Epidural injection of lidocaine reduces the response to dural puncture accompanying spinal needle insertion when performing combined spinal-epidural anesthesia.

During placement of needles for combined spinal-epidural anesthesia (CSEA), patients may experience pain, pressure, paresthesia, or discomfort during skin and deeper injection of local anesthetic, needle impingement on periosteum, dural puncture by the spinal needle, and insertion of the epidural catheter. We investigated the incidence of perception of and spontaneous verbal and motor responses to insertion of a spinal needle through the dura mater and pia mater and the effect of injecting lidocaine into the epidural space through the epidural needle before inserting the spinal needle through

A randomized prospective clinical study comparing wide-awake local anesthesia no tourniquet (WALANT) and spinal anesthesia in the surgical treatment of ankle fractures.

BACKGROUND: Ankle fractures are common injuries that often require surgical intervention to restore proper alignment and stability. Traditional anesthesia methods, such as general and spinal anesthesia, may not be suitable for patients with significant comorbidities. The wide-awake local anesthesia no tourniquet (WALANT) technique has emerged as a viable alternative, particularly for patients with cardiovascular or pulmonary conditions. This study aimed to compare the clinical outcomes of WALANT and spinal anesthesia in the surgical treatment of ankle fractures. METHODS: This prospective, ran

Comparison of maternal and neonatal outcomes between general anesthesia and combined spinal-epidural anesthesia in cesarean delivery for pregnancy complicated with placenta previa.

BACKGROUND: Placenta previa (PP) involves abnormal placental implantation in the lower uterine segment, partially or completely covering the internal cervical os, and is linked to severe maternal hemorrhage and fetal complications. The optimal anesthetic method between general anesthesia (GA) and combined spinal-epidural anesthesia (CSEA) for cesarean delivery (CD) with PP remains controversial. METHODS: We retrospectively analyzed 550 PP-complicated CD cases from the Longitudinal Placenta Previa Study (LoPPS) conducted in Southwest China between January 2018 and December 2024. Patients recei

Non-Axillary Chest Skin Temperature Underestimates Core Temperature During Redistribution Hypothermia Following Spinal Anesthesia.

Accurate temperature monitoring is essential for detecting perioperative hypothermia during spinal anesthesia, where rapid redistribution of body heat commonly occurs. Although zero-heat-flux (ZHF) thermometry has emerged as a noninvasive method for estimating core temperature, the reliability of chest skin temperature monitoring in this context remains unclear. In this prospective observational study, temperature measurements were obtained at 5-minute intervals using a forehead ZHF sensor, an infrared tympanic thermometer, and a non-axillary chest skin probe in patients undergoing orthopedic

Transforaminal Full-Endoscopic Ventral Facetectomy for Lumbar Lateral Recess Spinal Stenosis under Local Anesthesia.

This article describes a reproducible protocol for performing transforaminal full-endoscopic ventral facetectomy (TF-FEVF) under local anesthesia for lumbar lateral recess stenosis. Lateral recess spinal stenosis (LRSS) in the lumbar spine is generally treated with posterior decompression surgery under general anesthesia. The invasiveness of posterior decompression has decreased with the development of endoscopic techniques such as interlaminar full-endoscopic spine surgery and unilateral biportal endoscopy. However, these techniques still require general anesthesia. With population aging in d

Thoracic Segmental Spinal Anesthesia for Laparoscopic Surgeries in Pediatric Patients: A Retrospective Case Series.

Thoracic segmental spinal anesthesia (TSSA) is a developing regional anesthesia approach that involves administering local anesthetics into the thoracic spine to produce a targeted nerve block. This segmental block provides stable hemodynamics and early recovery. Lumbar spinal anesthesia is a common practice in children, and TSSA can be a promising technique due to its advantages. Despite the theoretical advantages of TSSA in pediatric patients, there is a paucity of literature describing its clinical use, safety profile, and outcomes in this population. This case series presents our initial c

Spinal Anesthesia for Awake Spine Surgery: A Paradigm Shift for Enhanced Recovery after Surgery.

Awake surgery has been applied for various surgical procedures with positive outcomes; however, in neurosurgery, the technique has traditionally been reserved for cranial surgery. Awake surgery for the spine (ASFS) is an alternative to general anesthesia (GA). As early studies report promising results, ASFS is progressively gaining more interest from spine surgeons. The history defining the range of adverse events facing patients undergoing GA has been well described. Adverse reactions resulting from GA can include postoperative nausea and vomiting, hemodynamic instability and cardiac complica

Spine surgery under awake spinal anesthesia: an Egyptian experience during the COVID-19 pandemic.

OBJECTIVE: Despite tremendous advancements in biomedical science and surgical technique, spine surgeries are still associated with considerable rates of morbidity and mortality, particularly in the elderly. Multiple novel techniques have been employed in recent years to adequately treat spinal diseases while mitigating the perioperative morbidity associated with traditional spinal surgery. Some of these techniques include minimally invasive methods and novel anesthetic and analgesic methods. In recent years, awake spine surgery with spinal anesthesia has gained attention as an alternative to g

Sedation depth during spinal anesthesia and survival in elderly patients undergoing hip fracture repair.

Low intraoperative Bispectral Index (BIS) values may be associated with increased mortality. In a previously reported trial to prevent delirium, we randomized patients undergoing hip fracture repair under spinal anesthesia to light (BIS >80) or deep (BIS approximately 50) sedation. We analyzed survival of patients in the original trial. Among all patients, mortality was equivalent across sedation groups. However, among patients with serious comorbidities (Charlson score >4), 1-year mortality was reduced in the light (22.2%) vs deep (43.6%) sedation group (hazard ratio [HR], 0.43; 95% confidenc
