BACKGROUND: Having used the nonfreeze Barraquer-Krumeich-Swinger set for refractive lamellar keratoplasty since January 1987, the authors found the results of this procedure to be disappointing for correction of aphakia. Insufficient refractive gains for epikeratoplasty (+39% of initial refraction at 12-month follow up) persuaded the authors to look for a more effective refractive corneal surgery technique for correcting aphakia.

We reviewed the charts of 39 patients (45 eyes) with uveitis who underwent cataract extraction (intracapsular or extracapsular) with or without implantation of a posterior chamber intraocular lens (IOL) at the Royal Victoria Hospital, Montreal, between 1981 and 1990. The fusion amplitude of image pairs with aniseikonia was measured using the synoptophore. Aniseikonia was produced in five subjects by alteration of slide drawings. The results of a prospective clinical study of hyperopic epikeratophakia grafts for the visual rehabilitation of aphakic patients are presented. Visual acuity data are given for 21 aphakic patients who have been followed for up to six months postoperatively. Among the possible methods of correcting aphakia the secondary implantation of a posterior chamber lens is considered to be a satisfactory procedure. Indications for such operations are discussed and typical cases are presented. The results in a series of 30 secondary implantations are analyzed.

Forty-two aphakic eyes of 37 patients were fitted with oxygen-permeable silicone rubber corneal contact lenses. Thirty-four eyes had the lens after two to 11 months of continuous use. Visual results, comfort, and patient acceptance were good, but the deposition of mucus on the lenses remains a problem.

Patients have been very satisfied with these lenses (78% wearing at the end of six months), not the least those with reduced function of the macula. The problems of infection have been discussed, and apart from improvement in lens materials patient selection is of paramount importance. The problem of deposits is presented, but theri elimination will depend on more research into their nature and development, improvement of the material's resistance to this form of attack, and improved lens design to overcome corneal and tear flow problems.
