Polycystic Ovary Syndrome and Sex Hormones in Bariatric Surgery Candidates.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a prevalent metabolic and endocrine disorder affecting women of reproductive age, particularly those with obesity. This syndrome is characterized by clinical/biochemical hyperandrogenism and/or ovulatory dysfunction, which can result in infertility. PURPOSE: To investigate the prevalence of PCOS in candidates for bariatric surgery and to examine the correlation between PCOS and sex hormones in women with obesity. METHODS: A retrospective study analyzed 577 bariatric surgery candidates from 2015 to 2021 using RStudio. Normally distributed data w

Exploring patients' perceptions and anticipated concerns regarding bariatric surgery.

BACKGROUND: Bariatric surgery, particularly laparoscopic procedures, is becoming increasingly common for achieving sustained weight loss. However, the decision-making process is complex and influenced by psychological, social, and medical factors. Despite its growing relevance, there is a lack of research exploring patients' perceptions and expectations regarding bariatric surgery in China. This study aims to explore patients' perceptions, concerns, expectations, and hopes related to the outcomes of bariatric surgery. Additionally, it investigates patients' information received, education, and

Sinus node dysfunction following bariatric surgery: A case report.

Sinus node dysfunction is often due to degeneration of the heart's impulse-generating system. We report a case of a 40-year-old woman with a recent gastric sleeve surgery, who presented with symptomatic sinus bradycardia following significant post-surgical weight loss. Extensive workup excluded common causes such as electrolyte disturbances, hypothyroidism, or cardiomyopathy. We hypothesize that alterations in leptin metabolism due to weight loss triggered increased vagal stimulation, leading to her bradycardia. The patient was treated with theophylline as a noninvasive alternative to pacemake

Drug poisonings following bariatric surgery: Case series report.

Bariatric surgery alters gastrointestinal anatomy and physiology, which likely impacts upon oral medication absorption. Drug- and alcohol-related deaths in this population are being increasingly reported; however, toxicological detail is lacking. Using data reported to the National Programme on Substance Use Mortality, we identified 18 deaths in people who had previously undergone bariatric surgery. Opioids were detected in almost all cases and were frequently implicated in causing death. Multiple medications were detected at post-mortem in every case and often included medications that the de

Endoscopic closure techniques of bariatric surgery complications: a meta-analysis.

BACKGROUND: Leaks following bariatric surgery, while rare, are potentially fatal due to risk of peritonitis and sepsis. Anastomotic leaks and gastro-gastric fistulae following Roux-En-Y gastric bypass (RYGB) as well as staple line leaks after sleeve gastrectomy have historically been treated multimodally with surgical drainage, aggressive antibiotic therapy, and more recently, endoscopically. Endoscopic clipping using over-the-scope clips and endoscopic suturing are two of the most common approaches used to achieve full thickness closure. METHODS: A systematic literature search was performed

Sleep and aberrant eating behaviors in metabolic/bariatric presurgical candidates.

BACKGROUND: Metabolic/bariatric surgery (MBS) is the most effective treatment for obesity, yet many factors influence successful individual weight loss. Among those are a variety of health behaviors that are assessed in the process of presurgical psychological evaluations, including eating pathology and sleep disturbance (both of which are relatively common among surgical candidates). OBJECTIVES: This study aims to examine the relationship between sleep, binge eating, and night eating behaviors among individuals seeking MBS. SETTING: Medical center and private psychological practice in Missi

Metabolic and hepatic phenotypes in sarcopenic obesity and impact of bariatric surgery.

BACKGROUND & AIMS: Sarcopenic obesity (SO) is associated with cardiometabolic disorders and steatotic liver disease and carries major health risks. We assessed the hepatic and metabolic clinical phenotype associated with SO in patients with obesity undergoing bariatric surgery (BS). We also evaluated whether weight-loss and metabolic improvement post-surgery differ between patients with and without SO. METHODS: 972 consecutive patients from a single-center BS cohort who underwent whole-body dual-energy X-ray absorptiometry (DXA) and peri-operative liver biopsy were included. SO was diagnosed

Bariatric Surgery in Patients with Previous Cardiac Revascularization: Review of Literature.

Background: The diet and physical activity of the world's population determine the increase in the number of bariatric surgeries. The most common types of bariatric surgery are laparoscopic sleeve gastrectomy (LSG) and Roux-Y gastric bypass (RYGB). Surgical interventions are carried out in patients with numerous comorbidities, among which the most common are cardiovascular diseases. The aim of the present study was to review the literature regarding the safety and results of surgical treatment in patients with cardiac revascularization prior to surgery. Methods: We performed an online search i

Case report: Wernicke-Korsakoff syndrome after bariatric surgery.

Vitamin and mineral deficiencies are prevalent nutritional disorders following bariatric surgery. Although they are more prevalent after malabsorptive procedures such as bypass, they also occur in restrictive procedures such as gastric sleeve. The mechanisms that lead to the occurrence of these deficits are related to the presence of poor nutritional intake or poor adherence to multivitamins and multimineral supplementation. Wernicke-Korsakoff syndrome (WKS) is an acute neurological disorder resulting from thiamine deficiency. This syndrome is composed of two distinct phases: first, Wernicke E

[Wernicke encephalopathy : a neuro-ophthalmological side effect of bariatric surgery].

Due to an increase in the worldwide prevalence of obesity and the efficiency of bariatric surgery, this procedure is more often performed. Besides its benefits, it has also disadvantages and may be the cause of nutritional deficiencies. Thiamin deficiency is particularly important to diagnose and to treat early as it can lead to major sequelae and even to death. Wernicke's encephalopathy is the most frequent presentation associating confusion, ataxia, ophtalmoplegia and nystagmus. The full triad is not usually observed, which may lead to sub-diagnosis of this affection. The diagnosis is clinic

Liver health and the interplay between obesity, alcohol and bariatric surgery.

The prevalence of obesity and metabolic consequences, including non-alcoholic fatty liver disease (NAFLD) has become a global health problem. Obesity has an important impact on chronic liver disease even beyond NAFLD, as it accelerates the progression of alcohol liver disease. Conversely, even moderate alcohol use can affect NAFLD disease severity. Weight loss is the gold standard treatment but adherence to lifestyle changes is very low in the clinical setting. Bariatric surgery can improve metabolic components and cause long-term weight loss. Therefore, bariatric surgery could serve as an att

Bariatric surgery in a patient with cystic fibrosis and diabetes: A case report.

Cystic fibrosis (CF) is incurable and chronic, causing severe multisystemic damage and long-term complications. The most prominent extrapulmonary long-term complication is CF-related diabetes, which is the most reported form of diabetes in individuals with cystic fibrosis. Here we present the first case of an individual with cystic fibrosis who developed type 2 diabetes due to obesity rather than CF-related diabetes. The type 2 diabetes went into remission due to extreme weight loss after gastric bypass surgery. To our knowledge, this case is also the first report describing the effect of bari

Bariatric surgery and changes in metformin distribution: About a fatal case.

A 65 year-old woman, suffering from mellitus type 2 diabetes and obesity, died at home, three days after bariatric surgery (Roux-en-Y gastric by-pass: RYGB). Her treatment, including metformin and dapagliflozin, was stopped before surgery and not postoperatively reinstalled. A forensic autopsy, toxicological and histological analyses were performed. No macroscopic or microscopic evidence of digestive perforation or peritonitis was identified, excluding an early surgery complication as the cause of death. Toxicological analysis revealed the presence of Metformin in all matrices tested, with a p

Hepatic Glucagonoma in a Post-bariatric Female Patient: A Case Report.

Glucagonomas, neuroendocrine tumors originating from the pancreas marked by excessive glucagon secretion, present a diagnostic challenge due to their rarity and diverse symptomatology. In this report, we present a 47-year-old female with a history of bariatric surgery, diabetes mellitus, and deep vein thrombosis who exhibited weight loss, anemia, migratory necrolytic erythema on the lower limbs and groin, and fecal incontinence. Imaging revealed liver secondary lesions without an identifiable primary tumor. After undergoing surgery, a pathologic examination of the excised tissue confirmed that
