In the aphakic eye, with intact iris diaphragm, silicone oil has frequently caused a pupillary block. In this situation aqueous humour accumulates behind the iris and forces silicone oil through the pupil into the anterior chamber. An iridectomy at the 6 o'clock position can effectively prevent this pupillary block. The iridectomy allows free passage of aqueous to the anterior chamber which remains free of silicone oil. No permanent contact with silicone oil and the cornea is established, and development of keratopathy is prevented. The effect of this iridectomy in 62 eyes with intact iris dia

Seven children who had undergone removal of congenital cataracts were evaluated postoperatively for cystoid macular edema during outpatient visits. Fluorescein was administered orally in a fruit juice cocktail, and the eyes examined by angioscopy 10-20 minutes and 45-60 minutes after dye ingestion. No evidence of macular leakage was observed in any of the 11 eyes examined. It appears that this diagnostic procedure is feasible in young patients, and that the incidence of cystoid macular edema after congenital cataract extraction may be less than has been previously reported.

Fluorescein iris angiography and fluorophotometry were performed on a series of 9 patients with bilateral and 11 with unilateral pseudoexfoliation, 12 bilateral aphakes with pseudoexfoliation, and 7 unilateral aphakes with bilateral pseudoexfoliation. Angiography showed a loss of radial iris vessels, a heavy leak of fluorescein from the pupil margin, progressive neovascularisation of the outer 2/3 of the iris, and less constantly a network of fine new vessels in the inner 1/3 of the iris stroma. These changes were absent in unaffected eyes. After cataract extraction there seemed to be a defini

The perceptual variations due to removal of the crystalline lens and the employment of powerful plus spectacle lenses are described. These include refractive change, distortion, magnification, limitation of the visual field, peripheral vision, sensitivity of light and changes in the required convergence. The technique for satisfactory refraction of the aphakic eye is discussed and the elements leading to error such as vertex distance and pantoscopic tilt are considered. Other factors necessary for understanding the refraction such as the apparent accommodative range and the limitations affecti

We measured apparent accommodation in 42 pseudophakic eyes (34 patients) after implantation of posterior chamber intraocular lenses. The mean apparent accommodation was 2.03 +/- 1.03 diopters. The mean accommodative power of 16 phakic eyes used as controls was 2.91 +/- 1.29 diopters. The diameter of the pupil appeared to be the most important factor in apparent accommodation--the smaller the pupil, the greater the apparent accommodation. Apparent accommodation was inversely proportional to the pupillary diameter. There was no correlation, however, between apparent accommodation and corrected v

A change in the indications for cataract surgery might be one of the many reasons for the steady increasing number of cataract extractions. A comparison between the indications for surgery in a group of cataract patients from 1970 and a similar group from 1980 was done. In bilateral cataracts, evaluated on basis of visual acuity in both eyes, no widening of the indications for cataract surgery could be found. In monaphakics, going to be operated on the second eye, visual acuity in both eyes revealed no change, but we do operate more second eye cataracts now, 41% against 25% in 1970, which mean

We treated eight neonates who had total, monocular, congenital cataracts with surgery, occlusion, and contact lenses. Visual results in all eight patients were good. In five patients visual acuities improved to 6/9 (20/30) or better in the aphakic eye. In three patients visual acuities improved to 6/24 (20/80) or better. Problems with contact lenses probably accounted for the poorer results in two of these three patients. Binocularity was not demonstrated in any of our patients.

The critical period in which human visual acuity may be lost after visual deprivation can be deduced by studying children with amblyopia which follows uncomplicated unilateral cataract and the subsequent aphakic blur. The visual acuity at the first contact lens fitting increases linearly on a log scale as a function of the age at onset between 3 and 10 years. Correction before 4 months of age produces less visual loss but we cannot specify the function within this period. If the eye is deprived between 6 and 30 months of age finger counting is the best visual acuity achieved. Deprivation comme
