The treatment effects of acotiamide in esophagogastric outflow obstruction: a prospective longitudinal observational study.

OBJECTIVES: We have found that an altered lower esophageal sphincter (LES) accommodation response is an underlying cause of esophagogastric junction outflow obstruction (EGJOO). The objective of this study was to examine the treatment effect of acotiamide, a prokinetic agent which improves impaired gastric accommodation in functional dyspepsia, in patients with EGJOO. METHODS: A prospective observational longitudinal study was conducted between October 2014 and March 2020. Acotiamide (100 mg, 3 times a day) was administered to 25 patients with EGJOO for 4 weeks. High-resolution manometry (HRM

Current and emerging therapeutic options for the management of functional dyspepsia.

Introduction: Functional Dyspepsia (FD), defined as chronic symptoms originating from the gastroduodenal region in absence of readily identifiable organic disease, is one of the most common gastrointestinal disorders. FD is divided into two subgroups: Post-Prandial Distress Syndrome (PDS) or meal-related FD, characterized by postprandial fullness and early satiation, and Epigastric Pain Syndrome (EPS) or meal-unrelated FD, characterized by epigastric pain and burning.Areas covered: This review summarizes the existing and off-label therapeutic options for FD.Expert opinion: The identification o

The use of ethamivan in the treatment of barbiturate poisoning.

Ethamivan was used as a respiratory analeptic in the treatment of nine cases of severe barbiturate poisoning. Initial intravenous injections of 100 to 150 mg. of ethamivan increased the depth of respirations within a minute. Prolonged respiratory stimulation was achieved by a continuous intravenous infusion of 500 to 3000 mg. of ethamivan per litre of fluid. If hypotension occurred, an intravenous drip of noradrenaline was used; fluid overloading was avoided by adjusting the concentrations of drugs given, so that no more than a total of 125 c.c. of fluid per hour was administered. The chief si

Pioneering Novel, Green, White, and Blue Fluorescence-Based Platforms for Sustainable and Concurrent Monitoring of Ciprofloxacin With Celecoxib or Itopride in Biological Matrices.

Two innovative spectrofluorimetric techniques were developed for the first time to enable simultaneous quantification of ciprofloxacin hydrochloride in binary mixtures with either celecoxib or itopride hydrochloride in biological fluids. The first relied on synchronous spectrofluorimetry at a constant wavelength interval (Δλ = 100 nm), which effectively reduced spectral interference and allowed accurate estimation of celecoxib and ciprofloxacin hydrochloride at their zero-crossing points of 276 and 328 nm, respectively. The second technique leveraged direct spectrofluorimetric measurement of c

Efficient detection of aklomide/nitromide based on a sensor of MPA capped CdTe QDs using fluorescence quenching method.

Based on the fluorescence sensor of 3-Mercaptopropionic acid (MPA) capped CdTe quantum dots (QDs), two novel detection methods for aklomide and nitromide were developed. The MPA-CdTe QDs were synthesized and characterized by transmission electron microscopy (TEM), thermogravimetric analysis (TGA), X-ray diffraction (XRD), ultraviolet visible (UV-vis) and fluorescence (FL). The quenchings were all static. The binding constants (Ka) at different temperatures were obtained. Electrostatic forces were the main forces for the two bindings. For the detection of aklomide and nitromide, under the optim

Prolonged Mpox Disease in People With Advanced HIV: Characterization of Mpox Skin Lesions.

We report 3 complicated and prolonged cases of mpox in people with advanced human immunodeficiency virus (HIV) not on antiretroviral therapy (ART) at mpox diagnosis. Multiple medical countermeasures were used, including prolonged tecovirimat treatment and immune optimization with ART initiation. Immunofluorescence of skin biopsies demonstrated a dense immune infiltrate of predominantly myeloid and CD8+ T cells, with a strong type I interferon local response. RNAscope detected abundant replication of monkeypox virus (MPXV) in epithelial cells and dendritic cells. These data suggest that prolong

Continuous flow synthesis of the antiviral drug tecovirimat and related sp3-rich scaffolds.

Herein we report a 2-step continuous flow synthesis of the antiviral drug tecovirimat, which is used for the treatment of monkeypox and smallpox. This work exploits a high-temperature pericyclic cascade process between cycloheptatriene and maleic anhydride generating a key sp3-rich scaffold, which affords the desired API after further condensation with an acyl hydrazide. Additional investigations of the key intermediate in reactions with different hydrazines revealed the accessibility of different heterocyclic chemotypes, depending on the substitution pattern of the hydrazine used. Ultimately,

Age-related genes affecting the immune cell infiltration in ulcerative colitis revealed by weighted correlation network analysis and machine learning.

OBJECTIVE: The crosstalk between age and immunity in the context of ulcerative colitis (UC) remains incompletely understood. Our objective is to elucidate the specific age-associated genetic factors that modulate immune cell infiltration in UC, with the aim of identifying innovative therapeutic targets for the treatment of this disease. MATERIALS AND METHODS: Potential batch effects between samples were removed by R package "inSilicoMerging". Unsupervised clustering analysis via the "ConsensusClusterPlus" R package was utilized to perform consensus molecular subtyping of immune subtypes in UC

Human monkeypox: epidemiology, transmission, pathogenesis, immunology, diagnosis and therapeutics.

"Zoonoses" describe diseases that may be acquired by humans from animals. Due to the constant contact between humans and other animals, many infectious diseases are disseminated. This may happen via direct contact, such as bites or scratches, or by indirect contact, such as when eating bush meat or using contaminated animal parts. Monkeypox disease is one such zoonotic infection which is now emerging as a disease of global concern, and the World Health Organization has already labelled it a public health emergency. The virus is related to other orthopox viruses and may be further classified in

Potential therapeutic targets for Mpox: the evidence to date.

INTRODUCTION: The global Mpox (MPX) disease outbreak caused by the Mpox virus (MPXV) in 2022 alarmed the World Health Organization (WHO) and health regulation agencies of individual countries leading to the declaration of MPX as a Public Health Emergency. Owing to the genetic similarities between smallpox-causing poxvirus and MPXV, vaccine JYNNEOS, and anti-smallpox drugs Brincidofovir and Tecovirimat were granted emergency use authorization by the United States Food and Drug Administration. The WHO also included cidofovir, NIOCH-14, and other vaccines as treatment options. AREAS COVERED: Thi
