Bradykinin-mediated angioedema (AE) is a rare disease characterized by recurrent cutaneous or mucosal angioedema. This hereditary or acquired disease is of rapid installation, non-pruritic, usually painless and can affect the face, lips, larynx, gastrointestinal tract or extremities. We report a case of acute dystonia of the face, jaw and tongue caused by metoclopramide and mimicking angioedema. The patient had attacks for several years before the correct diagnosis was made and we present the first ever published video footage of an attack. We report a 47-year-old male presenting with inferior ST-segment elevation myocardial infarction (STEMI) and a history of aspirin-induced angioedema. Despite the challenges of antithrombotic management in the context of aspirin allergy, the patient underwent successful primary percutaneous coronary intervention (PCI), followed by early in-hospital aspirin desensitization. This case highlights the feasibility and safety of early desensitization, allowing timely initiation of guideline-recommended dual antiplatelet therapy in a high risk setting.
