Pneumatosis intestinalis after laparoscopic appendectomy: case report and review of the literature.

Pneumatosis intestinalis (PI) is the presence of intraluminal gas within the wall of the intestine. As a marker for bowel injury owing to mucosal injury, PI may herald a severe underlying disease process in patients without a significant medical history. In other cases, PI is a benign process, and expectant management is appropriate. Here, we present the first reported case of pneumatosis associated with postoperative abscess after appendectomy and its successful management. Then, we describe the pathophysiology of pneumatosis and review the literature regarding its origin and management.

Delayed appendectomy in pediatric cancer patients with leukopenia is associated with worsened outcomes.

BACKGROUND: Acute appendicitis in leukopenic pediatric cancer patients is a clinical challenge. While the perceived risks of surgery in this population may delay immediate appendectomy, we sought to identify the consequences of delaying surgery based on the severity of leukopenia at diagnosis. We hypothesized that leukopenic cancer patients are more likely to experience delayed appendectomy and that delay would be associated with worsened outcomes. METHODS: Using ACS NSQIP-P data from 2016 to 2023, we identified pediatric oncology patients who underwent appendectomy for acute appendicitis. Le…

An Observational Review of Tonsillectomy and Appendectomy Procedures Conducted at a Tertiary Care Hospital.

BACKGROUND: Tonsillectomy and appendectomy are the most frequently performed surgical procedures in pediatric and adult populations. However, comprehensive data examining their occurrence within the same hospital setting remain sparse. This study aimed to characterize the demographic and clinical profiles of patients undergoing tonsillectomy and assess the frequency and outcomes of those who additionally underwent appendectomy. METHODS: A retrospective review was performed on 337 patients who underwent tonsillectomy at a tertiary care hospital from 2015 to 2017. Data collected included age, n…

Amyand's Hernia With Retractile Testis and a Normal Appendix: Is Appendectomy Truly Required?

An inguinal hernia is a common surgical condition in which abdominal contents protrude through the lower abdominal wall into the inguinal canal. The hernia sac can contain various structures, including the appendix, which, when present, is referred to as Amyand's hernia. The condition is rare, with a normal appendix found in less than 1% of inguinal hernias. This condition was first described by Claudius Amyand in 1735. The coexistence of the appendix and a testis (either undescended or retractile) within the hernia sac is a rare phenomenon, adding to the complexity of diagnosis and management

A Case of Appendiceal Endometriosis Diagnosed as Acute Appendicitis and Treated With Laparoscopic Appendectomy.

Appendiceal endometriosis is an uncommon manifestation of extrapelvic endometriosis that can clinically and radiologically mimic acute appendicitis. We report the case of a 41-year-old woman who presented with acute right lower quadrant abdominal pain and laboratory findings suggestive of appendicitis. Laparoscopic appendectomy was performed, and histopathology revealed endometrial glands and stroma within the muscularis propria and mesoappendiceal fat, consistent with appendiceal endometriosis. The postoperative course was uneventful, and the patient remained symptom-free at follow-up. This c…

Conversion Rates and Outcomes of Laparoscopic Appendectomy in Complicated Appendicitis: A Retrospective Study.

Background Laparoscopic appendectomy is a minimally invasive technique widely preferred for acute appendicitis due to less postoperative pain, faster recovery, and better cosmesis compared to open surgery. However, in complicated appendicitis, such as perforation, gangrene, abscess, or peritonitis-distorted anatomy, friable tissues, dense adhesions, and contamination can make the procedure technically demanding. These challenges may require conversion to open surgery, potentially increasing operative time, complications, and hospital stay. Objective To evaluate the conversion rate from laparos…

A Multicenter Collaborative Study on the Safety and Efficacy of Laparoscopic Appendectomy During Pregnancy.

AIM: Acute appendicitis in pregnant women requires accurate diagnosis and treatment because of potential pregnancy complications. Laparoscopic appendectomy (LA) has been associated with higher rates of fetal loss than open appendectomy (OA). Therefore, we conducted the first multicenter collaborative analysis in Japan to evaluate the safety and efficacy of LA during pregnancy. METHODS: We retrospectively reviewed 152 pregnant women who underwent appendectomy for acute appendicitis between 2012 and 2021 using data from the Japan Society of Laparoscopic Colorectal Surgery. The patients were div…

Outcomes of Daytime and Night-Time Appendectomies: A Systematic Review and Meta-Analysis.

BACKGROUND: Acute appendicitis is a common surgical emergency characterized by appendix inflammation. Surgery remains the gold standard for treatment with laparoscopy gaining in popularity. However, the optimal timing for appendectomy remains unclear. We are not aware of studies evaluating potential differences in clinical outcomes among appendectomies performed during the daytime and at night. METHODS: We followed the PRISMA guidelines and searched the ScienceDirect, Medline, and Google Scholar databases for studies published in English before June 2023. We included prospective and retrospec…

Incidental appendectomy in surgical treatment of ileocolic intussusception in children. Is it safe to perform?

BACKGROUND: Surgical treatment of ileo-colic intussusception (ICI) has been reported as the second cause of emergency laparotomy in children. The performance of incidental appendectomy after surgical reduction is currently controversial. The aim is to analyse the outcomes of performing incidental appendectomy after surgical ICI reduction with or without associated bowel resection. MATERIALS AND METHODS: A retrospective study was performed in patients with ICI episodes, who underwent surgical treatment in our institution between 2005-2019. Patients were divided in two groups according to the p…

A Case Report of Laparoscopic Left-Sided Appendectomy in a Child With Situs Inversus.

Acute appendicitis classically elicits right-sided abdominal pain. Abdominal pain in patients with situs inversus may create a diagnostic challenge. We share a case of a seven-year-old boy who presented to the emergency department with left lower quadrant abdominal pain and tenderness. A chest x-ray showed dextrocardia and an abdominal ultrasound confirmed left-sided appendicitis. Diagnostic laparoscopy and appendectomy were uneventful. Thorough peri-operative evaluation and planning are key to a successful outcome in this rare condition.

Are We Hitting the Target?: The Effectiveness of Distinct Antibiotic Regimens in Pediatric Appendectomies.

BACKGROUND: Currently, a range of different antibiotics are administered perioperatively during appendectomy surgery in the pediatric population. One reason for the lack of a uniform treatment regimen is the paucity of large studies on microbiologic data and susceptibility patterns. METHODS: A retrospective, monocentric study on antibiotic susceptibility, including 1258 pediatric patients undergoing appendectomy from 2012 to 2020 was performed. Predictors for prolonged length of stay (LOS) and complications were determined using regression analysis, and objective criteria for broad-spectrum a…

Appendectomy in patients with human immunodeficiency virus: Not as bad as we once thought.

BACKGROUND: The number of patients living with human immunodeficiency virus and acquired immunodeficiency syndrome is growing due to advances in antiretroviral therapy. Existing literature on appendectomy within this patient population has been limited by small sample sizes. Therefore, we used a large, multiyear, nationwide database to study this topic comprehensively. METHODS: Using the Nationwide Inpatient Sample, we identified 338,805 patients between 2005 and 2012 who underwent laparoscopic or open appendectomy for acute appendicitis. Interval appendectomies were excluded. We used multiva…

A Systematic Review of Perforated Appendicitis and Phlegmon: Interval Appendectomy or Wait-and-See?

Patients with appendiceal abscess or phlegmon have been traditionally managed with antibiotics and radiologically guided drainage of the abscess. Many studies have questioned the need for interval appendectomy. A systematic review of the nonsurgical treatment of patients with an appendiceal abscess or phlegmon was undertaken. The rate of recurrence after nonsurgical management, morbidity and length of hospital stay was measured. PubMed and Cochrane databases were queried to identify 21 studies reporting the morbidity of nonsurgical treatment of appendiceal phlegmon or abscess, and five studies…

Evaluation of the Effect of Reflexology on Pain Control and Analgesic Consumption After Appendectomy.

BACKGROUND AND OBJECTIVE: Appendicitis is the most common cause of severe abdominal pain in the world, and the associated postsurgical pain, as occurs with other surgical procedures, is one of the most common problems. Today, there is a growing tendency toward nondrug methods and alternative medicine to reduce the adverse effects of drugs. Reflexology involves applying pressure on certain areas of the palms, feet, and ears in order to reduce stress and pain in certain areas of the body. The aim of this study was to determine the effect of reflexology massage on pain relief after appendectomy. …
