with osteosarcoma of the left distal femur underwent resection and limb-salvage surgery with compressive osseointegration endoprosthetic reconstruction and experienced tumor recurrence 1 year after index surgery. A transfemoral amputation was performed preserving the existing compressive osseointegration device, and limb lengthening was achieved using intercalary and distal femoral segments. 3.5 years after amputation and lengthening, the implant remains well integrated and the patient is ambulatory.
CONCLUSION: Compressive osseointegration with distal femoral endop

The authors report on a young patient with previous radiation to her pelvis who presented with acute limb ischemia following iliac vein stenting believed to be secondary to extrinsic iliac artery compression in the setting of a frozen pelvis. She underwent revascularization and a trans-femoral amputation, ultimately needing a femoral to femoral artery crossover bypass in order to achieve amputation stump healing. This case describes a potential arterial complication of venous stenting in a previously irradiated field.

Three cases of below-knee stump reconstruction, using cross-legged, free sole flap, free foot filleted flap, and island pedicle sole flap, were reported. The variability of the flap size provided satisfactory coverage as needed in all cases. The causes of amputation were acute trauma and chronic osteomyelitis. The contour of the stump, the sensory recovery, the preserved range of motion of the knee joint, and the long-term durability were satisfactory in these patients. Some pitfalls of this procedure and criteria for evaluating the distal extremity soft tissue with respect to their suitabilit

One hundred and seventy patients with major lower limb amputation (MLLA) presenting to The National Prosthetic-Orthotic Centre (NPOC) in Khartoum over a 1-year period were studied. There were 141 males and 29 females giving a M:F ratio of 4.9: 1.0, with mean age of 37 years (range 5-72 years). Forty-one patients (24%) underwent amputation of diabetic septic foot, 30 patients (17.6%) underwent amputation as a result of trauma from road traffic accidents and Madura foot, and war injuries accounted for 29 amputations (17%). One hundred and eleven patients had below knee amputation (BKA), 52 had a
