Neural control of the canine pylorus.

After atropine administration the anesthetized dog exhibits significant spontaneous pyloric contractile activity and an inhibition-relaxation response upon electrical vagal stimulation. This inhibition-relaxation response was completely blocked by tetrodotoxin administration and by transection of the intraabdominal vagal nerves. In addition, tetrodotoxin administration caused a dramatic increase in spontaneous pyloric motor activity. The histaminergic receptor antagonists diphenhydramine and cimetidine did not block the inhibition-relaxation response. The octapeptide of cholecystokinin (CCK-OP

Autonomic dysreflexia after spinal cord injury: central mechanisms and strategies for prevention.

Spinal reflexes dominate cardiovascular control after spinal cord injury (SCI). These reflexes are no longer restrained by descending control and they can be impacted by degenerative and plastic changes within the injured cord. Autonomic dysreflexia is a condition of episodic hypertension that stems from spinal reflexes initiated by sensory input entering the spinal cord caudal to the site of injury. This hypertension greatly detracts from the quality of life for people with cord injury and can be life-threatening. Changes in the spinal cord contribute substantially to the development of this

[Effect of nifedipine on sinus node and atrioventricular node function following autonomic blockade in the human].

The effect of intravenously administered nifedipine (7.5 micrograms/kg) on sinus node and atrioventricular (AV) node function was evaluated in 17 patients, 9 of whom had sinus node disease after autonomic blockade (propranolol 0.2 mg/kg and atropine 0.04 mg/kg i.v.). The patients' ages ranged from 18 to 77 years. After nifedipine there was a nonsignificant increase in the median values for sinus cycle length from 750 ms to 760 ms, for sinus node recovery time from 1150 ms to 1240 ms, for corrected sinus node recovery time from 440 ms to 460 ms, and for sinoatrial conduction time from 63 ms to

A role for vagal activity in preventing the suppression of glucagon secretion by GLP-1 during hypoglycemia.

Glucagon-like peptide 1 (GLP-1) is generally safe against hypoglycemia, although it stimulates insulin and inhibits glucagon secretion. One explanation is that glucagon secretion is not inhibited by GLP-1 during hypoglycemia. We aimed at understanding the lack of suppression of glucagon secretion by GLP-1 by exploring the paracrine and neural regulation of pancreatic hormone secretion during hypoglycemia. Isolated rat pancreas (A) and an organ block comprising pancreas and stomach (B) were perfused. We performed 1) dose-response studies with GLP-1 (7-36) at hypoglycemia; 2) studies with GLP-1

Narcotics and local anesthetics.

The recognition and alleviation of animal pain is a growing veterinary and public concern. Pain can be of an acute or chronic nature with different behavioral manifestations. Physiologically, pain is a dynamic and complex phenomenon that produces changes in the central and autonomic nervous systems as well as in the endocrine system. Horses and other animals appear to possess an endogenous pain-suppressing system involving the brainstem and spinal cord. This system can modulate pain perception and the responses to it. The recently discovered endogenous opioid peptides (endorphins and enkephali

Double-blind randomized comparison of xylocaine and saline in paracervical block for diagnostic fractional curettage.

Comparative study of the level of the reported pain between patients who received xylocaine and normal saline for paracervical block during fractional curettage was carried out in 70 patients in a double blind randomized controlled trial. One group of patients received xylocaine for paracervical block just before the procedure was performed while the other group received normal saline in the same manner. Self-reported pain intensity using visual analog scale was assessed at four time points including the first time point when Allis tissue forceps was applied on the cervix, the second and third

Characterization of vagal input to the rat esophageal muscle.

There is recent morphological evidence for an interaction of autonomic nerve fibers and extrinsic motor nerves of the rat esophagus. The aim of the present study was to investigate a possible functional role of this autonomic innervation of vagal motor fibers on rat esophageal smooth and striated muscle function in vitro. The entire esophagus with both Nn vagi, including the Nn recurrentes, was dissected and placed in an organ bath with oxygenated Krebs-Ringer buffer. Contractile activity was measured in longitudinal direction with a force transducer. Both Nn vagi were placed on a bipolar plat

[Anatomic basis for the innervation of the male pelvis].

To reach an optimal outcome after radical prostatectomy or cystoprostatectomy, minimal bleeding from the dorsal vein complex, meticulous apical dissection without distraction of the rhabosphincter and preservation of the cavernous nerves are necessary.A total of 19 male cadavers were used, ranging in age from 52-78 years and including 8 fetuses. Fourteen complete pelves and five tissue blocks of prostate, rectum, membranous urethra and the rhabdosphincter were investigated. Besides anatomical preparations, the lower urinary tract was studied by means of serial histological sections. The cavern

The internal anal sphincter in the cat: a study of nervous mechanisms affecting tone and reflex activity.

1. Smooth muscle activities in rectum and internal anal sphincter have been recorded using intraluminal balloons.2. Reflex activation of the sphincter, caused by distension of the rectum, has been assessed before and after various combinations of blocking drugs.3. Responses to stimulation of hypogastric or sacral nerves, or to the administration of drugs with autonomic actions have been tested before and after various combinations of blocking drugs.4. Results indicate that the tone of the internal anal sphincter is influenced by a number of neural mechanisms. These include motor pathways invol

[Mitral valve prolapse. Atrial stimulation, ajmaline test and "pharmacological denervation" in the evaluation of ventricular repolarization].

In patients with mitral valve prolapse syndrome (MVP) various electrophysiological abnormalities occur. There are convergent opinions concerning QT distance variability and the influence of autonomic nervous system on ventricular repolarization in this syndrome. In 38 MVP patients (group I) and 24 subjects without this abnormality (group II) ecg was recorded during transvenous right atrial pacing at baseline, after ajmaline administration and after pharmacological autonomic blockade (atropine + propranolol). The following ventricular repolarization parameters were analysed: QTe (distance to th

Electrophysiological techniques for the study of urethral and vesical innervation.

A new electrophysiological technique for the study of urethral and vesical innervation is reported. The method gives valuable information on the structural integrity and function of two of the reflex arcs involved in micturition. Normal and pathological patterns are described, illustrating the diagnostic value of the method in the assessment of neurogenic disturbances of bladder and urethral function.

Detrusor bladder neck dyssynergia revisited.

To our knowledge, direct measurement of active closure of the bladder neck during bladder contraction resulting in bladder neck dyssynergia and outflow obstruction has not yet been demonstrated. A total of 34 spinal cord injury patients underwent urodynamic investigation with 2 micro-transducer catheters in the urethrovesical and anorectal regions, respectively. Proper localization of the transducers was done with an image intensifier. The respective role of the striated and smooth muscles on bladder neck activity was evaluated after pudendal nerve blocks and phentolamine injections. Of the pa

Comparative pharmacology of cisatracurium (51W89), atracurium, and five isomers in cats.

BACKGROUND: Atracurium has four chiral centers and the marketed product is a mixture of ten optical and geometric isomers. Six of the isomers were prepared and evaluated for neuromuscular blocking activity and autonomic effects in anesthetized cats. This study reports the comparative pharmacology of the six isomers and atracurium that led to the selection of one isomer, cisatracurium (Nimbex, 51W89) for clinical development. METHODS: Purpose bred cats, anesthetized with alpha-chloralose (80 mg/kg) and pentobarbital sodium (7 mg/kg) administered intraperitoneally, were used in this study. Neur

Effects of obturator and femoral nerve blocks on pain and kinematic gait parameters in patients undergoing unicompartmental knee arthroplasty: a retrospective cohort study.

BACKGROUND: Postoperative data indicate that 15-30% of patients undergoing unicompartmental knee arthroplasty (UKA) experience moderate-to-severe pain during the early recovery period, impeding rehabilitation. Due to the complex innervation in the knee, continuous femoral nerve block (FNB) is often administered but provides incomplete analgesia. Although the analgesic effects of nerve blocks are well studied, less is understood about their effects on postoperative rehabilitation and gait kinematics. Thus, in this study, we aimed to investigate the impact of ultrasound-guided obturator nerve bl

Possible Role of Fibrinaloid Microclots in Postural Orthostatic Tachycardia Syndrome (POTS): Focus on Long COVID.

Postural orthostatic tachycardia syndrome (POTS) is a common accompaniment of a variety of chronic, inflammatory diseases, including long COVID, as are small, insoluble, 'fibrinaloid' microclots. We here develop the argument, with accompanying evidence, that fibrinaloid microclots, through their ability to block the flow of blood through microcapillaries and thus cause tissue hypoxia, are not simply correlated with but in fact, by preceding it, may be a chief intermediary cause of POTS, in which tachycardia is simply the body's exaggerated 'physiological' response to hypoxia. Similar reasoning

Cardiac electrophysiology of patients with spinal cord injury.

Spinal cord injury is a clinical syndrome encountered frequently in trauma centers and is accompanied by both acute and chronic heart rhythm abnormalities. The injury is characterized by sympathetic nervous system impairment with preservation of parasympathetic output via the vagus nerve. Severe bradycardia in the form of life-threatening sinus arrest or complete heart block may be observed in the acute recovery phase. Therapy for arrhythmias in the acute phase includes atropine, intravenous chronotropes, methylxanthines for the prevention of episodic bradycardia, and pacemaker implantation in

The effect of preoperative stellate ganglion block on postoperative delirium in elderly patients undergoing gastrointestinal oncologic surgery: protocol for a randomized, double-blind, sham-controlled trial.

BACKGROUND: Postoperative delirium (POD) is a common and severe complication in elderly patients following gastrointestinal oncologic surgery, associated with prolonged hospitalization, increased morbidity and mortality, and substantial healthcare costs. The stellate ganglion block (SGB), a sympathetic nerve block with anti-inflammatory and autonomic-modulating effects, has shown promise in improving perioperative outcomes. However, high-quality randomized controlled trials evaluating its efficacy in preventing POD in this older population are lacking. METHODS: This is a single-center, prospe
